6. Location and screening of outdoor storage (change of use only)
7. Limits of clearing /grading with existing and proposed topography at 2' intervals extending 5' beyond property's
boundaries
8. Identify location of sensitive area slopes 20% or greater, wetlands, watercourses and their buffers (change of
use only)
9. Identify location and size of existing trees that are located in sensitive areas and buffer (TMC 18.45.040), of
those, identify by size and species which are to be removed and saved
10. Landscape plan with irrigation and existing trees to be saved by size and species (exterior changes or change
of use only)
11. Location and gross floor area of existing structure with dimensions and setback
12. Lowest finished floor elevation (if in flood control zone)

9).

Food service establishments require two (2) sets of stamped approved plans by the Seattle -King County
Department of Public Health prior to submitting for building permit application. The Department of
Public Health is located at 201 Smith Tower, Seattle, WA or call (206) 296 -4787. (Form H -5)

contractor has been selected at time of application a copy of this license will be required before the
permit is issued OR submit Form H -4, "Affidavit in Lieu of Certificate of Contractor ".

by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and
obtain the permit will be required as part of this submittal

Permit Conditions:
1. No changes will be made to the plans unless approved by the
Architect or Engineer and the Tukwila Building Division.
2. Plumbing permits shall be obtained .through the Seattle- King
County Department of Public. Health. Plumbing will be
inspected by that.agency, including all gas piping
(296- 4722).
3. Electrical permits shall be obtained through the Washington
State Division of Labor and Industries and all electrical
work will be inspected by that agency (243 - 6630).
4. All mechanical work shall be under separate permit issued by
the City of Tukwila.
5. All permits, inspection records, and approved plans shall be
available at the job site prior to the start of any con-
struction. Thee documents are to be maintained and avail
able until final inspection approval is granted.
6. Any new ceiling grid and light fixture installation is •
required to meet lateral bracing requirements for Seismic
Zone 3.
7. Partition walls attached to ceiling grid must be laterally
braced if over eight (8) feet in length.
t ?. VENTILATION IS REQUIRED FOR ALL NEW ROOMS AND SPACES OF NEW
OR EXISTING BUILDINGS IN CONFORMANCE WITH THE UNIFORM .
BUILDING 'CODE AND THE WASHINGTON STATE VENTILATION AND
INDOOR AIR QUALITY CODE, CHAPTER 51 -13 WPC.
9. All construction to be done in conformance with approved
plans and requirements of the Uniform Building Code (1994
Edition) as amended, Uniform Mechanical Code (1994 Edition),
and Washington State Energy Code (1994 Edition).
10. Inspection of the lighting & controls shall be called for
after the installation of all lighting and controls, but
before concealment of the lighting equipment.
11. Any exposed insulations backing material shall have a Flame
Spread Rating of 25 or less, and material shall bear identi-
fication showing the fire performance rating thereof.
12. Ail 'rood preparation establishments must have 'Seattle -King
County Depar of Public Health sign -off prior to opening
or doing any food processing. Arrangements for final Health
Department inspection should be made by calling Seattle -King
County Department of Public Health, 296- 4787, at least three
working days prior to desired inspection date. On work
requiring Health Department approval, it is the contractor's
r e s p o n s i b i l i t y to have a set of plans approved by that
agency on the job site.
13. There shall be no occupancy of the building(:) until the
final inspection has been completed by the Tukwila Building
Inspector.
14. A CERTIFICATE OF OCCUPANCY WILL BE REQUIRED FOR THIS PERMIT.
15. Validity of Permit. The issuance of a permit or approval of
plans, specifications, and computations shall not be con-
strued to be a permit for or an approval of, any violation
of any of the provisions of the building code or of any
other ordinance of the iurisdiction. No permit presuming to
give authority to violate or cancel the provisions of this
code shall be valid.

Thank you for your call on regarding the mechanical inspection. As you are well aware of from building
many of our restaurants, we have never had a two hour rated hood system. The two burner elements that
we have are for boiling beans.
I have enclosed a menu for your examination. We use rice cookers, cold vegetables and beans to make our
Wraps (burritos). Our concept is "High Energy" "Healthy" food. The thought of fat, fat frying, broiling or
any other method of carcinogens or unhealthy way of cooking is completely against everything that we
stand for.

I have reviewed the sample of the proposed window graphics to be used at the Todo
Wraps building in Tukwila. I find that the graphics would constitute a sign because they
are "intended to be used to attract attention to the subject matter for advertising or
identification purposes" and indicate "the goods sold or produced on the immediate
premises" TMC 19.08.210, TMC 19.08.140. Therefore any use of these graphics would
have to comply with the restrictions of the Tukwila Sign Code.

The attached set of building plans have been reviewed by
The Fire Prevention Bureau and are acceptable with the
following concerns:
1. The total number of fire extinguishers required for
your establishment is calculated at one extinguisher for
each 3000 sq. ft. of area. The extinguisher(s) should be
of the "All Purpose" (2A, 10B:C) dry chemical type. Travel
distance to any fire extinguisher must be 75' or less.
(NFPA 10, 3 -1.1)
Extinguishers shall be installed on the hangers or in
the brackets supplied, mounted An cabinets, or set on
shelves (NFPA 10, 1 -6.9), and shall be installed so
that the top of the extinguisher is not more than 5
feet above the floor. (NFPA 10, 1 -6.9)
Extinguishers shall be located so as to be in plain
view (if at all possible), or if not in plain view,
they shall be identified with a sign stating, "Fire
Extinguisher ", with an arrow pointing to the unit.
(NFPA 10, 1 -6.3) (UFC Standard 10 -1)
Clear access to fire extinguishers is required at all
times. They may not be hidden or obstructed. (NFPA
10, 1 -6.5)
Maintain fire extinguisher coverage throughout.
2. Exit doors shall swing in the direction of exit travel
when serving any hazardous area or when serving an occupant
load of 50 or more. (UBC 1004.2)

Exit doors shall be openable from the inside without
the use of a key or any special knowledge or effort.
Exit doors shall not be locked, chained, bolted,
barred, latched or otherwise rendered unusable. All
locking devices shall be of an approved type. (UFC
1207.3)
Obstructions, including storage, shall not be placed
in the required width of an exit, except projections
as permitted by the Building Code. Exits shall not be
obstructed in any manner and shall remain free of any
material or matter where its presence would obstruct
or render the exit hazardous. (UFC 1203)
Dead bolts are not allowed on auxiliary exit doors
unless the dead bolt is automatically retracted when
the door handle is engaged from inside the tenant
space.
Exit hardware and marking shall meet the requirements
of the Uniform Fire Code. (UFC 1207.1- 1212.8)
Internally illuminated exit signs shall have both
bulbs working at all times. (UBC 1013.3)
Aisles leading to required exits shall be provided
from all portions of buildings. Aisles located within
an accessible route of travel shall also comply with
the Building Code requirements for accessibility.
(UFC 1204.1)
Combustible material shall not be stored in exits or
exit enclosures. (UFC 1103.3.2.3)
Current supply to one of the lamps for exit signs
shall be provided by the premises' wiring system.
Power to the other lamp shall be from storage
batteries or an on -site generator set and the system
shall be installed in accordance with the Electrical
Code. (UBC 1013.4)

3. All new sprinkler systems and all modifications to
existing sprinkler systems shall have fire department
review and approval of drawings prior to installation or
modification. New sprinkler systems and all modifications
to sprinkler systems involving more than 50 heads shall
have the written approval of the W.S.R.B., Factory Mutual,
Industrial Risk Insurers, Kemper or any other
representative designated and /or recognized by The City of
Tukwila, prior to submittal to the Tukwila Fire Prevention
Bureau. No sprinkler work shall commence without approved

DIVISION 1 - GENERAL CONDITIONS
1. General Conditions: The general conditions of the "abbreviated form of
- agreement between Owner and Contractor for AIA latest edition is hereby
made a port of this contract as If attached hereto or written herein.
2. Contrcctor's Duties: in addition to duties specified throughout the
contract documents, the Contractors duties are generally as follows.
A. Construction Costs. Except as specifically noted, provide and pay for
labcr, materials and equipment, tools, construction equipment and
machinery, water, heat, electrical power and Tight, utilities required
for construction, and other facilities and service necessary for
proper execution and completion of the work.
B. Pemdts and Fees: Secure and pay for government fees, licenses, and
permits except as specifically noted herein. As with all other
permits, the Contractor shall obtain the building permit.
C. The Owner sholl pay for all testing. The Contractor, however, will be
responsible for the cost of test reports that are substandard. or
foiled and all appropriate retesting. All tests shall be made by a
well established independent testing laboratory, having the facilities
to make the test required, and which is satisfactory to the Owner's
Representative. The testing loboratory shall report the results of
all tests in writing to the Contractor and the Owner's Representative.
0. Certificate of Occupancy. Obtain and record certificate of occupancy
at completion of work. Forward copy of same to the Owner.
E. Regulations and Notices: Comply with codes, ordinances, rules,
regulations and other requirements of public authorities which pertain
to the completion of the work and give all required notices, Including
written notice to the Architect, of any observed variances in the
contract documents with legal requirements.
F. Contractor shall attend preconstruction meetings with the mall and /or
the Owner or Owners Representative as required.
G. Coordination: Coordinate work of the various trades and
subcontractors to assure efficient and orderly sequence of
installation of construction elements, with provisions for
accommodating Items installed later. Verify that characteristics of
interrelated operating equipment are compatible; coordinate work of
various sections having interdependent responsibilities for installing,
connection to, and placing in service, such equipment.
H. Supervision: All work Included in this contract shall be done under
constant supervision of a qualified superintendent. Superintendents
cannot be changed without prior approval of the Owner.
3. Use of premises: Confine operation at site to area permitted by law,
ordinances, permits, moll representative and the contract documents. Do
not load structure with weights which will endanger structure.
4. Protections:
A. Barricades dividing the work site from the mall must be coordinated
with and approved by the mall. Contractor to Incur all costs for
any existing barricade provided by Landlord.
B. Personnel: Protect personnel, whether occupants of existing building
and construction ion is spaces,
hased from harm the d i or shoppers if
injury.
C. Construction: Protect existing and adjoining spaces, utilities, and
work of any kind which is to remain from damage, defacement, or
interruption o f service, except as may be specially directed or
authorized. All public spaces and adjacent tenant - paces shot be
repaired if damaged, and left in a condition matching the original
condition. Any merchandise damaged in adjacent tenant's spaces shall
be replaced by the Contractor.
D. Existing Utilities: Protect any existing ° utilities to remain' from
any damage or Interruption of service. If necessary, obtain
permission of mall and utility company and relocate as required for
completion of the work. Overtime required for the installation,
connection or relocation of utilities shall be included In the
Contractor's bid.
E. Adjacent Property. Prior to commencement of any work, inspect
adjacent tenant spaces and property for existing signs of damage,
Record any such defects observed and document for future determination
of any damage caused by the work of this contract.
5. Warranty. Provide written warranty of not less than one (1) year against
defects in workmanship, materials and equipment. Warranty period begins
upon date of substantial completion.
6. Work not included: All items marked ° N.I.C. ", by Owner' or by others."
7. Site Coordination:
A. Lines and Levels: Maintain true ollgnment and levels to existing
construction. All new work and patchwork to match existing lines,
levels and finishes unless otherwise noted.
B. Existing Conditions: Verify all existing conditions and existing
Items to be relocated (noted on drawings). In addition, once ny
required demolition Is complete, verify that nothing Interferes with
the proposed construction. Deviations from conditions shown shall
promptly be reported to the Architect for resolution.
8. Contractor & Subcontractor Qualifications: All subcontractors engaged by
the General Contractor shall be bondable, licensed subcontractors, possess
good labor relations, and be capable of performing quality work. The Owner
reserves the right to approve or disapprove of any subcontractor. The
General Contractor and subcontractors must be licensed in the state of
the project site, and Contractors obtaining permits shall have a business
license from the site city, or as required by the building deportment.

9. Insurance
A. Purchase and maintain the following insurance, unless otherwise
directed by the moll criteria:
(1) Worker's compensation and employers' liability insurance as
required by any employee benefit act or the statutes applicable
where the work f3 to be performed. as will protect Contractor
from any and all liability under the aforementioned acts. but not
less then $2,000,000.
(2) Comprehensive general liability Insurance (Including Owner and
Contractor for future liability insurance) with following limits
of liability.
(a) Bodily Injury liability, each occurrence $2,000,000
(b) Bodily injury liability, aggregate $2,000,000
(c) Property damage liability, each occurrence $500,000
(d) Property damage liability, aggregate $500.000
(e) Personal injury liability, aggregate $2,000.000
Such insurance shall provide for explosion and collapse coverage
and contractual liability coverage, and shall Insure Contractor
against any and all claims for personal Injury, Including death
resulting therefrom and damage to the property of others arising
from its operations under its contract, whether such operations
are performed by Contractor or by anyone directly or indirectly
employed by the Contractor.
Comprehensive automobile liability insurance including the
ownership, maintenance and operation Of any automotive equipment
owned, hired, and non-owned in the following minimum ounts:
(a) Bodily Injury liability, each person $500,000
(1) Bodily injury liability, each occurrence $1,000,000
(c) Property damage liability, each occurrence 5500,000
Such Insurance shall Insure against any and all claims for bodily
Injury, including death resulting therefrom and damage to the
properly of others arising from Its operations under its contract
with Owner, whether such operations are performed by Contractor
or by anyone directly or Indirectly employed by the Contractor.
(4) Builder's risk completed value form affording 'all risks of
physical loss or damage" on its work in the premises as It
dated to the building In which the premises ore located, naming
the Interests of General Contractor and all Subcontractors as
their respective interests rnay appear, within a radius of 100' of
the premises. Amount of insurance shall equal one hundred
percent (100%) of the replacement cost.
(5) All such insurance policies required shall include Owner, its
Architect, its General Contractor, & Subcontractors as additional
insureds: Except worker's compensation insurance, which shall
contain on endorsement waiving all Fights of subrogation against
the mall, its Architect, its General Contractor, and
Subcontractors. Certificates of insurance shall provide that no
such insurance coverage shall be canceled or allowed to expire
until at least thirty (30) day prior to written notice to the
Owner.
10. Conflicts in Drawings and Specifications:
A. Bring all conflicts in drawings immediately to the attention of the
Architect, whereupon a final decision will be made. Where conflicts
or omissions occur, it will be assumed that the Contractor has bid
the most expensive method. After signing contract, n o c
'n rease in
price will be allowed for any conflict or omission when the intent (by
decision of Architect) is established by the character of the project.
B. Do not scale the drawings for dimensional information. Should a .
dimension be missing or in conflict, contact the Architect
Immediately.
11. Substitutions: The General Contractor shall notify the Architect within
ten (10) day from start of constriction of all proposed substitutions due
to procurement difficulties, and upon award of bid for all substitution Items
bid other than specified. Failure to do so will re It in the General
or subcontractor being held responsible for any additional cots necessary
to obtoln the product as specified, including air freight premium charges,
and maintain the construction schedule. For a substitution to be
considered, the Contractor will submit complete data, Including samples, for
both the specified and proposed item for comparative review by the Architect
12. Submittals: Submit reports of inspections, tests and approvals required by
the contract documents to the Owner in duplicate. Submit one additional
copy of reports of inspections, tests and approvals of structural,
mechanical or electrical work directly to the respective Consulting
Engineer.
The Contractor shall famish two blueline copies and one sepia copy of shop
drawings to the Architect or Engineer as required by the contract
documents Shaw dimensions and other selected characteristics. Furnish
catalog cuts, if appropriate, In triplicate. Delete material which does
not apply to this job.
A. Architect's Review of Submittals: The Architect will review
Contractors submittals for conformance with the design concept and
requirement for the contract documents and will approve or take other
appropriate aotfbn. os Indicated by his stamp an returned submittal.
B. Submit shop drawings for all custom fabricated work prior to
fabrication. This includes casework and store fixtures. Also submit
cut sheets of lighting, mechanical, door hardware, appliances and
equipment. Architect will not review shop drawings unless they have
already been reviewed and stamped approved by the Contractor.
C. Architect's Approval: The Architect's approval of any submittal shall
not relieve the Contractor of his responsibility for compliance with
the contract documents. Such approval means that the Architect has
reviewed the submittal and finds it compatible with the design concept
of the work and fn conformance with requirements of the contract
documents, unless otherwise noted. Approval shall not Imply that the
Architect has checked for quantities, space requirements, coordination
of trades, or possible means or cause of injury to person and /or
properly.

D. Submit physical samples of all finishes specified fn the Finish Schedule
to illustrate materials and Workmanship and to establish-standard by
which the work will be judged. Submit samples in triplicate in as
nearly the form in which the material will appear In the work as
practicable, unless otherwise noted. The Architect w91 review the
submitted samples for compliance with the contract documents and
compare them with file samples. Finishes are not to be applied prior
to approval
13. Spri Wer Layout: The sprinkler subcontractor shall modify the exiting design
and provide an approved 'calculated" sprinkler system layout to the architect that
conforms to new design. Revisions must be sensitive to the overall visual impact,
as 'determined by the Architect. Drawings and calculations for such systems
shall be submitted to the Owners•fnsurance agency (prior to fabrication
of any material) for review and approval prior to submission of the code revew
authority. Drawings and calculations must also be signed and sealed by an
engineer licensed fn the state where the subject buldng is located. All
costs' for review, permit, signing and sealing of drawings and calculations
shall be borne by the sprinkler subcontractor.
14. Sequence of Construction: Use any sequence of construction, subject to the
mall's restrictions, compatible to meet the cnstructian schedule
15. On -Site Telephone /Fax. The General Contractor ft required to maintain' an on-
site telephone /fax for the use of all parties having any legal respns30ity
for any portion of the work. All users shall reimburse the General
Contractor for all long - distance use. With the exception of such use, all
costs for installation, maintenance, operation, and removal a f this
telephone are the responsibility of the General Contractor. . The telephone
will remain connected until final completion or the store telephone ft
18. Procedures for change order and claims for extra work
A. All work considered by the Contractor to be N edddion to the contract
must be authorized by the Owner's representative: In order for slot work to
be considered by the Owners representative, the Contractor must first submit
a written proposal that includes on outline of wok and subsequent cost
including any overtime charges. All executed Change order farms must
be sent to the Owner's .. representative prior to work being performed.
Overtime work required to complete the originally contracted work
within the approved project schedule wll not be considered as an
additional cost to the contract sum.
17. Construction Schedule: The Contractor must prepare and present a
"Completion Schedule' for construction within 48 hours after awarding of
project. Such schedule shall show 011 items of work, start date, date of
completion and duration of work. All major and critical delivery items
shall be flogged and the earliest delivery and latest delivery shag be
noted on such schedule. This schedule shall be presented, discussed, and
approved by the Owner prior to contract execution. Ability of Contractor
to provide such a schedule showing completion within the required time
schedule shall be a condition prerequisite to the signing of the contract

18. Owner Occupancy. The Omer reserves the right to take initial occupancy of
the building as scheduled, particularly with regard to delivery and

19. Project pose -Out z
A. Final Cleaning: Clean in accordance with excellent housekeeping
practices. Remove grease mastic, adhesives, dust, dirt, stains,
fingerprints, labels and other foreign materials torn sight on all exposed
surfaces. Wash and shine storefront glazing. Polish glossy surfaces
to shine dearly. Clean and vacuum all carpets.
B. Operations and Maintenance Data:
(1) Materials and Maintenance Manuals:. Furnish two (2) final copies'
of 'Materials Manual Deliver final copies to Omer at time of
final acceptance. Manuals to include materials as listed below
and to include maintenance instructions where noted. Bind
finished copies in hardbound black covers, with label to include
project name, Owner, architect, contractor and date of
completion. Include bdex and tabbed dividers between
categories. Manuals to include:

provide temporary heaters, dehumidifiers and other means deemed required by the
Installer, to maintain the proper conditions so that the temperature and relative

1 Installation:
A. Condition of Surfaces all surfaces to receive paint shall be clean, dry,
a mooth, and dust free before application of any material. Prepare surfaces

holes, and other pits or depressions flush and smooth with putty of
pad dough after priming. Color putty to match finish paint coat.
