PHSKC # <br />• Manage public information activities <br />• Provide laptop computers <br />• Administer data management activities <br />• Coordinate communication with city, county, state, and federal agencies, as needed <br />• Provide instructions for set up of facilities <br />• Provide all vaccines, supplies and equipment needed for operations <br />• Provide staffing for delivery of vaccinations and /or dispensing of medication <br />• Arrange for food service for staff and patients <br />• Dispose of biomedical waste <br />• Conduct post utilization decontamination <br />VI. Responsibilities of the City of Tukwila <br />• Cancel events scheduled for facilities <br />• Provide internet connectivity as identified in the Site Plans to the best of its ability <br />• Assist in set up of facilities based on instructions from PHSKC to the best of its ability <br />• Ensure availability of parking identified in the Site Plans <br />• Ensure availability of equipment identified in the Site Plans to the best of the City of Tukwila's <br />ability <br />• Arrange for maintenance of equipment identified in the Site Plans during response operations , if <br />needed <br />• Ensure continuation of janitorial services to the best of its ability <br />VII. Cost and Payment <br />The City of Tukwila shall provide the facilities without rental cost to PHSKC. PHSKC agrees to pay <br />the City of Tukwila for valid and invoiced "assistance costs," including direct material costs, <br />equipment rental fees, fuel, and labor costs incurred in providing the facilities. PHSKC also shall pay <br />the City of Tukwila for valid and invoiced costs of facility repairs resulting from PHSKC use of the <br />facility. Reusable materials and supplies which are returned to the City of Tukwila in clean, <br />damage -free condition shall not be charged to PHSKC. The City of Tukwila shall determine whether <br />items returned are clean and damage -free and the items shall be treated as non - returnable materials if <br />found to be damaged. The City of Tukwila shall send PHSKC an invoice or invoices for all valid <br />costs incurred and PHSKC shall pay the invoice(s) within sixty (60) days of receipt of each invoice. <br />PHSKC will provide instructions to the City of Tukwila regarding cost - tracking requirements and the <br />City of Tukwila agrees to track costs accordingly. <br />VIII. Term and Termination <br />This agreement is effective upon signature by both parties. Either PHSKC or the City of Tukwila may <br />terminate this agreement with written notification to the other party with no less than thirty (30) <br />calendar day's notification. <br />Agreement between Page 3 of 4 <br />PHSKC and City of Tukwila <br />
