You are hereby directed to make the herein described changes to the plans and specifications or do the following described work not
included in the plans and specifications on this contract:
NOTE: This change order is not effective until approved by the "Owner" and a notice to proceed is issued.
Conditions:
A. The following change, and work affected thereby, are subject to all contract stipulations and covenants;
B. The rights of the "Owner" are not prejudiced;
C. All claims against the "Owner" which are incidental to or as a consequence of this change are waived; and
D. The Contractor must submit all Field Overhead and Horne Office Overhead Rates for approval in advance of all change
orders.

DESCRIPTION OF WORK
The scope of work for this change order includes providing all supervision, labor, and equipment necessary to construct and install the
improvements shown on the referenced project plans. Work includes saw cutting and removing asphalt as required to install
approximately 43 linear feet of 6 -inch PVC pipe, one Type 1 catch basin, and performing patch -back. Scope also includes adjusting
existing structures to finished grade. Materials required to complete the storm drainage improvements per Tukwila standards, including
but not limited to a Type 1 catch basin, necessary risers, frame and grate, sand collars, PVC pipe, and grout, will be furnished by the
contractor. The work includes 100 percent import-export with new CSTC. Prior to final acceptance, the contractor shall water test
roadway and ramp flow to ensure no ponding. The contractor shall also perform utility locates prior to setting the new catch basin at no
additional cost to the contract, and shall not claim delays or request additional compensation for any underground utility conflicts within
the limits of the proposed drainage improvements. It is assumed by the owner that the contractor is aware of and has documented all
underground utilities in the vicinity of this work.

Provide proposal and time impact analysis, within ten (10) days or as stated above for the scope of work
described:
NOTE: This is NOT a change order and does NOT constitute approval or notice to proceed on the issue.
SCOPE:
Per RFI -08, NWC took an elevation shot on existing CB -1754 to the west of the bus stop and found the
rim is 33.02'. Elevation Point 39 at the bus stop is 33.01' meaning there is backfall from the CB.
Furthermore, the elevation of the existing asphalt south of Point 39 (behind the jersey barrier) at the
wedge curb is 32.97'. Given the curb in this area is depressed, there is not enough fall to "push" the water
from the east to the west past this area without creating puddling at the northwest corner of the bus pad.
This raised concerns that if the water is allowed to puddle in this area, it will eventually overflow the
asphalt wedge curb that parallels the jersey barrier and erodes the slope to the south.
To mitigate this issue, the city would like to install a Type -1 catch basin at this low point and run 6" PVC
(SDR -35) pipe back to existing CB -1754. Pipe shall be sloped to drain at a minimum of 0.5% with 3'
minimum cover. Please provide a cost estimate for the work described above and shown in the
attachments. Unit of measurement shall be Lump Sum.

MEASUREMENT AND PAYMENT:
The unit Contract price shall be full compensation for all material, tools & equipment, supplies, labor
necessary and incidental to complete the work.

The purpose of this action request is to provide a lump sum price for RFP -01.
NWC is proposing to complete the work provided in the RFP -01 plan set for $23,698.00
Scope of Work
Our scope of work for this proposal includes the supervision, labor and equipment necessary to construct and install per
the above referenced project plans, including sawcutting, removal of asphalt needed to install approx. 43 LF of 6" PVC,
one type 1 catch basin, patch back, and readjusting structures to grade. Our proposal will be based on NWC being able to
perform continuous uninterrupted work, 8 hours per day, 5 days per week and includes 1 (one) mobilization per alignment
only. Overtime hours will be worked when directed by the client, if the client agrees in writing to reimburse NWC overtime
labor costs.
Specific Exclusions and Clarifications
1. Use tax included.
2. All permits, fees, bonds, and assessments by others.
3. All engineering, staking (Per NWC standard request), soils testing, compaction testing, soil inspection and
survey are by others.
4. NWC has figured normal daytime working hours, Monday -Friday only.
5. No hazardous or contaminated material removal, soil removal, water, monitoring, training, AHERA inspection,
testing, government reporting, abatement, or survey is included in this price.
6. Current traffic control plan to still be useable.
7. The disposal and/or replacement of unsuitable excavation material is not included in the base bid price.
Please respond to this in a timely manner and let us know how the city would like to proceed to
with our lump sum price. We would also request the City to add 3 working days to the contract
schedule to complete this work. If you would like to discuss this matter in more detail, I can be
