MTE inspector arrived on site and completed reinforcing steel inspection of holding tank footing at
central location of jobsite per details on approved plans. Previous report written on 1/4/06 is no
longer a preliminary inspection report due to the fact that the #4's below embeds were placed per
detail on 3 on page S2.0. Checked clearances and cleanliness prior to concrete placement.
Observed the placement of 126'/2 cubic yards of Stoneway Concrete Mix # 4852 -A at footings
mentioned above. Concrete placed by pump truck with mechanical consolidation by vibration. A
composite sample was taken and a set of (4) 4 x 8" cylinders was cast for strength verification.
Placement started after 10:00am due to 2 different pump breakdowns.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

Structural Calculations
These calculations are provided as part of a submittal for the construction of a foundation to support (4) 11'0 x
40' tall, (2)13'0 x 34' tall and '(1) 6'0 x 8' tall storage tanks. Tanks are considered to have continuous bearing
around it's perimeter with equal distribution of load. The design of each tank to support it's own weight
including contents is not within the scope of Anderson - Peyton's work. Foundation is designed to support both
gravity and lateral loads induced from the tanks themselves. Foundation is cast directly over art existing slab on
grade over native soil per the project soils report.

Inspection: Structural Steel Visual Welding
Samples: N/A
MTE inspector arrived on site to perform visual weld inspection of the connection of the New
Tanks to the Foundation Embeds. Looked at the welds connecting the five new tanks to the
embeds in the foundation. The welds appear to be per plan and acceptable per AWS D1.1.
To the best of our knowledge, items inspected this date are in accordance with approved plans
and specifications.

RE: Permit No. D05 -404
5795 S 130 PL TUKW
Dear Permit Holder:
In reviewing our current records the above noted permit has not received a final inspection by the City of Tukwila Building Division.
Per the International Building Code and/or the International Mechanical Code, every permit issued by the Building Division under the
provisions of this code shall expire by limitation and become null and void if the building or work authorized by such permit is not
commenced within 180 days from the date of such permit, or if the building or work authorized by such pemut is suspended or
abandoned at any time after the work is commenced for a period of 180 days.
Based on the above, you are hereby advised to:
Call the City of Tukwila Inspection Request Line at 206 - 431 -2451 to schedule for the next or final inspection.
This inspection is intended to determine if substantial work has been accomplished since issuance of the permit or last inspection; or.if
the project should be considered abandoned.
If such determination is made, the Building Code does allow the Building Official to approve one or more extensions of time for
additional periods not exceeding 90 days each. Extension requests mist be in writing and provide satisfactory reasons why
circumstances beyond the applicants control have prevented action from being taken.
In the event you do not call for the above inspection and receive an extension prior to 06/17/2006, your permit will become null and
void and any further work on the project will require a new permit and associated fees.
Thank you for your cooperation in this matter.
Sincerely,
d iZfe rshall,
