APPLICATION SUBMITTAL In order to ensure that your application is accepted for plan review, please make sure to till out the
application completely and follow the plan submittal checklist on the reverse side of this form. Handouts are available at
the Building counter which provide more detailed information on application and plan submittal requirements.
Application and plans must be complete in order to be accepted for plan review.
VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of
Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting
application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is
subject to possible revision by the Building Division to comply with current fee schedules.
BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor
licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this
permit application and obtain the permit will be required as part of this submittal.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall
expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180
days upon written request by the applicant as defined, in Section 304(d) of the Uniform Building Code (current edition).
No application shall be extended more than once.

SPECIFICATION
The luminaire sidewalls shall be radius die - formed from
a single piece of extrusion of aluminum alloy 6063 -T4.
The housing shall have a 1.25" outside radius at each
corner. The crowned aluminum sheet canopy shall be
crimped and sealed to the housing. Finish on the
housing shall be . The flat, thermal and impact
tempered glass lens is set into an extruded aluminum
lens frame, sealed with an E.P.D.M. gasket. Vertical lamp
models utilize a drop convex thermal and impact
tempered glass lens. Door frame assembly shall be made
of aluminum extrusions and secured with extruded
internal corner keys, no welds. Door to be hinged with
aluminum pins and retained with a quick release safety
latch of stainless steel. The door frame shall be
removable. The GN -17 and GN -21 available with a choice
of IFS Type I, II, III, V or F (Forward Throw) patterns,
IES-V horizontal and vertical. GN -29 is available in IES II,
III or F distribution and IES V horizontal and vertical.
All sizes available with Type II and III Faceted Reflectors.
The reflector is precision hydro- formed or spun
aluminum, chemically brightened and anodized to a
semi - specular finish. Reflector system is sealed to
prevent entrance of dust or moisture.' Reflector system is
hinged and latched to the gasketed door frame for easy
cleaning or removal. (GN-29 reflector is not hinged). A
fuseholder inside the fixture is optional. Lamp socket
shall have nickel - plated current carrying parts with lamp
grip. Ballast components are mounted to the housing on
a separate, easily removable extruded aluminum heat
sink hanger bracket. Ballast shall be capable of starting
type of lamp of watts at voltage
down to -20 °F. The 200 °C., 600 volt insulating wiring
shall be installed between the ballast and the lampholder.
Primary voltage to the ballast is disconnected by a
plunger type switch when lens door is opened. The
standard arm assembly shall be of rectangular aluminum
extrusion complete with non -slip tie rod channels serving
as a compression member with two threaded and zinc-
plated steel tie rods and lock nuts in tension between
pole and luminaire. The luminaire shall bear the listing
mark of Underwriters Laboratory, "Fixtures suitable for
wet locations."

Magnuliter series MV /MH luminaires combine energy saving efficiency of H.I.D. sources
with the styling required for applications such as sportslighting, parks and recreational areas,
parking lots, street lighting, automobile sales lots and building facades.
Housing — One piece die -cast with hinged, full front access door.
Optics - Twin hydroformed parabolic reflectors, three piece forward throw cutoff, one piece
faceted hydroformed cutoff street MCIII distribution or one piece hydroformed parabolic
reflector having a horizontal spread with vertical control. Exclusive ANODAL® finish to seal
the high purity aluminum for long consistent performance. MV series offers vertical lamp
position, MH series offers horizontal lamp position, forward throw and street optics have
horizontal lamp position.
Glass Lens — Thermal shock, impact resistant, clear glass.
Ballast — 180 °C insulated -20 °F starting ( -40 °F HPS), 60 Hz H.P.F. ballast. 50 Hz 220/
240 volt available, consult factory.
POWR -PANL® - All electrical components, including socket, are mounted on an easily
removed support plate complete with handle. An access plate located on the panel provides
easy access for voltage change on QUAD-TAP® or TRI -TAP units and reduced installation
time. 1000W Metal Halide and 600W H.P.S. fixtures have electrical components mounted
directly to the luminaire body to ensure cooler operation and maximum product life. Units
with "F" and "A" optics have electrical components mounted directly to the luminaire body
allowing for maximum optical design and efficiency.
Mounting — MVM /MHP series uses '/4" heavy gauge steel trunnion (yoke). MVK /MHK
series has a 2" pipe (2W1e" O.D.) adjustable slipfitter. MVW /MHW series incorporates an integral
adjustable knuckle fitter for flat surfaces (compatible with Hubbell pole drilling patterns).
Both MVK /MHK and MVW /MHW knuckles provide concealed wiring chamber and a two
piece cast aluminum construction with serrated teeth provides for positive positioning. MVA
series is arm mounted with a 4 or 10 inch arm.
Additional Features. — Corrosion resistant hardware (exclusive HUBBELL GARD®
externally); high temperature gasketing; LEKTROCOTE® finish bronze or gray; field installable
PCR, or button type PC; U.L. 1572 listed, suitable for wet locations. C.S.A. certified for indoor
or outdoor use.600W H.P.S. provides 4 second restrike. IP Suitability - IP -65.
