MEMO FOR RECORD DATE; 8/12/92 FROM: BLDG OFFL SUBJECT: WORK WITHOUT A PERMIT AT 3400 S. 150TH ST (OLYUMPUS PRESS) , -D 178 FIRE MARSHAL CALLED ME AT 3:40 PM, 8/11/92, AND SAID ONE OF THE FIRE DEPT INSPECTION CREWS WERE REPORTING A POSSIBLE DANGEROUS ROOF CONDITION THAT THEY NOTED DURING A ROUTINE INSPECTION. THE CREW WAS STILL ON THE SITE AND WERE REQUESTING A BUILDING INSPECTOR TAKE A LOOK AT IT. I CONTACTED DAVE LARSON (BLDG INSP) AND HE DEPARTED FOR THE SITE AT 3:44. DAVE CALLED BACK AT 4:10 REPORTING THAT HE THOUGHT THERE WAS A POTENTIAL PROBLEM. EXCAVATION WORK WAS BEING ACCOMPLISHED INSIDE THE BUILDING, IN CONJUNCTION WITH SOME SORT OF CONTAMINATED SOIL CLEANUP PROJECT, AND A PORTION OF THE CONCRETE FLOOR AND A FOOTING FOR A ROOF SUPPORTING COLUMN HAD BEEN REMOVED. THERE WAS NO RECORD OF ANY PERMITS FOR ANY OF THIS WORK AND DAVE SAID THE TEMPORARY SUPPORT OF THE ROOF MIGHT NOT BE ADEQUATE. HE SAID THERE WERE NO NON - CONTRACTOR WORKERS IN THE IMMEDIATE VICINITY BUT THERE WAS PORTION OF THE BUILDING AT THE FAR END WHERE NON - CONSTRUCTION PERSONNEL WORK. DAVE'S CONCERN WAS THAT IT APPEARED THE TEMPORARY BRACING LOOKED LIKE IT MIGHT BE OVERSTRESSED AND HE THOUGHT THAT IF THE ROOF COLLAPSED THERE WAS A POSSIBILITY IT MIGHT AFFECT THE AREA WHERE THE PEOPLE WERE WORKING. I RECOMMENDED THAT HE AND THE FIRE DEPT PERSONNEL CONTACT THE MANAGER AND HAVE THEM EVACUATE THE BUILDING UNTIL A BUILDING PERMIT WAS OBTAINED, WHICH WOULD REQUIRE DOCUMENTATION BY A LICENSED STRUCTURAL ENGINEER CERTIFYING TO THE SAFETY OF THE ROOF AND TEMPORARY BRACING. I TALKED TO DAVE AGAIN AT 5 PM, JUST BEFORE GOING HOME, AND HE SAID THEY HAD POSTED A DANGEROUS BUILDING TAG, ROPED OFF THE AREA, AND THE BUILDING HAD BEEN EVACUATED. HE SAID SOMEONE WOULD BE IN TO FIND OUT WHAT WAS NEEDED AND APPLY FOR PERMITS THE NEXT MORNING. FRED BENZ FROM NORTHWEST ENVIROFIELD SERVICES CALLED THIS MORNING AND SAID THEY HADN'T REALIZED THEY NEEDED A BUILDING PERMIT BUT HE AND THE STRUCTURAL ENGINEER WOULD BE IN THIS MORNING TO DISCUSS THE SITUATION. HE, BRUCE ERICKSON (KPFF CONSULTING ENGINEERS, AND DENNIS O'NEILL FROM DAMES & MOORE CAME IN ABOUT 9:45 AM. WE HAD A MEETING ATTENDED BY THOSE THREE PLUS MYSELF, BOB BENIDICTO (SR PLAN CHECKER), AND JOHN PIEROG FROM PUBLIC WORKS. DURING THE MEETING THE FOLLOWING THINGS WERE DISCUSSED: O'NEILL DESCRIBED THE REASON FOR THE EXCAVATION INSIDE THE BUILDING (REMOVAL OF CONTAMINATED SOILS). HE SAID THE SOIL RECEIVED CITY OF ruf;wIIA AUG 13 1992 WAS BEING LEGALLY REMOVED FROM THE AREA AND WOULD BE LEGALLY DISPOSED OF. JOHN TOLD THEM THEY WOULD HAVE TO GET THE APPROPRIATE PUBLIC WORKS HAUL PERMITS, POST THE REQUIRED BONDS AND INSURANCE DOCUMENTS, AND SAID HE WOULD CHECK ABOUT THE NEED FOR A LAND ALTERING PERMIT. O'NEILL SAID THEY WERE CONCERNED ABOUT BEING ABLE TO ALLOW THE OLYMPUS PRESS PEOPLE BACK IN THEIR AREA TO WORK. I SAID WE WOULD NEED A SIGNED STAMPED STATEMENT FROM THE STRUCTURAL ENGINEER INDICATING THERE WOULD BE NO DANGER TO THAT PORTION OF THE BUILDING IF THE OTHER PART OF THE ROOF WAS TO COLLAPSE. HE INDICATED THE ENGINEER WOULD GO TO WORK ON THAT IMMEDIATELY AND WE SHOULD HEAR SOMETHING BEFORE THE END OF THE DAY. O'NEILL ASKED HOW LONG IT WOULD TAKE TO GET THE PERMIT AND I TOLD HIM WE WOULDN'T BE ABLE TO GET IT ISSUED BEFORE NEXT WEEK AT THE EARLIEST. HE SAID HE DIDN'T HAVE A PROBLEM WITH THAT AND THEY COULD PUT THE PROJECT ON HOLD UNTIL THEY OBTAINED THE PERMIT. We understand that the temporary shoring system will be an addendum to our previous approved building permit. As a result of petroleum- stained soils being located beneath the interior office area, we anticipate removing a limited amount of soil beneath a structural wall located within the interior office area. The area of proposed soil removal is presented in the attached Figure. We anticipate installing the temporary shoring, removing a portion of the structural wall approximately 8 feet long, removing an area of 6 feet square of the concrete floor and removing the petroleum - stained soils with a portable trackhoe. Previous analytical results indicate that petroleum- stained soils extend approximately 5 feet beneath the concrete floor and extend to a depth of approximately 5 feet below ground surface. The petroleum- stained soils appear to be associated with a leaking floor drain located within this area. We estimate that approximately 15 cubic yards of petroleum- stained soils will be removed and transported to the Woodworth & Company Treatment facility located in Tacoma, Washington. The transportation of the soils was accounted for in our previous hauling permit submitted to your office. Following the removal of the petroleum- stained soils, the excavation will be backfilled and compacted to 95% in accordance with our geotechnical program being conducted throughout the site. Subsequently, the concrete floor will be installed, the structural wall replaced in accordance with the enclosed design and the temporary shoring removed. Dames & Moore understands your concerns regarding the potential for on -site personnel to be injured during the implementation of the work described herein. As a result of a subsurface environmental investigation previously conducted at the site, three areas associated with shallow petroleum- stained soils were identified at the site (see attached Figure). Based on analytical results obtained during our subsurface investigation, shallow soils within the three areas contained petroleum - stained soils which exceed State of Washington Model Toxic Control Act Method A cleanup levels for total petroleum hydrocarbons (TPH). Based on our evaluation of historical and current site activities, it appears that the source of petroleum hydrocarbons was a combination of surface machinery and a below grade drainage system. In accordance with Department of Ecology's (Ecology) guidelines /regulations, Dames & Moore initiated an independent cleanup program to remove the shallow petroleum - stained soils. Prior to the start of field activities, Ecology representatives were notified of the independent cleanup program. In addition, a site specific Health & Safety Plan was developed in accordance with OSHA regulations. Northwest Envirofield Services (NWES) was subcontracted to perform the excavating, shoring and backfilling activities, Initially, we anticipated the petroleum - stained soils were limited to less than 5 feet below ground surface (bgs) and limited in lateral extent. Following our initially excavation phase, petroleum - stained soils appeared to extend to a depth of approximately 10 bgs and beneath a footing support beam associated with the roofing system. At that point, a practicing license engineer specializing in shoring design from KFPP was subcontracted to design an appropriate shoring system. The designed shoring system was installed by NWES personnel. Based on the recent analytical results, we have basically removed the petroleum - stained which exceed MTCA Method A cleanup levels for TPH within the three areas of concern. Upon approval from your office, the remaining phases of field activities consists of: (1) installation of a replacement footing, (2) backfilling and compaction of the excavations, (3) resurfacing of the concrete surface and (4) transportation and treatment of the petroleum- stained soils to the Woodworth & Company, Inc. thermal desportlon /asphalt batching facility located in Tacoma, Washington. Presented below is additional information regarding the remaining field activities to be conducted at the site. OFFICLS \ \'OI(LDWII)Ii CITY OF RECEIVED AUG 19 1992 DAMES & MOORE FOOTING DESIGN AND REPLACEMENT As discussed earlier, KFPP has been retained to design a replacement footing for the footing that was disrupted during the previous excavating activities. KFPP designated project engineer has been instructed to submit the design and specification to your office for review and final approval. It is our understanding that KFPP is currently working on these items and will submit this information as soon as possible. REQUIREMENTS FOR BACKFILLING OF EXCAVATIONS A Dames & Moore geotechnical engineer has designed and will monitor the backfilling and compaction activities. The following scope of services are associated with the requirements for this project as regards to backfilling of excavations: • • • • Soils that are to be used for backfilling have been submitted and are currently being tested at Dames & Moore laboratory prior to the start of the backfilling operations. A test for laboratory compaction characteristics (i.e, a Proctor compaction test) will be conducted on each significantly different type of soil to be used. Testing will be in accordance with ASTM D -698. Backfill soils will be compacted in 8 to 12 -inch lifts using either hand - operated compaction equipment (e.g. gasoline powered vibrating plate compactor) or mechanized equipment that Is appropriate to the size and location of the excavation (e.g. backhoe- mounted HoPac; walk - behind roller). Where backfill soils are used to support a floor slab or parking pavement, the upper 3 feet will be compacted to at least 95 percent of the maximum dry density of the backfill soil. Soils greater than 3 feet below the bottom of the slab /pavement will be compacted to at least 92 percent of the maximum dry density. The bottom of the excavation will be free of debris and standing water prior to the start of compaction. Based on our previous environmental subsurface investigations, ground water is anticipated to exists at a depth greater than 40 feet bgs In the site vicinity. The subgrade receiving the initial lift of fill soil will be firm and free of loose or soft soil or organic materials. For the location where the existing 4 -foot deep footing for roof support has been removed and will be replaced, there are two alternatives for footing replacement. The first alternative is to construct the footing on the natural ground surface at the base of the excavation. In this case the backfill can be placed as recommended above. The second alternative is to place the new footings in the compacted fill at the 4 -foot depth. In this event, all fill placed beneath the footing and its zone of influence will be compacted to at least 95 percent of the maximum dry density. The zone of influence of the footing extends from the bottom edge of the footing downward and outward (away from the footing edge) at an angle of about 45 degrees from the horizontal. In situ density tests will be conducted during the fill compaction process to ensure that the fill soil is being compacted in accordance with the density requirements set forth above. These tests will be conducted by Dames & Moore using a nuclear densometer. For excavations with a maximum dimension of about 40 feet or less, the testing frequency will be one test for every 2 to three lifts. Test locations within the excavation will vary and may be selected at random. Additional compaction will be required where tests do not achieve the required density. Dames & Moore may waive the density requirement where visual inspection of the compacted soil indicates that an adequate degree of compaction is being achieved. RECEIVED CITY OF TUKWILA AUG 1 3 199 DAMES & MOORE CONCLUDING STATEMENT TRANSPORTATION AND TREATMENT OF PETROLEUM- STAINED SOILS Approximately 300 tons of excavated soils are currently stockpiled at the site. A temporary storage facility was constructed which consists of the soils stockpiled on and covered with visqueen sheeting and surrounded with balls of hay. Multiple sand bags have been placed on top of the visqueen to inhibit the sheeting from being disrupted. The petroleum - stained soils are scheduled to be transported and treated at Woodworth & Company thermal desportion /asphalt - batching facility located in Tacoma, Washington. The hauling will be provided by NWES. We anticipate the majority of the soils can be removed in one day and thus eight trucks (two trips each) will be employed to provide this service. A map showing the designated hauling route is attached for your review. Dames & Moore objective is to conduct this investigation in a safe and professional manner in accordance with local, state and federal guidelines /regulations. We understand our subcontractor was proceeding under the assumption that the site is located in the City of Seattle Jurisdiction. Consequently, the appropriate permits were not obtained from your office. We trust this information as well as the Information recently submitted by KPFF meets your current requirements. MONITORING /INSPECTION NORTHWEST MOTOR WELDING TUKWILA, WASHINGTON Job No. 16373-012-008 October 1, 1992 Presented herein Is the report of geoteohnloal monitoring and Inspection of backfliling and foundation preparation activities following removal of petroleum - stained soils from three areas of environmental concern at this site. Our report of recommendations regarding foundation design and construction and compacted fill soil placement was Issued on August 25, 1992. A Dames & Moore geotechnical representative was on -site during all backfllling operations, which extended from August 27 to September 10, 1992. During that period we provided advice on removal of unsuitable fill soils, selection and compaction of fill soils, selection of compaction equipment and methods, preparation of footing and slab subgrades and other matters of a geotechnical nature. We also conduoted laboratory compaction tests on proposed fill soils, conducted in -situ density tests using a nuclear densometer and used other methods to assess the adequacy of the compacted fill. Excavation and /or placement of compacted fill soil occured at three locations identified as Excavations #1, #2 and #3. Excavation #1 was the largest and deepest, and contained the replaoment footing for roof support. Soft unsuitable fill soils were removed from this excavation down to either firm fill soils or to the dense, undisturbed glacial till which was encountered at depths up to 12 feet below the ground surface. At the footing location, the undisturbed till was exposed, The bottom of the footing was set at a depth of approximately 8 feet, which was roughly 4 feet above the bottom of the excavation. Fill soil beneath the footing and within the surrounding zone of influence of the footing consisted of a mix of crushed stone combined with pit run sand and gravel (called "Mix 117" by Woodworth, the Tacoma supplier) This soil was compacted to at least 95 percent of the maximum dry density. Other fill soils In this excavation as well as the other two excavations consisted of recycled concrete as well as the Mix 117, Compaction was provided either by a manually operated vibrating sled or a backhoe - mounted "Hoepaok". The fill was placed In lifts that were typically 10 to 12 inches thick. Fill placed within the zone of influence of the footing and within 3 feet below the bottom of the slab was compacted to at least 95 percent of the maximum dry density. Fill at other locations within the excavations was compacted to 92 percent. SENT BY : N. W. E. S. Excavation operations will be confined to interior site areas and the excavated material will be inherently moist, and prevent or reduce the potential for dusting. The contaminant is a petroleum product of significant viscosity which will also reduce or eliminate dusting or particulate migration. (As a control measure to reduce dusting, light water /agent spray mists may be used.) Erosion control will prevent the environmental impact of water runoff on various areas of the site, the following steps will be implemented. Plastic sheeting will prevent rainwater penetration and absorption to existing stockpiles and protected areas.
