Inspect and maintain this stabilization weekly and immediately before, during and following storms. 29: From May 1 through September 30, inspect and maintain temporary erosion prevention and sediment at least monthly. LLC PAGE Introduction The subject site is located on the southeast corner of the intersection between Andover Park East and Industry Drive at 515 Industry Drive in Tukwila, Washington. The site is irregular in shape and is currently occupied by three existing office buildings with associated parking areas and is called Andover Executive Park Buildings 1, 3, and 5. The King County parcel number for the property is 0223400070. The existing buildings will be demolished prior to site development. On October 14, 2008, Earth Solutions NW, LLC, (ESNW) performed a pre - demolition asbestos containing materials (ACMs) survey for the three existing office buildings for Doug Swan and Associates (Client). The survey was performed by an AHERA Building Inspector, (Certification #10302337, Expiration Date 09/24/2009). The scope of work included surveying onsite structures for the presence of ACMs. This resulted in a total of 24 samples taken from the subject property. This report describes the sampling method used, sample locations, observations and sampling results. ACM Survey Summary of Findings The Environmental Protection Agency (EPA) and the Occupational Safety and Health Administration (OSHA) have industry and construction standards which protect the environment and limit worker exposure levels for airbome asbestos: 40 Code of Federal Regulations (CFR) 763 identifies material containing asbestos as environmental and health hazards. These standards are identified in 40 CFR Part 763 subpart E, 40 CFR Part 763 Subpart G (EPA), 40 CFR Part 61 subpart M National Emissions Standards for Hazardous Air pollutants (NESHAP) and 29 CFR, Part 1910.1001 and Part 1926.1101 (OSHA). An ACM survey was completed by a certified ACM inspector to comply with these regulations. Possible ACMs were observed on the site, consisting of ceiling and floor tile thermal insulation and rooting materials. Samples removed from these locations were tested for asbestos. The test results indicate 22 samples are non - detect (ND) for asbestos. Samples from buildings 1 and 3 are ND for asbestos. Asbestos was detected in two samples from building 5. Those samples were from the mezzanine utility closet. The other samples from building 5 are ND for asbestos. ACM Survey Sampling Method. Locations and Site Materials with an asbestos content greater than 1 percent by weight are considered to be an ACM. ACM "friability" is defined by the EPA as a material that can be crumbled or reduced to powder using hand pressure. Friable ACM is thought to release fibers into the air more readily. However, many types of non - friable ACM can also release fibers if disturbed. While reasonable discovery efforts were used in obtaining possible ACM samples, some ACM may remain concealed and every square inch may not be exposed during a site survey. Normally, deep wall interiors, underground pipes, septic tank liners, and floor layers beneath surfaces that tile and carpet normally attach to, are examples of this. Possible ACM samples were obtained in accordance with EPA standards (40 CFR 763). An approximate one square inch area, full- thickness sample was removed and the sample immediately placed within an air -tight container and labeled with sample number, location, friability and other notable characteristics in order to complete sampling documentation. Chain of Custody /Analytical Report Request forms were filled out in triplicate. The forms included the sampling time and date, site designation, sample identification numbers, analysis required (Polarized Light Microscopy EPA/600 /R- 93/116) and authorized Chain of Custody signatures of site sampler and accredited laboratory personnel. According to the owner no known industrial or manufacturing sites have existed in the area. The interior of the office buildings were in fair to good condition. Insulation was visible to the inspector and samples were obtained. Analytical results were ND for asbestos in 22 of the 24 samples. No insulation around existing pipes was observed. The plumbing appeared to be updated as the inspector found no evidence of ACM. Tile, mastic, wall surfaces, vinyl binder, roofing exterior and interior were observed and sampled by the inspector. The samples are highlighted gray in the table. The table also indicates the other areas in buildings 1, 3, and 5 that were sampled do not contain materials containing greater than 1 percent asbestos. Survey observations indicate that those surface texture materials were friable in nature but ACMs were not detected. Conclusions and Recommendations This concludes the ACM survey completed for Andover Executive Park located at 515 Industry Drive, Tukwila, King County, Washington property. Within King County, demolition projects must submit a Notice of Intent" permit with the Puget Sound Clean Air Agency. When discharge samples exceed a benchmark, additional permit requirements must be followed. Discharges to Impaired Waterbodies (Special Condition S9, pages 18 -21 for additional information) • If your site discharges into a water body that is on the impaired waterbodies list (i.e., the "303(d)" list) for turbidity, fine sediment, high pH, or phosphorus, additional sampling is required. • EPA recently approved the 2004 303(d) list, which includes water bodies not previously listed. Ecology will be reviewing the newly approved list and will notify you if any additional sampling requirements apply to you. Permit Transfer • When you sell or transfer operational control of all, or a portion, of your site to one or more new operator(s), you must also transfer permit coverage. • To transfer permit coverage, submit a Transfer of Coverage form to Ecology. Notice of Termination (Special Condition S10, page 29 for additional information) • You may request termination (cancel) when the site has undergone final stabilization with permanent vegetation or equivalent measures that prevent erosion or when all unstabilized portions of the site have been sold. • To request termination of permit coverage, submit a Notice of Termination (NOT) to Ecology. If you do not submit an NOT, you will remain responsible for permit compliance and permit fees.
