APPLICATION SUBMITTAL In order to ensure that your application Is accepted for plan review, please make sure to fill
nut tha application completely and follow the plan submittal checklist on the reverse side of this form. Handouts
are available at the Building counter which provide more detailed information on application and plan submittal
requirements. Application and plans must be complete in order to be accepted for plan review.
VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of
community D uelopm9nt prior to application submittal. Contact the Permit Coordinator at 433 - 1951 prior to
submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be
reviewed and is subject to possible revision by the Building Division to comply with current fee schedules.
BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or
contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent
to submit this permit application and obtain the permit will be required as part of this submittal.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of
application shall expire by limitations. The building official may extend the time for action by the applicant for a
period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform
Building Code (current edition). No application shall be extended more than once.

Tremana Inc., mover, and Jan -Wes Homes Inc., owner, for and in
consideration of the sum of $10.00 consideration in hand hereby contract and
agree to allow Tremana Inc. and its agent Jackie Dempere to move the home
structure located at 14951 57th Avenue South in Tukwilla, King County,
Washington subject to the following restrictions and limitations:
1. STRUCTURE TO BE MOVED: Mover will remove and relocate home
structure currently situated at the above address excluding the garage; if any,
and the add -on one -story portion of the rear of the main home structure.
Mover will not fill -in, be liable for or otherwise modify in any manner the
basement area that remains after the home structure is removed. Mover will
not be responsible for removal, stability or demolition of any structures that
remain on the above property after the home structure is removed.
2. HOME REMOVAL PROCESS: Mover will retain the services of a licensed
and bonded agent experienced in the process of moving homes to facilitate
the relocation of the structure indicated in number 1 above. Mover will be
allowed to have reasonable access to the structure during the term of this
contract to prepare the structure for moving.
3. TIMING: Owner agrees to give the mover 30 -days written notice of the
deadline for removing the structure described in number 1 above. Upon
receiving said notice mover will have 30 -days to effectuate the move. If the
move is not effectuated within the 30 -days this contract will expire by its
terms. Mover shall have unlimited access to the property during the entire
30 -day period, and will be allowed to disconnect the structure from all
services during this period. If the structure is rented to a third -party or other
individual owner agrees to give mover 30 -days written notice to remove the
structure from the time that the tenant vacates the premises. Time will be
calculated in accordance with the procedure described in Washington
Superior Court Civil Rule 6(a).
4. INDEMNIFICATION: Mover agrees to indemnify, release and hold owner
harmless from liability and costs associated with the mover carrying out the
activities described herein.
