Handouts are available at the Building counter which provide more detailed information on application and plan submittal requirements. Application and plans must be complete in order to be accepted for plan review. VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of Community Development prior to application submittal. Contact the Permit Coord!nator at 433 - 1851 prior to submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is subject to possible revision by the Building Division to comply with current fee schedules. BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architeci/engineer, or contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the permit will be required as part of this submittal. EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition). No application shall be extended more than once. BCS Representative A BCS Facilities representative is assigned to each project, and is your sole initial contact for all job related activities. Notifications of any utility work and /or shutdowns required of short duration only (less than 4 hours) must be made 24 hours in advance to the BCS Facilities representative and BCS Plant ServIces In BCS owned buildings, Longer outages must be arranged two weeks in advance with the BCS Facilities representative. Emergencies in BCS owned buildings shall be reported to BCS Plant Services at 865 -5351 and a 24 hour Customer Service number is maintained at 865 -5422. Emergencies in leased buildings shall be reported to the building management. Contact the BCS representative for the correct telephone number prior to starting work. 2. Permits Unless otherwise noted, BCS will obtain and pay for the building permit. The contractor is responsible for arranging all inspections and for obtaining and paying for all other permits and bonds required for the work. 3. Coordination Coordinate work activities through the BCS Facilities representative that involve tenants' activities and other contractors which may be engaged in the work area or along access routes to the work area. 4. Schedule Contractor to submit a construction schedule for all projects over 550,000 in value, within a week of notification to proceed. The schedule will show coordination of all trades and types of work required with start and completion dates. 5. Submittals/Correspondence Submit catalog cuts and technical data for approval of all items of mechanical and electrical equipment, switchgear, hardware schedules and plumbing fixtures prior to ordering to BCS Facilities representative. No substitutions for specified items will be allowed, unless approved by the BCS Facilities representative prior to submittal of bids. Address: 8C5 Facilities, Attn: Representative's Name. P.O. Box 24346, MS 7R -32, Seattle, WA 98124.0346. (206 -865 -7701) 6. Traffic On -Site Observe speed limits, directional signs and pedestrian right -of -way at crosswalks. All construction material traffic shall be directed to receiving/loading docks and unloaded through service doors only. 7. Parking Workers must park personal vehicles in unmarked or "compact" parking spaces only. Loading areas are to be used for pick -up and delivery only. Contractor's service vehicles required for continual material re- supply may use parking spaces marked "service vehicles only." Do not park in driveways, handicapped stalls, fire access lanes, or marked spaces not mentioned above. 8. Access Building access cards are available when necessary to contractor foremen for access through service doors during the construction period. Submit a completed temporary access authorization form available from the BCS Facilities representative. All contractor personnel must obtain temporary badges at the lobby reception counters for access through supervised building lobbies and for identification on -site at all times. Badges must be worn chest high so that they can be seen. Service doors shall remain locked at all times, except when moving construction materials into or out of areas, and then the cardholder must be present to monitor door access. Cardholders are responsible to see that service doors are relocked after they use them. 9. Safety Comply with applicable sections of State safety standards WAC 296 -24 and 26, 296.44, 62, 95, and 155, in addition to other state and local code requirements. Prior to the use of any chemicals such as solvents, cleaners, concrete tratments, adhesives, etc., having a Material Safety Data Sheet (MDSA) per OSHA Standards and Interpretations 1910.1200 change 34 and 37. Sheets must be submitted to the BCS Facilities representative for review by the SCS Facilities Engineering Services and BCS Safety Manager. Request a copy of The Boeing Company Instruction # 9. 4150.04 for any work required in "confined spaces" with hazardous atmospheres 2/14/90 • 10. Smoking The Boeing Company's "no smoking policy" requires that all persons refrain from smoking within Boeing owned or leased buildings. 11. Food Service Cafeterias and vending machines (if available within the building in which the work area is located, or an adjacent building) may be used by contractor personnel. 12. Utilities Existing on site utilities may be used by outside contractors, subject to the approval of the BCS Facilities representative as follows: Toilet facilities (nearest to the work area) Water (if available within the work area, or with reasonable distance) Power (120V, 20A max) Local telephone (if available within the work area, and only without interference with tenants' activities) 13. On -Site Storage Limited quantities of materials and equipment required for the work may be suitably stored in designated areas only. Avoid storage in hallways, aisles, and circulation areas except for short periods when necessary. BCS will not be responsible for damage to or loss of construction equipment, or tools left on the job site. 14. Protection/Clean -up Protect ceilings, walls, doors and floor surfaces from construction dirt, cart wheels, and foot traffic. Contractor shall repair damaged surfaces at his expense. Protection of floor shall be non-skid. Protect existing disconnected computers, electronic equipment, furniture, etc., which are to remain in place from dust and construction dirt, with plastic sheeting sealed with tape. Relocation of furniture and equipment out of the way of construction activity must be coordinated with the 8C5 Facilities representative. Keep work areas clear of construction debris during the progress of the work, and provide a final broom cleaning of the area at the conclusion of the work. Dispose of all construction waste off -site, at authorized disposal sites. 15. As -Built Drawings/Manuals Furnish a marked -up copy of the job drawings showing as -built conditions and notes based on data collected in the field for any work installed other than as shown on the drawings to the BCS Facilities representative, along with three copies of equipment operating and maintenance manuals prior to submittal of the final invoice. 16. Extra Work By submitting a bid, the contractor acknowledges that he has visited the site of the work seen the conditions to be encountered, and has reported any discrepancies noted. Work required for the job but not shown on the documents issued will be authorized only by written Facilities Work Order or Coordination Sheet issued by the BCS Facilities representative prior to the work being performed. No other claims will be recognized. 17. Warranty The prime contractor will warrant to BCS that he is fully licensed to undertake the work, and warrants all Contractor furnished equipment and material to be new, of first quality and in addition to labor furnished, be warranted for a minimum period of one year from date of final acceptance. 18. Payments Submit invoices for the work promptly on a monthly basis with the Facilities Work Order number shown, to BCS Facilities Funding, Attention Carolyn Crossland, P.O. BOX 24346, MS 7R -33, Seattle, WA 98124 -0346, (206 -865- 7612). Bids exceeding 5100,000 will be subject to a ten percent retainage, payable after final acceptance. 19. Insurance Contractors engaged in the work shall carry and maintain comprehensive general liability and property damage insurance, vehicle liability, and property damage insurance, and workmen's compensation insurance. Certificates of insurance may be requested. 20.
