APPLICATION SUBMITTAL In order to ensure that your application is accepted for plan review, please make sure to fill out the
application completely and follow the plan submittal checklist on the reverse side of this form. Handouts are available at
the Building counter which provide more detailed information on application and plan submittal requirements.
Application and plans must be complete in order to be accepted for plan review.
VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of
Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting
application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is
subject to possible revision by the Building Division to comply with current fee schedules.
BUILDING OWNER / AUTHORIZED AGENT ff the applicant is other than the owner, registered architect/engineer, or contractor
licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this
permit application and obtain the permit will be required as part of this submittal.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall
expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180
days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition).
No application shall be extended more than once.

Dear Mrs. Jensen,
In response to a final inspection request on 12 -9 -92,
which resulted in a list of five corrections, I have
contacted certain individuals in an attempt to piece
together a scenario of what construction practices took
place on your retaining wall project. You reported that you
are now handling the project since your husband is reported
to be in a treatment hospital and is not available for
comment. You also stated that the both of you are separated
and that your knowledge of the project is quite limited.
The first correction item pertains to the retaining
wall being constructed without benefit of inspection. You
reported that a contractor, Mel Estegoy 722 -7517, actually
did that portion of the project. I was able to contact his
wife and she indicated that he was not a contractor but did
help on the wall along with many others. She also said that
Mel was in the hospital and in a coma. She commented on how
the well had a "ton of rebar" in it. In our file, we have a
letter from Gary Linkous, an employee at Star Sheet Metal
where Randy Jensen is also employed. He states that the wall
was built in conformance with the approved plans. He was one
of the helpers on this project. Short of requiring
additional special testing or tearing the wall down, the
above information will serve as a basis for conditionally
accepting the wall.
The second correction item, construction of a deck and
stair, is of concern only where the stair is involved since
the deck is under 30" above grade. The stair is acceptable
per code except for absence of a grippable handrail.
Guardrail spacings will not be applied since the stair is
not a part of the house or this permit.

according to our Public Works inspector, G. Villanueva. We
don't have that code in our office for actual verification
to you of this requirement nor do we have enforcement .
capabilities for their regulations. You would be best
advised to look into this requirement for your own
protection.
Correction item #4 raises concern over the material
used in the construction of the deck and stairs. The wood
does not appear treated but could be cedar. Nevertheless,
since . it does not pertain to this retaining wall project as
far as what was permitted, and it is exempted from permit
per section 301(b) of the U.B.C., we will let
you, the owner, assume all responsibility for its
construction.
Correction item #5 pertains to illegal electrical work,
meaning that it was performed without benefit of a permit or
inspection. The work is minor in nature and affects the deck
and stair project, not the retaining wall. This issue should
be handled through the Dept. of Labor and Industries. Their
number is 248 -6630.
At this time, we are approving the final of the
retaining wall project. The concerns mentioned above which
remain outstanding do not fall under the scope of this
permit. It would be advisable for you to act upon them on

10. Any new ceiling grid and light fixture installation is required to meet lateral
bracing requirements for Seismic Zone 3.
11. Partition walls attached to ceiling grid must be laterally braced if over eight
(8) feet in length.
12. Readily accessible access to roof mounted equipment is required.
13. Engineered truss drawings and calculations shall be on site and available
to the building inspector for inspection purposes. Documents shall bear
the seal and signature of a Washington State Professional Engineer.
14. Any exposed insulations backing material to have Flame Spread Rating of
25 or less, and material shall bear identification showing the fire
performance rating thereof.
15. Subgrade preparation including drainage, excavation, compaction, and
fill requirements shall conform strictly with recommendations given in the
soils report prior to final inspection (see attached procedure).
16. A statement from the roofing contractor verifying fire retardancy of roof
will be required prior to final inspection (see attached procedure).

materials shall bear identification showing the fire performance rating
thereof. Such identification shall be issued by an approved agency
having a service for inspection at the factory.
20. Notify the City of Tukwila Building Division prior to placing any concrete.
This procedure is in addition to any requirements for special inspection.
21. All spray applied fireproofing, as required by U.B.C. Standard No. 43 -8,
shall be special inspected.

During the building of the forms, reinforcement with rebar, and the actual
pouring of the concrete, I was present and can state with assurance that there is
supporting rebar in the concrete retainer wall that was poured recently.
