APPLICATION
APPLICATION SUBMITTAL In order to ensure that your application is accepted for plan review, please make sure to fill out the
application completely and follow the plan submittal checklist on the reverse side of this form. Handouts are available at
the Building counter which provide more detailed information on application and plan submittal requirements.
Application and plans must be complete in order to be accepted for plan review.
VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of
Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting
application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is
subject to possible revision by the Building Division to comply with current fee schedules.
BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor
licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this
permit application and obtain the permit will be required as part of this submittal.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall
expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180
days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition).
No application shall be extended more than once.

Special Provisions for
Permits and Franchises
2. A copy of the permit or franchise must be on the Job site, and protected from the elements, at all times during any of the construction authorized by said
pemtlfranchlse. •
I 3. In the event any milepost, right of way marker, fence or guard rail is located within the limits of this project and will be disturbed during construction, these
items will be carefully removed prior to construction and reset or replaced at the conclusion of construction to the satisfaction of the Department. AU signs
and traffic control devices must be maintained In operation during construction.
® 4. Prior to construction, the Utility shall contact the Department's representative (listed under Special Provision Number 1) to ascertain the location of
survey control monuments within the project limits. in the event any monuments will be altered, damaged or destroyed by the project, appropriate action
will be taken by the Department, prior to construction, to reference or reset the monuments. Any monuments altered, damaged or destroyed by the
Utilitys operation will be reset or replaced by the Department at the sole expense of the Utility.
® 5. During the construction and/or maintenance of this facility, the Utility shall comply with the Manual on Uniform Traffic Control Devices for Streets and
Highways. If determined necessary by the Department, the Utility shall submit a signing and traffic control plan to the Department's representative for
approval prior to construction or maintenance operations.
El 6. The Utility shall notify the Departments representative upon completion of the work under this permit/franchise so that a final inspection can be made
and shall immediately furnish to the District Utilities Engineer a revised franchise or permit plan of the final location or relocation of its facilities U the
original permit/franchise plans have been revised during the course of construction.
❑ 7, Prior to the beginning of construction, a preconstruction conference shall be held at which the Department and the Utility and utility's engineer,
contractor, and inspector shall be present. '
❑ 8. Should the Utility choose to perform the work outlined herein with other than its own forces, a representative of the Utility shall be present at all times
unless otherwise agreed to by the District representative. All contact between the Department and the Utilitys contractor shall be through the
representative of the Utility. Where the Utility chooses to perform the work with Its own forces, k may elect to appoint one of ka own employees engaged
In the construction as its representative. Failure to comply with thls provision shall be grounds for restricting any further work by the Utility within the
State right of way until said requirement Is met. The Utility, at its own expense, shall adequately police and supervise all work on the above described
project by itself, its contractor, subcontractor, agent, and others, so as not to endanger or Injure any person or property.
❑ 9. The Utility agrees 10 schedule the work herein referred to and perform said work in such a manner as not to delay the Department's contractor in the
performance of his contract.
® 10. Work within the right of way shall be restricted to between the hours of 9:00 a.m. and '3:00 p.m. , and no work
shall be allowed on the right of way Saturday, Sunday, or holidays, unless authorized by the Department. Any lane closures must be submitted for
approval In advance of use. The hours of permitted closure may differ from the above noted hours.
® 11, If determined necessary by the Department, any or all of the excavated material shall be removed and replaced with suitable materiel as specified by the
Department.
® 12. Wherever deemed necessary by the Washington State Department of Labor and industries and/or the Department for the safety of the workers and the
protection of the highway pavement, the sides of the trench (or excavation) shall be adequately supported to reduce the hazard to workers and prevent
any damage by cracks, settlement, etc., to the pavement. No other work in the trench or excavation area will be allowed until this requirement is met.
® 13. Trenches shall be backfifed as soon as possible behind the laying of pipe or cable. No open trenches shall be left overnight. This Includes boring or
Jaddng pits which shall be covered with material of sufficient strength to withstand the load of highway traffic If the pit Is not to be bacidllled with
material each night.

SPECIAL PROVISIONS FOR PERMITS AND FRANCHISES
x34. Construction of the retention/detention system, flow restrictor and pollution
control device shall be part of the initial construction of this system.
x35. The Utility is responsible for compliance with all federal, state, and local
laws pertaining to the discharge received.by the State under this permit or
franchise.
. x36. Construction of this facility will not be permitted from the through traffic
roadways, shoulders or ramps of SR 5. All access•during construction shall
be from outside the State right of way.
• x37. The Grantee/Utility shall be required to maintain traffic at all times, no lanes
or ramps shall be allowed closed without written State approval and a State
approved traffic control plan.

This letter is an addendum to our letter dated January 12, 1996 in which we summarized
our geotechnical consultation related to an Ultrablock retaining wall to be installed along the east
side of building A at the Park Place project site in Tukwila, Washington.
We have made a further review of the drainage requirements for the wall as shown on the
cross- section prepared by Landplan, P.S. The primary sources of infiltration into the drainage
material behind the block wall will be irrigation water or rainfall. The pavement above the wall
will be sloped to drain away from the wall. As such, the volume of water collecting in the
drainage material behind the wall will be small and will not, in our opinion, require a tight line
disposal outlet.
We recommend that the following design changes be made to provide for the relief of any
water which may collect behind the wall;
• The block wall should be constructed with the toe set back three inches from the
adjoining property line. In order to accommodate this setback without changing the planned
alignment of the top of the blocks, the batter of the face of the blocks should be steepened
to 1 (horizontal): 6 (vertical). This will not materially affect the stability of the wall.
• The base row of blocks is to be founded on a layer of the same clean, free - draining
granular material placed behind the blocks. The area along the wall alignment should be
excavated to permit placement of a zone of this material to a depth of at least six inches
below the base of the blocks. This zone should also extend to the adjoining property line.
• After the lowest course of blocks has been set, the space between the face of the blocks
and the existing pavement along the wall should be filled with. l - draining granular

material. This will create a free pith for seepage if water should collect in the drainage
zone behind the blocks.
By crating a flow path for seepage in this manner, the level of any saturated backfill behind the
blocks will be limited to the level of the lower adjoining pavement. Any seepage at the edge of
the pavement will be minor and, during the wet season, likely indistinguishable from rainfall
collecting on the surface, The amount of irrigation water applied during the summer months can
be limited to avoid seepage at the bus of the wall.

This letter summarizes our geotechnical consultation related to a retaining wall to be
installed along the east side of building A at the Park Place project site in Tukwila, Washington.
We have reviewed various concepts for the wall which have been developed by Turner &
Associates in conjunction with Shutler Consulting Engineers, Inc., Bush, Roed & Hitchings, Inc.
and Landplan, P.S. The preferred concept includes a concrete ( "Ultrablock ") wall used to retain
structural fill for the parking area east of building A. The wall will have a length of about 78
feet, and will extend along the southeast property line (near the adjacent furniture store). We
understand that the maximum grade difference between the new parking area and the existing
parking area for the furniture store will be about 6 feet. This grade difference is being confirmed
by Bush, Roed & Hitchings.
Cross section A -A, prepared by Landplan, P.S., shows the proposed "Ultrablock" wall with
a wall height of 4.5 feet and a backslope of about 2H:1V (horizontal to vertical). The total
horizontal distance from the toe of the wall to the edge of the new pavement will be 6 feet. The
wall is shown as two blocks high (each block having a height of 2.5 feet) and a batter of 1H:4V.
The backslope is to be covered with topsoil. We understand that ornamental trees mkt be
planted at intervals just behind the wall. u'a/
Based on our review and experience with various types of retaining walls in this area, we
conclude that the concept shown in Section A -A is generally adequate. We have the following
recommended modifications:
• The backslope behind the wall should be inclined no steeper than 2H:1V.
• The wall face batter could be steepened to 1H:6V, if needed. This would help achieve a

• Any loose or disturbed soils on the slope face along the alignment of the wall which result
from construction should be recompacted, if practical, or removed and replaced with
suitable structural fill soil that is compacted to a firm, unyielding condition. It may be most
effective to remove this material and replace it with compacted drainage material as the
blocks are placed.
• Root balls for the ornamental trees to be planted behind the wall should be placed into flat -
bottomed notches cut into the existing fill slope, rather than placing the topsoil cover
around the root balls on an inclined existing fill surface.
• The perforated pipe shown in Section A -A can be bedded in the same granular soil used as
a base beneath the wall and can be terminated in a french drain sump in the landscape
island located near the east end of the wall. Based on the limited size of the catchment area
behind the wall, the flow of water in the perforated pipe should be relatively low.

1. Always start with a smooth base, joint lines in the wall can only
be as straight as the base on which they are placed. A six inch
blanket of granular material, compacted and raked or screeded
smooth is normally a sufficient base.

tailing care to align the front face of the wall. ( remember, there Is
approximately 1/Z" of clearance In every direction In the M
of the keyways)
6. Should the dearance become tight for the second row of blocks,
merely place the next block along the bottom row, with a slight gap
(1/41, the second and subsequent rows will then have sufficient
clearance. Continue placing subsequent rows, taking care to align
the front face.

INSTALLATION TIPS
7. Non right -angle corners, or corners where walls have
different batters are achieved more easily by building the 2 walls
independently and pouring the comer afterwards. Chamfer strips
attached to the Inside of the formwork will blend the comer in with
the rest of the wall.
S. If desired, the wall can be curved either vertically, horizontally,
or both.
9. For speed & convenience a track mounted hydraulic excavator
is the best machine to place the blocks.
10. Pressure washing lit sealing the surface will minimize
algae growth and prolong new appearance.
11. Some useful tools to have on the job-site include:
• A transit to lay out a level base. - Shovels and rakes for base
preparation.
• A lifting jig to hold blocks at the correct batter.
• A broom to clean the keyways before placing the next layer.
• One or more 5 foot pry bars for jostling the blocks Into position.
