City of Tukwila <br />6200 Southcenter Boulevard, Tukwila WA 98188 <br />Agreement Number: <br />CONTRACT FOR SERVICES <br />Amendment #2 <br />Between the City of Tukwila and David Evans and Associates, Inc. <br />That portion of Contract No. 16-063 between the City of Tukwila and David Evans and <br />Associates, Inc. is hereby amended as follows: <br />Section II Scope of Work: <br />Additional scope of Work is attached (see Exhibit A) <br />. Section III Duration of Agreement: <br />Contract duration is extended through 12/31/2019 <br />Section IV Payment: <br />Amendment # 2 provides additional payment in the amount of $ 124,581.00 which is for <br />additional GIS surveying of the Sanitary Sewer and Surface Water systems per David <br />Evans and Associates, Inc Scope of services dated March 12, 2018.
