7. Limits of clearing /grading with existing and proposed topography at 2' intervals extending 5' beyond property's
boundaries
8. Identify location of sensitive area slopes 20% or greater, wetlands, watercourses and their buffers (change of
use only)
9. Identify location and size of existing trees that are located in sensitive areas and buffer (TMC 18.45.040), of
those, identify by size and species which are to be removed and saved
10. Landscape plan with irrigation and existing trees to be saved by size and species (exterior changes or change
of use only)
11. Location and gross floor area of existing structure with dimensions and setback
12. Lowest finished floor elevation (if in flood control zone)
13. See Public Works Checklist for detailed civil /site plan information required for Public Works Review (Form H-
9).

❑ ❑ Copy of Washington State Department of Labor and Industries Valid Contractor's License. -ii no
contractor has been selected at time of application a copy of this license will be required before the
permit is issued OR submit Form H -4, "Affidavit in Lieu of Contractor Registratior.r.

Dear Mr. Chin:
At your request, we have conducted a subsurface soil investiga-
tion at the above referenced site. The following report presents
our observations, conclusions and recommendations.
. PROJECT DESCRIPTION
We understand that the proposed project will consist of three (3)
large concrete tilt -up, office - warehouse buildings with concrete
slab -_o. -grade floors. A site plan dated 6 -14 -88 and elevations
dated 6 -15 -88 (P.D.A. #88 -5047) were provided for our use. We
understand that the. Healthco and Pacific Western Agencies buildings
will utilize a ramp down to a dock -high loading area. We under-
stand that the warehouse portion of the Pacific Western Agencies
Building will have floor loads of approximately 600 p.s.f. A
covered storage area is planned adjacent to the north side of the
Pacific Western Building. The storage area will have floor loads
of about 600 p.s.f. also. The floor loads of the Healthco Building
will be 150 p.s.f. As we understand it, a total of 10 to 20 heavy
trucks will visit the two buildings each day. The O'Sullivan
building will be used as an office and a heavy equipment shop.

SCOPE
The scope of our study was to test the subsurface soils of the site
and to present conclusions and recommendations regarding foundation
design and placement, floor support, drainage, fills and pavements.
Our field work included a visual reconnaissance of the site, the
excavation of six (6) backhoe test pits and the drilling of three
(3) test borings. The test pits and test borings were located in
reference to the proposed building locations shown on the site plan
which you provided. The test locations were limited by the single
family residence in the northeast portion of the site. In addition
to our field work we have reviewed a previous soil report by our
firm dated June 3, 1985 which was done for a different project
proposed on a portion of the O'Sullivan site. The previous site
work included three (3) test borings and fifteen (15) test pits.
The test pit and test boring locations are shown on the site Flan
presented in Appendix A. The test pit logs, test boring logs and
detailed soil descriptions are presented in Appendix B. All eleva-
tions mentioned in this report are referenced to the existing
grades at the time of our site visit.
SITE DESCRIPTION
The site is located in Tukwila, Washington, northwest of the South
134th Street, 47th Avenue South intersection. The majority of the
site is relatively flat except for the far east side which slopes
gently up to 47th Avenue South.
Two (2) old abandoned buildings are located in the west half of the
site. The larger structure is an old, wood frame warehouse with a

dirt floor. The smaller wood frame structure appears to have
been used as an office. The area east of the existing wood
structures appears to have been used as a parking and driveway
area. A large, concrete tilt -up building (Far West Paints) is
located adjacent to the west property boundary. As we understand
it, the adjacent building area had been filled and pre - loaded. A
small asphalt paved loading dock is located on the east side of the
old parking area.
The eastern portion of the site is slightly higher in elevation
than the rest of the site. The east side is vegetated with thick
grasses and small trees. An existing single family residence is
located in the northeast corner of the site.
SUBSURFACE CONDITIONS
The site lies within an area that has been geologically mapped as
recent alluvium deposits underlain by overconsolidated glacial till
as described on the Preliminary Geologic Map of Seattle and Vicin-
ity. The soils observed throughout the site appear consistent with
this classification.
The test pits and test borings showed that most of the southwest
half of the site has been filled. The uncontrolled fill varied in
thickness from two (2) to four (4) feet. The test pits and test
borings in the northern half of the site showed organic silts at
the surface. The organic silt unit contained occasional layers of
fibrous peat. The thickness of the organic silt unit varied from
fourteen (14) feet in test pit 1, to three and one -half (3 1/2)
feet in test pit 8. Fill material was found overlying the soft
organic silts in test pits 9, 10 and in test pits 16 through 20.

Below the above described materials a five (5) to eleven and
one -half (11 1/2) foot section of very soft clayey silt to silt
with interbedded layers of peat was found and described in test
pits 2 through 11, test pits 16 through 19 and test borings 1
through 3. Just beneath these materials, a loose, fine to
medium - grained sand with occasional red flecks was encountered.
This sand represents the basal Duwamish River Alluvium deposit
found on the site. Most of the test pits were terminated within
this sand. The sand was found between thirteen and one -half (13
1/2) and fifteen (15) feet below the existing surface elevation.
The sand was between one (1) and five (5) feet thick.
-The six (6) test borings drilled during our site investigation
extend through the Duwamish River sand into a layer of glacial till
which varied from four and one -half (4 1/2) to eighteen (18) feet
in thickness. Test borings 2, 4, 5 and 6 were terminated in the
till.
In test borings 1 and 3 the holes extended through the glacial till
into a fine -to coarse - grained sand. These test borings were
terminated in this dense sand. This sand has been interpreted as
being part of the Older Sand Formation.
Seismic Stability

The seismic stability of the alluvial soils is poor. Moderate
seismic events may cause the soils to liquefy and /or consolidate
under load.

GROUND WATER
Ground water seepage was encountered throughout the stratigraphic
section of this site. All materials sampled above the glacial till
were moist to saturated. Most of the materials examined and
sampled during our site investigation are moisture sensitive and
drain poorly.

LABORATORY TESTING
Several tests were performed on soil samples in our laboratory.
The soil samples were obtained from test borings 4 and 5. The
laboratory testing included twelve (12) moisture content tests, one
(1) sieve test, two (2) tests for percent passing #200 sieve, and
two (2) Atterburg Limits. Laboratory testing was also performed
for our previous study.
The results of the current laboratory tests are presented in Appen-
CONCLUSIONS AND RECOMMENDATIONS
This site has very poor soils and will be difficult and ex- pensive
to develop. The proposed development is feasible if all our
recommendations are closely followed.

We understand through conversations with you that the maximum tol-
erable settlements are one - fourth (1/4) inch in ten (10) feet dif-
ferential, and three - fourths (3/4) inch across the buildings, ex-
cept the covered storage, where one (1) inch of differential
settlement can be tolerated. The surficial alluvium deposits ob-
served throughout the site would undergo very large settlement if
buildings were placed directly on them.

Pre - loading the building areas does not appear feasible for any
of the proposed buildings. The proximity of the Far West Paints
building would not allow an adjacent pre -load; the existing build-
ing would be badly damaged. The very high warehouse loads combined
with the light office loads in the Pacific Western Agencies build-
ing would result in long term differential settlement of several
inches, even if the site is preloaded The large difference in
thickness of organic soils under the O'Sullivan building would also
cause substantial, long term differential settlement.
We recommend that all building and floor loads be supported by
reinforced auger cast concrete piles. Treated timber piles are not
recommended for this project. Vibrations caused by pile driving
may cause settlements to occur within the neighboring Far West
Paints building.
Seasonal Working Conditions
The surficial natural and fill materials encountered on this site
are moisture sensitive. We do not recommend initiating site devel-
opment during wet weather conditions. Additional site develop-
mental costs could be incurred during wet weather conditions and
greatly increased difficulty would be experienced.
Site Preparation and General Design Considerations
Placing fill over the soft silt, clay and peat will cause
settlement of these soils. It should be recognized that settlement
of parking areas and sidewalks adjacent to buildings on piling will
be noticeable, so provision for utility connections, sidewalks or
other features to accomodate settlement should be made. Generally
settlement will be proportionate to the fill thickness, so large
fills will cause much more noticeable settlement.

Raising grades has the advantage of reducing cuts into the soft
wet subgrade. Thicker fills will improve the pavement subgrade.
Leaving the sod in place would make it easier to stabilize the
parking area subgrades. We should be asked to review and comment
on the proposed grading plan for the site when the plan is develop -
ed.

We recommend that trees, brush and all existing buildings be
removed from the site. Any subsurface structures that may be
encountered during site preparation should be removed and backfill-
ed with imported fill that meets the following gradation.

The fill should be placed in twelve (12) inch loose lifts and com-
pacted to 90% of the ASTM D -1557 maximum dry density value. Due to
the sensitivity of the surficial materials found in the northern
and central portion of the site, we recommend the use of wide track
equipment and "arms length" excavation techniques. Special consid-
erations may be required for equipment access and loading and un-
loading of trucks over the soft and yielding surficial soils.
Fourteen (14) inch auger cast piles which penetrate a minimum of
five (5) feet into the deep, medium dense to dense, silty sand with
gravel unit and are a minimum of ten (10) feet long will support

safe net working loads of twenty (20) tons each. Higher working
loads per pile may be achieved with deeper penetration into the
dense material. We anticipate that the auger cast piling lengths
will vary from a minimum of ten (10) feet to approximately forty
(40) feet. The loads above are adjusted for down drag forces
imposed by consolidation of the upper soils. They should be
reviewed after grading and construction plans are prepared.
It is very important that we observe the pile installation to
confirm that adequate bearing is reached. We recommend that a
minimum grout strength of 3000 p.s.i. is utilized to help insure
pile quality. All piles should be reinforced as specified by the
structural engineer.
Slabs on Grade
We recommend placing a minimum of four (4) inches of free draining
material below the slab floor to act as a capillary break. The
free draining material could consist of a clean sand or crushed
rock that has less than 5% material passing the #200 sieve, based
on the fraction passing the #4 sieve. We recommend placing a vapor
barrier between the slab and the material used for the capillary
break. If desired, one (1) to two (2) inches of sand may be used
on top of the vapor harrier to assist in concrete curing.
Paved Areas
We recommend that all work in paved areas take place during dry
weather. Due to the moisture sensitive nature of the on -site
soils and the highly variable nature of the fill and native soils,
we recommend that all work in paved areas be observed by a repre-
sentative of Cascade Geotechnical so that additional or alternate

recommendations can be made. A minimum of about two (2) feet of
gravel will be required to stabilize the subgrade. In many areas
the existing fill is stable and may not require additional fill.
The fill should conform to the specifications in the Site Prepara-
tion section of this report. Fabric should be considered to help
stabilize the softest areas, especially if the grass is excavated.
The variability and consolidation characteristics of the alluvial
soils found throughout the site will affect the durability of the
pavement on this site. Some settlement and resulting pavement
repair should •be _expected. Therefore, we recommend the use of a
temporary pavement section which can be easily and inexpensively
repaired for one (1) to two (2) years. After this time period, a
conventional pavement surface would be placed.
We recommend that the temporary pavement section consist of a mini-
mum of twelve (1 2) inches of 1 1/4 inch -minus crushed rock with a
chip seal surface placed over a stable gravel subgrade. Prior to
the placement of the crushed rock, we recommend that all proposed
paved areas be extensively proof rolled with a loaded dump truck to
reveal any soft areas. All soft areas should be sub - excavated as
determined by the Cascade Geotechnical representative and replaced
with a geotextile fabric and 1 1/4 inch minus crushed rock.
Stabilizing the subgrade will require a substantial amount of field
engineering,.due to the highly varied conditions.
We recommend that the permanent surfacing consist of a minimum of
three (3) inches of Class B Asphaltic Concrete meeting WSDOT
specifications.

The loading dock ramps will almost certainly require excavation
into the soft native soil. We believe fabric and about three (3)
feet of crushed rock will be required to provide a stable subgrade
in this area.
Utility lines which are small and shallow may be .supported on fill,
but some settlement should be anticipated. Deep or settlement sen-
sitive utility lines may require pile support. Utilities should
be backfilled in accordance with the fill specifications contained
in the Site Preparation section of this report.
We recommend that all roof downspouts be tightlined away from the
building to a suitable drain outlet. Perimeter drains, bedded and
backfilled with pea gravel should be placed around all buildings.
It is especially important that we be engaged to review the plans
when they become available. We should also be engaged to work with
your design professionals in the development of these plans. We
recommend that we be engaged to observe site preparation, the
placement of auger cast piles, fills and the construction of the
paved areas.

PILE FOUNDATION NOTES
1. AUGERCAST PILES shall be installed by drilling to the required
depth with a continuous - flight, hollow -stem auger, then cement -sand.
grout shall be pumped under pressure through the hollow auger as
the auger is withdrawn, resulting in a cast -in -place pile. The
contractor shall provide a pressure gauge in the grout line between
the pump and the auger and a means of measuring the quantity of
grout used in each pile. Immediately following withdrawal of the
auger, reinforcing steel shall be placed into the fresh grout column.
See drawings and details for pile size and reinforcing.
2. THE DESIGN PENETRATION DEPTHS, pile grouting and pile reinforcing
shall be verified in the field by a qualified inspector who can
evaluate the contractor's operation and collect and interpret
installation data.
3. PILES to be 14" diameter, F'c = 4000 psi
Vertical capacity = 40 k, Lateral capacity = 10 k

PILE FOUNDATION NOTES
1. AUGERCAST PILES shall be installed by drilling to the required
depth with a.continuous- flight, hollow -stem auger, then cement -sand
grout shall be pumped under pressure through the hollow auger as
the auger is withdrawn, resulting in a cast - in -place pile. The
contractor shall provide a pressure gauge in the grout line between
the pump and the auger and a means of measuring the quantity of
grout used in each pile. Immediately following withdrawal of the
auger, reinforcing steel shall be placed into the fresh grout column.
See drawings and details for pile size and reinforcing.
2. THE DESIGN PENETRATION DEPTHS, pile grouting and pile reinforcing
shall be verified in the field by a qualified inspector who can
evaluate the contractor's operation and collect and interpret
installation data.

The building located at 4650 S 134 PL in Tukwila is currently used to manufacture
sheet metal duct. We have a special permit issued by the Tukwila Fire Department to
use, store, or handle flammable /combustible liquids and to conduct hot works operations
(welding) in that portion of the building. A current inspection by the Tukwila Fire
Department was conducted 30 days ago on August 27 with no violations. We have
included a copy of the follow -up complementary letter from the fire department for our
regard to fire safety.
There will be no use, storage or handling of flammable materials in the portion of the
building designated for new office space.

Re: Permit application to add cement slab to Shamrock Bldg C fabrication shop.
Holaday -Parks is in the process of re- working its fabrication shop layout to promote a
more efficient flow of materials through the fabrication areas. In order to accomplish
this, we will be relocating several pieces of equipment. One of these pieces is our metal
sheer machine. This machine weighs 12,500 pounds and is currently anchored to a
reinforced cement slab on the East Side of the floor plan. We want to move it to the
south side of the fabrication shop and will need to duplicate the support for this
machine. Hence, the need for new cement slab for structural support under this machine
in anew location:
Also, note that the letter from Robert Fossatti Associates describes Items 1. & 2
(concrete slabs 1 and 2) and item one is referenced above. The other slab is for out door

Enclosed are structural details SCD - 1 to SCD - 7 and structural calculations for the proposed
equipment relocation at Building C.
Sketch SCD -1 is an exterior 10" slab for coil storage. You indicated it would be located near the :SE
corner of the building.
Sketch SCD - 2A to_SCD - 6 are the plans and details at the isolation slab required for the relocated
shear. The shear is to be located either side of Grid 1', south of Grid B. You will notice I have a total
of 6 new augercast piers. Four piers support the 18" isolation slab and two piers support the existing
slab edge at the cut. I am requiring the reinforcement all around the new opening at the existing slab
as well as connection to the existing slab rebar in the north -south direction with couplers. The aim is
to support the slab edges around the cut in the existing slab.
SCD -7 is a plan at the Duct -o -matic 11 Coil. The Coil is to be relocated along the south wall
of the bu •'1 : . est of Grid 3 between Grids A and B. I have reviewed the existing slab and the floor
slab and piers shoul • . - • equate without any additional reinforcment: However, it is my
understanding that the portion • machine supporting the coils is supported by two curbs along the
north and south edges of the machine. i ' • ads the existing slab in an undesirable manner. The
general machine does not overload the floor slab, . . - er, 'the section of the machine that supports
the coils is very heavy and the floor slab will be overloaded I - oad is not distributed. I recommend
adding north -south curbs at the three locations indicated on SCD - 7 ead the load to the column
strips of the existing slab. This should reduce the possibility of slab distress. Fro r conversation, 1

crane wheels are whiling allowable loads for the existing slab and should not cauSC slab failure.
This review of the existing slab and piers was done with the available information including some
existing drawings and the original soils report. There were some assumptions required because of
lack of information. These assumptions were conservative and should not result in any surprises.
It should be expected that some floor slab cracking may occur because of a change in the floor
loading; We do not expect the integrity of the slab and foundation system to be affected.
Evaluating changes in loads in an existing structure with a structural floor slab and pier system is
not an exact science and it is possible some of the existing reinforcment was not placed exactly
according to plan. If any cracks or excessive deflections occur, please contact our office for
further review.
I reviewed the soils report, and the report specifically recommended augercast piles and
discouraged driven piles. The concern regards vibration. Although we are coming off the dry
time of year, these soils are highly susceptible to liquefaction and the vibration from pounding was
considered high risk. The report only gave allowable capacities for 14" diameter augercast, so a
new soils investigation would be required if any alternate piles are used. I assume that you do not
want to spend the time and money to go in that direction.

Re: Permit application to add cement slab to Shamrock Bldg C fabrication shop.
Holaday -Parks is in the process of re- working its fabrication shop layout to promote a
more efficient flow of materials through the fabrication areas. In order to accomplish
this, we will be relocating several pieces of equipment. One of these pieces is our metal
sheer machine. This machine weighs 12,500 pounds and is currently anchored to a
reinforced cement slab on the East Side of the floor plan. We want to move it to the
south side of the fabrication shop and will need to duplicate the support for this
machine. Hence, the need for new cement slab for structural support under this machine
in a new location.
Also, note that the letter from Robert Fossatti Associates describes Items 1 & 2
(concrete slabs 1 and 2) and item one is referenced above. The other slab is for out door

The building located at 4650 S 134 PL in Tukwila is currently used to manufacture
sheet metal duct. We have a special permit issued by the Tukwila Fire Department to
use, store, or handle flammable /combustible liquids and to conduct hot works operations
(welding) in that portion of the building. A current inspection by the Tukwila Fire
Department was conducted 30 days ago on August 27 with no violations. We have
included a copy of' the follow -up complementary letter from the fire department for our
regard to fire safety.
There will be no use, storage or handling of flammable materials in the portion of the
building designated for new office space.

Dear Mr. Parks:
You are to be congratulated on the relatively good
condition of your facility in regard to fire safety.
However, the following permits do need to be renewed at
this time.
Fire department special permits are required to be renewed
every 18 months. The following permits are required to be
renewed. Failure to return the application (green form
attached to this letter) with the appropriate fee within 30
days will result in a $40.00 reinspection fee.
1. A permit is required to conduct hot work operations.
(UFC 105.8 -h.3)
2. A permit is required for the use, storage, handling or
sale of flammable or combustible liquids. (UFC 105.8- F.3,
article 79)
Please return the permit application with the appropriate
fee within 30 days. Failure to return the permit
application with the appropriate fee will result in an
additional fee of $40.00.
If additional time is needed to correct the violations
identified above, a written request for a time extension
must be sent to the fire marshal.
Any overlooked hazardous condition and /or violation of the
