Appendix E <br />Required documents and records from Participating agency <br />1. On Agency letterhead, verification of "General Training Requirements" for rostered <br />personnel. <br />2. Certificates of completion for requirements listed in the "Position Description Directive" <br />dated 2008 -05, or any subsequent revisions of this document. <br />3. Participating Agencies that have an annual or bi annual medical screening program will <br />provide a fit for duty letter signed by the agencies medical service provider per FEMA <br />directive 2005. This is to include a respiratory questionnaire. <br />22 <br />
