INSPECTOR: Craig Kuntz 11 o w 4 -3 -01 v 0 z MTE inspector arrived on site to monitor the adhesive anchoring in the 2n floor interior beam seat repair at the southeast, south center and southwest area. The contractor epoxied #3 bar per faxed sheet SK -1 per epoxy manufacturer's recommendations. Simpson Strong Tie ETF (high strength and fast curing epoxy was used). The Simpson epoxy is approved per general notes sheet S -600. The contractor also epoxied %s" diameter all thread bolts with Simpson Strong Tie ETF in the spandrel beam repair at the southeast entrance. Bolts were installed per manufacturer's recommendations and sheet SK -3 dated 3 -28 -01 MTE inspector arrived on site to monitor the adhesive anchoring in the spandrel beams at the southeast and south center areas. Bolts were installed using Simpson Strong Tie ETF (high strength and fast curing) epoxy per manufacturer's recommendations and SK -3 revised on 3- 28 -01. The beams in the south center area were not all completed and will be inspected on a later date. The contractor also installed #3 bar (using the Simpson epoxy) in the beam seats on each side of the southwest entrance. Bars were installed per SK -1 and manufacturer's recommendations. To the best of our knowledge, items inspected are in accordance with approved plans and specifications. INSPECTOR: Craig Kuntz 4-4 -01 MTE inspector arrived on site to monitor and sample the grout placed in the pour backs for the beam seat support along the southern interior of the building. Reinforcement was per SK -1. The contractor mixed Sika Monotop 611 grout on site for the pour backs. The grout was mixed per manufacturer's recommendations stated on the bag. Sampled the concrete with slump approximately 7 ' /z', grout temperature 60 degrees and cast one set of four 4 "x8" cylinders for strength verification. Resteel for the 3'x14'6" slab on grade entry to freezer room/test chamber was inspected for size, grade and clearance and found per plans. The contractor placed 1 cubic yards of Miles Sand & Gravel Mix #15NWA3A in the above locations using a wheelbarrow and mechanically consolidated. Sampled the concrete with slump 4 % ", concrete temperature 53 degree, entrained air 5.3% and cast one set of four 6 "x12" cylinders for strength verification. Observed the installation of a total of 10 Y2" all thread anchor bolts with a minimum embedment of 4 Y2" to 6 ", depending on location, at the south side of the building underside or roof on 13 /H -J. the contractor is using Simpson Strong Tie epoxy ETF Fast Cure epoxy per manufacturer's recommendations. The contractor prepared 5/8" diameter anchor bolt holes per manufacturer's recommends. To the best of our knowledge, items inspected are in accordance with approved plans and specifications. Seattle, WA 98138 Sincerely, CONSULTANTS • DESIGNERS • ENGINEERS • CONSTRUCTORS Subject: Repair for Localized Structural Damage to Building #745 Due to 2/28/01 Earthquake Dear Mr. Thoreson, Attached are sketches and supporting calculations required to repair the following items: 1. Concrete damage to Glu -Lam beam — to — concrete pier connection. A field walkthrough showed obvious damage to piers at column lines B -13, H -13, and K -13. We recommend the attached fix be applied to the balance of beam /pier connections of this type in case any were missed in the inspection. These locations include (in addition to the ones already mentioned) 8-13, J -13, and S -13. All locations are on the south wall of the building. (See ref. Dwg. S-4 attached.) The repair fix submitted does not qualify the building or any part of the building to meet current building codes. That is beyond the scope of this submittal. 2. Structural damage to pre -cast concrete spandrel beam connections to concrete piers. This is a generic fix to be applied to all spandrels. Preliminary field investigation was done on spandrels located between columns K -13 and H -13. We recommend this fix be applied to all spandrels. HIT Adhesive Anchor System: General: The Hilti HiT HY-150 and Hilti HIT HY -20 consist of a two- component resin adhesive in two foil cartridges separating the resin from the hardener. The foil pack- ages are attached to an opening and dispensing manifold. An auger -style nozzle is attached for proper mixing of the adhesive components. The HIT HY -15015 for use with rebar sizes No. 3 through No. 11, threaded rod sizes 3/4-inch through 1 -inch diameter or HFA anchors with 3 /e•inch through 3 /4 -inch internal thread diameter for anchoring into normal - weight con- crete, lightweight concrete or grout - tilled block. The threaded rods con- form to ASTM A 36, ASTM A 193 Grade 87, ASTM F 593 or Alloy Group 1 Type 304 condition CW. The HFA flush inserts conform to AISI 12L14 material. The HIT HY -20 is used with a galvanized metal screen and all - thread rods of 3 /8-inch through 3/4 -inch diameter to anchor into clay brick masonry. Steel reinforcement dowels are No. 3 through No. 11 conform - ing to ASTM A 615 Grade 60. 8. Installation: The anchor is Inserted into a predrilied hole that has been cleaned with a wire brush or water so as to be tree of dust, debris and standing water. The hole is drilled to the embedment depth specified in the appropriate tables. For clay brick masonry. the galvanized metal screen tube is completely failed with HIT HY•20 adhesive and inserted into the predrilled hole. The threaded rod is inserted into the adhesive -filled metal screen tube, turning slightly to ensure the complete bonding while pushing the anchor to the bottom of the screen. For concrete, Lightweight concrete and grout - filled masonry units, the predrilled hole is titled one - half full with HIT HY -150 adhesive. The threaded rod, reinforcement dowel or HFA flush insert is then inserted into the hole, turning slightly to ensure the complete bonding while pushing the anchor to the bottom of the hole. The anchor should not be disturbed until completely cured. The curing times and respective base material temperatures for the adhesive mix- tures are in Table 4. Dimensional and installation criteria are in Tables 1 and 2. Allowable tension and shear values are in Tables 5 through 10. . Allowable Toad values are influenced by the temperature of material into which anchors are installed. (6 mm) substrate profile and 3/4 in. (20 mm) clearance behind corroded reinforcing steel. The perimeter of the area to be patched should be sawcut to a minimum depth of 1 In. (25 mm) to prevent featheredges. After concrete removal and prior to placement, mechanically abrade the concrete sur- face to remove all bond - inhibiting materials from the concrete substrate and to provide additional mechanical bond. Substrate should have a minimum amplitude of 1/4 in. (6 mm). Limit the size of chipping hammers to 151b. (6.8 kg) to reduce micro fractures. C. Square cut or under cut perimeter of the area to be repaired to a minimum depth of 1 in. (25 mm). Do not cut existing steel reinforcement. D. Where reinforcing steel with active corrosion is encountered, comply with the following: 1. Abrasive blast reinforcing steel to remove rust and contaminants. 2. If half or greater of the diameter of the reinforcing steel is exposed, chip out behind the reinforcing to a 3/4 in. (19 mm) minimum depth. 3. Splice new reinforcing steel to existing steel where corrosion has depleted the cross - section area' by 25 %, as directed by the Architect/Engineer: .
