The approximate STA for the Nelson property is 148 +00. The toe of the slope was excavated and ecology blocks were installed as a retaining wall. This created a level area for the installation of the Hesco units. A total of 130' of ecology block retaining wall was installed. This work included the clearing and grubbing, excavation, procurement and placement of the ecology blocks, and the 1 additional night of traffic control for the installation of this work. This work shall be paid under the new item 109 "Ecology block foundation for Hesco Units on a lump sum basis. This work shall be full payment for all labor, equipment, and materials for the work described. One (1) additional day is granted for this change. Please read carefully for Point #6: 1. I discovered that all IDRs used to support these change orders, were not signed. Please sign all of them. 2. Please move the "Change Order Narrative" to the front page "Change" section 3. Please show the River station for the Nelson property. 4. In Field Directive #8, fill in the "Contract No. sign the "City Construction Inspector" spot, and date it. 5. Please use 29% 21% for the Independent Cost Estimate labor, equipment material mark- ups. And please show that KBA did the Independent Cost Estimate. The Independent Cost Estimate ($5,346.65) is not even half of this change order cost $11,950). Even after adding back the TCS flagger wage rates, and correcting all the mark- up %s, I still only came up with $6,650.71. Therefore, the $11,950 is not justified. The Contractor will either need to justify the cost, or come down to the cost quite close to 650.71. Please see what you can do about this. As of this point, I won't approve this change order amount. 7. In the IDRs, please circle out the relevant sections. In five locations on the east side of the river, 6 -inch ductile iron drain lines were installed under the Hesco barrier to allow for surface water to drain and eliminate ponding in normal rain events. The five locations are as follows: 1. NC Machinery Sales yard approximate STA 134 +00 2. NC Machinery railroad bridge approximate STA 136 +00 3. NC Machinery rental yard approximate STA 140 +00 4. Highway 181 guard rail removal area approximate STA 144 +00 5. Highway 181 guard rail removal area approximate STA 145 +50 Flap valves were installed on the outfall end of all of the drain pipes. The flap valves allow drainage during normal rain events and close to block backflow in the event the river rises to the pipe elevation. In total, there was one additional day of time impact to the project schedule. This work shall be paid under the new item 108 "Install drain lines under Hesco units on a lump sum basis. This work shall be full payment for all labor, equipment, and materials for the work described. After the top course was filled on a 40' section the bottom row bowed out /bulged on the river side of the units. This caused the top row to lean out and welds on one of the 3' panels in the bottom row broke. A large pond of water was trapped on the east side (road) of the Hesco units from the rain. The bottom course of the Hesco units has settled into the subgrade 2 -3" in the failure area. In addition the bottom of the units had not been pulled out as much as necessary during the filling operation to create the convex footing shape recommended by the manufacturer. Lane Shinnick (GBI) proposed removing the sand from the existing top course, setting a third row on the bottom course on the road side and installing new top course on the middle row of the bottom course. This would create a pyramid configuration in this area. Dennis said that would be a good repair. GBI will work on this fix starting on Monday. 3:OOpm Daniel Clark and I observed the water level in the pond behind Homestead Suites and on the river at the Best Western. The installation started at the south end of the fire lane at Highway 181. 11:30am Met Randy Dove and Andrew O'Conner (GBI) at the railroad bridge. Andrew shot the elevation of the trail grade on the west side approximately 100' north and 100' south of the bridge with the GPS rover. He added 4' to these shots and calculated the slope between. He then shot the center of the tracks and calculated the cut/fill to the trail grade +4' line. The center of the tracks on the west side showed a fill of 1' to reach the trail +4' elevation. Using the same shots on the trail He recalculated the fill for trail grade +2' and shot the east side of the tracks. The east side showed a cut of 1' at the track grade. Change Order Narrative Install barrier in front of Hesco units on Highway 181 CITY OF TUKWILA CONTRACT CHANGE ORDER NO. 07 Sheet 1 of 2 09 -164 (g) Approved By Council Res. #1689 &1697 Description and Justification for Change The Hesco units were installed either in front of, or in place of the existing guardrail on Highway 181. WSDOT has determined that the Hesco barriers to be a roadside hazard within the clear zone, and requires protection by means of a proven system. WSDOT was not satisfied with the crash tests performed on the Hesco units, and believed that the Hesco units were inadequate for the glancing impact, which is most likely to occur with this condition. Eventually, WSDOT approved the installation of concrete barrier in front of the Hesco units. The barrier for the section from NC Machinery to S. 180 Street was provided by WSDOT, at no cost to the City. The Contractor loaded the barrier sections at a WSDOT storage location, transported them to the site and installed them. Additional pins had to be purchased by the contractor for the WSDOT provided barriers. The space between the Hesco units and the fog line was too narrow between Barnaby's (approximate STA 147 +00) and NC Machinery approximate STA 143 +50) for the standard barrier (2' wide) that was available from WSDOT. The contractor purchased and installed narrower type 4 barrier (1' wide) for this section. An impact attenuator was required for the type 4 barrier at the private residence driveway at approximate STA 148 +00. Actually, it was more cost effective to purchase the type 4 barrier than rent it for 2 to 5 years. Overall, a total of 2,312 LF of WSDOT provided barrier and 556 LF of the type 4 barrier were installed. The Contract spent 2 working days clearing the blackberry vines and other vegetation at the WSDOT storage site for the concrete barrier, to facilitate loading the barrier for transport to the project site. Additional traffic control was also required for the 2 days to set the concrete barrier. The reduced shy distance is acceptable to WSDOT. 2. We will need an impact attenuator at the south edge of the driveway opening, and a flared section on the north. The State did not accept the flare at this location as we were unable to get the barrier outside the guardrail. Please propose an impact attenuator that will fit the field conditions at this point. For the concrete barrier placement south, where we had discussed flaring the barrier, that will work, if we flare at a rate of 10:1, and have the end of the barrier a minimum distance of 15' from the fog line. This complies with the WSDOT design manual (Section 700). Thank you for preserving two lanes of traffic in each direction while using positive protection for the HESCO Units. For the second item, how to protect the ends of the concrete barrier at the private driveway, the seven foot terminal end section works since it is on the trailing end of the barrier that is outside of the clear zone for the opposing traffic. However, the flare offset for the three foot shift in the leading end of the barrier is still within the clear zone and should be protected. From your previous communication, we have been unable to provide an engineer approved design that would involve the installation of W beam buardrail at the face of the Hesco units. For this reason, we will be proceeding with the installation of the temporary concrete barrier. The primary issue is that this will impede further into the clear zone. This leaves a remaining distance of 1' from the fog line to the edge of the concrete barrier. Though this is less than a desirable distance, it does provide a proven system for the barrier, and will aid in traffic calming. We have explored reducing the lane widths in this vicinity, and that is not a viable option. There is curb and gutter on the east side, and the current lanes are 11 feet in width. In some cases, the existing lane widths are approaching 10.5'. The City of Tukwila would like to get approval to place the concrete barrier in this manner until such time as the Hesco units are removed and the guardrail is restored. Access to the property will be maintained by an approximate 15' opening in the Hesco units. The City will be staging Super Sacks nearby that will be used to seal the driveway opening in the event of a flood. At issue is the blunt edge of the Hesco unit and the concrete barrier. The longitudinal flare rate is consistent with an unrestrained rigid barrier system, as defined in Section 710 of the WSDOT Design Manual (Figure 710 -3). For the second item, how to protect the ends of the concrete barrier at the private driveway, the seven foot terminal end section works since it is on the trailing end of the barrier that is outside of the clear zone for the opposing traffic. However, the flare offset for the three foot shift in the leading end of the barrier is still within the clear zone and should be protected. She did not have any issues or concerns with the proposed installation. 10:30pm 1 Justin Symanski at the NC Machinery site delivering additional hes clips. 11:30am- Ivan Dunken from DOSH was at the NC Machinery site. He said that the step ladders 1:30pm used to access the Hecso units were too short and not being used properly (people stepping /standing on the top rungs etc.). He also said the method of fall arrest being used was not correct. The lanyards were too long for the 8' height of the baskets and the method of attachment to the baskets was not adequate for fall arrest. Russell Yoon (GBI safety officer), Lane Shinneck and Tim Barnes were the GBI representatives present during the inspection. Justin Symanski from Hesco was also present for a portion of the inspection. Many alternatives were discussed for achieving a compliant means for installing the fill in the baskets. Russell and Ivan agreed that switching to a fall restraint system with short tethers was the best option. Longer ladders were delivered during the time Ivan was on site. The Hesco units were to be located over eight of the catch basin grates, blocking drainage to the basins and creating ponding during normal rain events. Steel hoods were fabricated and placed over the basins, allowing the Hesco units to be set on top of the hood and maintain the drainage flow. The Hesco units were modified to fit over the hoods and geotextile was installed around the hood to keep sand from migrating into the basin. Prior to installation of the hoods, the basins were cleaned out with a vactor truck. This work included the fabrication and installation of the steel hoods, modification of the Hesco units, additional traffic control, and other incidental work associated with this item. There are 7 catch basins along the guard rail that will be covered by the Hesco units. Possible options: 1 1. Have a three sided steel box fabricated to fit over the CB grate. The box could be 3' wide X 3' long X 6" tall and the open side would face the highway. We could cut 6" off of the bottom of the Hesco unit to fit over the box. 2. Build 6" high asphalt ramps on either side of the CB grate. Place a steel sheet on the asphalt ramps to span over the CB grate. The back side (river side) would be closed with asphalt. One of the pipes was broken and leaking upstream of the outfall. The broken pipe had to be excavated and a swale created to channel the water. These outfalls would back up if the river rose and could potentially result in back flow condition behind the Hesco barrier. Due to the cost and long lead associated with Tideflex valves, the City approved the use of PVC flap gates for these small drainage outfalls. One of the outfall locations will be closed off in the event of a flood event and has been identified in the emergency closure plan. The locations of the flap gate installations are as follows: 1. An option that was discussed with WSDOT and the project team was to install the W -Beam guardrail to the face of the Hesco units, and then transition that guardrail to the existing guardrail, taking advantage of the existing terminal sections. Attachment options were discussed. GBI prefers to use a through bolt connection. 10128109: There will need to be modifications to the WADOT Plan. Generally, Mike: to work with there is a 4 foot elevation drop from the south terminus (Mitchell Moving and Storage) to 1 -405. Brian Dobbins The sand bags will add 3 feet to the levee with 1 foot of free board, at the same slope. In ongoing general, the west levee elevations are consistent. The east levee has been determined to vary in elevation. Areas like NC Machinery have been identified to need a double stack of Hesco units to match the elevation directly opposite on the west levee. The Modeling is expected to be completed tomorrow that will further define other areas that will need added height. Lane asked how much of an elevation difference can be tolerated on the East levee. Ryan Larson stated that 3 feet of effective height on the east levee is acceptable, as Tukwila Levee is 1 foot higher than USACE requirements. After some discussion it was determined that if placement of the Hesco unit achieves a full 3 feet that would be acceptable. Lane said this would make a big difference in the number of Hesco units. He believes a number of locations will be between 3 and 4 feet. So to achieve 4 feet everywhere will add a significant number. What is the direction of flow at this location. In the field it appears to drain the east facing slope off the shoulder of I5. Looking at this location I thought the flow was from north to south. Your email sounds like the reverse. We don't want to cause localized flooding and damage by installing the wrong gate. Please advise me of the direction of flow. What is the direction of flow at this location. In the field it appears to drain the east facing slope off the shoulder of I5. Looking at this location I thought the flow was from north to south. Your email sounds like the reverse. We don't want to cause localized flooding and damage by installing the wrong gate. Please advise me of the direction of flow. Work commenced on Monday, October 19, 2009, with GBI filling Supersacks on the Segale staging area. Today was the first day placing sandbags on the levee, the day was spent working out the issues, and refining the process. GBI is mobilized on the Segale site. Upcoming Work 10/26: GBI will add a second crew on the west levee. 10/26: A third crew will work on the Hesco wall at the south end, at Mitchell Moving and Storage. 10/29: A forth crew will go to work clearing and preparing to set Hesco on the east levee near Hwy 405. Vikki reminded GBI that NC Machinery has scheduled at Customer Open House on November 3 -5. Lane and Ron said that Bob O'Conner could follow up on this. Additional Hesco units are expected to be delivered in approximately 5 to 7 days, per Lane. •Efforts to obtain Signed License to Construct are progressing. PROJECT ISSUES itKion 001 Traffic Control SR 181 Zulven at Mitchell Moving 005 Hesco Units across 5'outhcenter Parkway Weekly Progress Meeting Tukwila Emergency Work Meeting Notes 10/22109: Mike R. provided WSDOT approved traffic control plans and re- channelization plans for SR 181. These will be formally transmitted later. GBI requested a modified traffic control plan to provide one lane in each direction on SR 181 for night time operation. Mike R. to develop plan. 002 Barrier for Hesco Units on SR 181 10122109: WSDOT has determined that the Hesco units are a fixed object in the clear zone. There will be no immediate change to the posted speed of 40 mph. Installation of temporary concrete barrier has been discussed with WSDOT. This would require the removal of the existing guardrail along SR 181. An option that was discussed with WSDOT and the project team was to install the W -Beam guardrail to the face of the Hesco units, and then transition that guardrail to the existing guardrail, taking advantage of the existing terminal sections. Attachment options were discussed. GBI prefers to use a through bolt connection. 003 Double Stack of Hesco on East Levee I SR 181 10122109: Lane said they are nearly complete with the modeling the survey data. Generally, there is a 4 foot elevation drop from the south terminus (Mitchell Moving and Storage) to 1 -405. The sand bags will add 3 feet to the levee with 1 foot of free board, at the same slope. In general, the west levee elevations are consistent. The east levee has been determined to vary in elevation. Areas like NC Machinery have been identified to need a double stack of Hesco units to match the elevation directly opposite on the west levee. The Modeling is expected to be completed tomorrow that will further define other areas that will need added height. Lane asked how much of an elevation difference can be tolerated on the East levee. Ryan Larson stated that 3 feet of effective height on the east levee is acceptable, as Tukwila Levee is 1 foot higher than USACE requirements. After some discussion it was determined that if placement of the Hesco unit achieves a full 3 feet that would be acceptable. Lane said this would make a big difference in the number of Hesco units. He believes a number of locations will be between 3 and 4 feet. So to achieve 4 feet everywhere will add a significant number. The identification of the lower sections will be identified and the City will help determine where 2 stacks of Hesco units will be used or where we might use and bags to increase the free board. T 10122109: Initial costs for an 18" gate valve at the culvert were $15,000. John Howat will see if the City has a valve in the bone yard that can be used. GBI will explore other valve types. Options discussed were a swing gate, irrigation valve, and butterfly valve. Butterfly valves were not recommended due to their propensity to clog. The 18" culvert can be installed now, and the valve added later. A field directive will be issued for this work. The valve, when installed, will have a gravel pad around it to permit operation. 10122109: Lane provided a profile drawing (attached) with existing ground topography for the Hesco units proposed at the south end of the project. The topography demonstrates the need to extend the barrier across Southcenter Parkway and onto the property to the west. Vikki and Ryan are to communicate with the business and Segale, to determine the best course of action. This is an open issue to be resolved. 006 Maintenance of Hesco and Supersacks 10122109: GBI is not responsible for ongoing maintenance. Ryan is going to follow up on contacting Monty Kaiser at the Corps for a visit. The current installation is near Mitchell Moving and Storage near station 3 +00. 008 UPRR Crossing Permit 10122/09: Mike R. noted that UPRR has requested minor changes to the permit application. Once approved by UPRR engineering, the permit will be sent to UPRR by the City with the processing fee (approximately $3,500). The units cannot be closer than 12' of the centerline of the tracks. No disturbance is permitted to the existing RR embankment on the east side. 009 Retaining wall 1 S 180th St Interface 10122109: At the existing trail crossing under S. 180th Street bridge, the placement of the Supersacks or Hesco units needs to be decided. The attached sketch shows a possible installation. The question needs to be answered whether the units will be placed on the trail grade, or on the top of the wall. Top of wall placement will require removal of the chain link fence material (leaving the posts), and removal of landscaping and constructing a level pad for the units. 010 SPU Watermain on SR 181 10122109: John Howat indicated that SPU has an existing watermain, running at 220 PSI crossing under the project. The proposed location for the Hesco units is on the sidewalk. In this area, the City rents the sidewalk from SPU. Vikki to contact SPU and discuss the potential impacts of the Hesco units on the sidewalk, which have an estimated weight of 480 pounds per square foot (assuming 120 pounds cubic foot). Pat Brodin should have the contact information for SPU 011 Private SD flap gate at Best Western 10122109: John Howat indicated that the existing outfall near the Best Western does not have a flap gate at the outlet, but may have one in a structure on private property. As part of the clearing operation for the Hesco installation, GBI to investigate to see if there is an existing flap gate. 012 Trail Closure 10122109: GBI needs information pertaining to the trail being closed. The City believes the park department has provided information to the public that the trail is closed, but no signs have been posted. It was determined that the signs should go up and be mounted on 4X4 posts. GBI should use their construction sign, now to close off the areas they are working in. The public is still actively using the trail and there is concern they will continue to use the trail even when it is posted. Jamie in the Parks department needs to be contacted about the signs. The City is preparing a public information sign. 013 Additional staging area 10122109: The City is looking to secure the SCL yard on the east side of SR 181, and north of Longacres Blvd. Ryan: Call Monty Kaiser. Change Order Narrative Highway 181 guardrail removal and cold mix asphalt installation The initial site investigation and design plan was to place Hesco units at the face of the existing guardrail. With the requirement by WSDOT to add concrete barrier at the face of the Hesco, there was inadequate room to install the Hesco units, without taking a lane of travel on SR 181. WSDOT agreed with a proposal to remove the guardrail and place the Hesco and concrete barrier as an alternative to closing one of the southbound lanes of SR 181. The affected section was along SR 181, from the north property line of NC Machinery, to the southern end of Nelson property (near Barnaby's) approximately River Station 143 +00 to 147 +00. Additionally, 50 LF of guardrail was removed at the southern end of NC Machinery, approximately at River Station 114 +50 to 115 +00. SR 181 has been overlaid several times, resulting in an area under the guard rail 4" to 6" lower than the adjacent roadway. This low area had to be brought up to grade with cold mix asphalt to create a level area for the Hesco installation. A total of 395' of guard rail was removed, salvaged and delivered to the City shops. A total of 44 tons of cold mix was installed. Additional traffic control was required for this night time operation over a period of two nights. This work shall be paid under the new item 102 "Highway 181 guardrail removal and cold mix asphalt installation on a lump sum basis. Description and Justification for Change The US Army Corps of Engineers directed the City to change the detail for installation of the Super Sack sand bags. This direction was given to the contractor at the preconstruction meeting on October 16 2009, as shown in the attached detail "Green River Levee Super Sack Installation Detail" (Revised 10/17/09). The change eliminated the plastic sheeting under the super sacks, added two small sand bags between each super sack and increased the number of the small bags placed on the plastic sheeting. The plastic sheeting was a special order item that had already shipped at the time the change was made. The width of the rolls of plastic sheeting could not be changed therefore no credit could be realized from the narrower plastic. The Contractor trimmed the excess plastic after the Super Sacks were covered and disposed of the excess. A total of 26,248 of additional sand bags were added due to the change in the installation detail, from the original quantity of 10,000 sand bags. Cost and Time Impact This work shall be paid under the new item 101 "Revised Super Sack Installation Detail on a lump sum basis. This work shall be full payment for all labor, equipment, and materials for the work described. Four (4) additional days are granted for this change. PROJECT OVERVIEW Due to potential for high levels of flow from the Howard Hanson dam, the City of Tukwila and the US Army Corps of Engineers (USAGE) have developed a plan to provide additional measures to heighten the current levee system through the southern part of Tukwila. The City of Kent and Auburn are also taking on such projects. As part of this work, the USACE has provided the City with 10,300 LF of Hesco Floodline units and 27,000 LF of Supersacks. The City of Tukwila Resolution No. 1689 permitted the Tukwila Public Works department to enter into negotiations with a contractor on an emergency basis to install the Hesco units and Supersacks. KBA was retained by the City, under this same resolution, to develop the construction documents and provide oversight during the installation. The City of Tukwila will be the lead agency for this work. Goodfellow Brothers was selected to perform this work. The work includes installing a Hesco barrier 'dam' at the southern end of the west side of the levee. This will be installed on the south side of the property occupied by Mitchell Moving &Storage. Along the remainder of the west levee, Supersacks will be placed. Along the east side of the levee, Hesco units will be installed. The work also includes work on 3 separate outfalls and 2 separate spot adjustments to the existing levee system near S. 180' Street. A notice to proceed letter, dated October 16, 2009 was provided to Goodfellow Brothers at this meeting. The effective NTP for this contract is Monday. October 19. 2009, having a duration of 30 calendar days. This NTP requests that the City's project number be included on all general correspondence, and that the City's Budget line item number be included on all requests for payment. The NTP also requests W -9, a Tukwila Business License, and Intent to Pay Prevailing Wages forms. Staging area. City has obtained permission from La Pianta (Segale Properties) for the use of the paved parking lot to the south of S. 180 Street and east of Andover Park West. GB1 will utilize this area to fill Supersacks. GB1 has also secured some area in the NC Machinery property, west of SR 181. Additional sites were discussed. The City has a staging area that is being used by Reed Sand and Gravel, who is currently installing a diversion pipe on Strander Blvd. This staging area (aka: the bone yard) is full of equipment and materials, and may not be large enough for turning movements of a truck and pup. POLICE, FIRE, HOSPITALS Valley Medical Center is the closest hospitals. Discussed emergency response, if needed. UTILITIES AND OTHER SERVICES Olympic Pipeline has been contacted by Vikki Stocker. GBI advised to contact one -call for other utility locates. No excavation is expected for the levee work, but there is some minor ground disturbance for the levee repair work and outfall construction. PUBLIC RELATIONS Detours. There should be no need for any street detours. The trail system will be closed for the duration of the work, and until such time as the Supersacks and Hesco units are removed. Access to homes and business should be maintained and coordinated. impacts to houses on SR 181 was discussed and site assessment is needed to determine the best location for Hesco unit installation. Should Hesco units need to be installed across their driveways, a section will be left out across the driveway to permit access. Details for this will be provided by the Hesco representative. There is no known residences requiring special medical attention or access issues. The contract documents address the city noise ordinance, which was waved for the work in this area due to the emergency nature of this work. License to Construct. Vikki Stocker is working with property owners to obtain LTC's. Mike Mathia will also be assisting. Should there need to be escalation with the property owner, Bob Giberson will provide the final decision and possibly meet with property owner. Access. GBI provided with a key to the City locks and will attempt to daisy chain their locks onto the existing gates. King County may have other gate locks. Complaints. Any complaints received during construction should be forwarded to Vikki Stocker, who will notify the City. Press. News media is expected to be on site to monitor the project. GBI and KBA staff will be advised to refer any requests for comment to the City's Public information Representative, Rhonda Berry 206.833.1850. Rhonda works for the Mayor's office. News media will be permitted to take video and /or pictures of the site and the construction, permitted there is no safety concern. PERMITS HPA Permit. Ryan Larson is working to obtain the HPA permit for this work. TRAFFIC CONTROL GBI will be required to provide a traffic control plan should they intend to take any lanes within the central business district (CBD). Most of the truck traffic will use S. 180 to gain access to the Segale staging area (SE corner of Andover Park West S. 180" Street). Traffic control will be needed on SR 181 for the work on the West Valley Highway. Mike Roberts is working with WSDOT to obtain approval for the re- striping plan on SR 181 and also to obtain a traffic control plan for the work on this roadway. The plan will most likely be a variant of the WSDOT K Plans. All flaggers must possess flagging cards. A TCM/TCS will be provided on West Valley Highway (SR 181). STORMWATER, EROSION CONTROL, SPILL CONTROL The project provides for installation of silt sacks, which will be provided as needed. Erosion control will be on an as- needed basis. Silt fence may be needed in areas where grading is needed for the installation of the Hesco units on the east side of the levee. A SPCC will be provided by GBI TRAINING GOAL, DBE GOAL None will be required for this project. DAILY OPERATIONS Work will be reviewed by KBA in the field Any contractor proposed change will be forwarded through KBA. Acceptance of the work will be daily by KBA. Following the completion of the work, the City will provide final acceptance. A single punchlist will be compiled by KBA at the substantial completion phase of the project and provided to GBI. MATERIALS DOCUMENTATION Material information requested on sand supplier, specifically the gradation and pit source(s). No specific materials testing will be performed for this project. CONTRACTOR'S RESPONSIBILITIES GBI is responsible for damage caused by their crews. Safety. GBI will have weekly safety toolbox meetings. Discussed river safety. Should river levels rise, a site specific plan will be developed and implemented. FORMS Certified weekly payrolls are requested to aid in force account calculations. Also requested is a listing of construction equipment with rates. As indicated in the NTP letter, Statement of Intent to Pay Prevailing Wage forms are required. The only anticipated subcontractor will be Apply -A -Line for the re- striping on SR 181. Wage rate information is included in the contract documents. KBA will track weekly statement of working (calendar) days. PROGRESS PAYMENTS Due to the short duration of this work, there will be one primary progress payment made at the end of the work. There may be some punchlist work that would necessitate a second progress payment. KBA will track quantities daily and agree with GBI on quantity installed on a weekly basis. KBA will prepare the progress payments for review by GBI and approval by the City of Tukwila. PROJECT PLANS Plans consist of a 31 sheet set of aerial drawings showing river centerline. General location of the Supersacks and Hesco units is defined an overview aerial. The exact location of the Supersack and Hesco units will be determined in the field by KBA. The plans also include a separate 7 sheet set that describe the work to repair 3 outlets and adjust the levee near the Segale staging area in two separate locations. The last 7 sheet plan set includes plans to re- stripe SR 181. The re- striping plans have yet to be approved by WSDOT. The re- striping plan is markers (RPM's) with yellow tubular markers (18" hi h). Supersack installation. The instal ation detail for the Supersacks, included Levee Temporary Flood Protection), provided by the City of Kent, is revised. Supersacks and also did not address the space that exists between the Supersacks at the ground level. This gap is due to the Supersacks bulging during filling. A revised detail, titled "Green River Levee Supersack Installation Detail' is provided as an attachment to these meeting notes. The key differences of this detail, over the Kent detail are: 2 sand bags between Supersacks to fill gap. This could result in the need for 18,000 additional sand bags to fill the gap. Plastic sheeting to be placed over the top of the Supersacks following their installation. This requires additional sandbags on each side of the plastic sheeting to weight the plastic sheeting. This could also result in the need For more sand bags. Weekly meetings will be held on Thursdays at 10 :00 a.m. at the Public Works office. Record Drawings. Both KBA and GBI will record the location of the work on the plan sheets provided. KBA will collate the comments at the end of the project and provide the City of Tukwila with a record set of drawings. Other Contracts. Reed Sand and Gravel is working on Strander Blvd to install a diversion pipe and pump station. No other projects are known. These meeting minutes represent a summary of the meeting discussions.
