Current copy of Washington State Department of Labor and Industries Valid Contractor's License. If not available at the time of
application, a copy of this license will be required before the permit is issued OR submit Form H-4, "Affidavit in Lieu of Contractor
Registration ".
Building Owner /Authorized Agent: If the applicant is other than the owner, registered architect/engineer, or contractor licensed by the
State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the
permit will be required as part of this submittal.

Expiration of Plan Review - Applications for which no permit is issued within 180 days following the date of application shall expire by
limitation. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written

Floor plan and system layout
Roof plan required to identify individual equipment and the location of each installation (Uniform
Mechanical Code 504 (e))
Details and elevations (for roof mounted equipment) and proposed screening
Heat Loss Calculations or Washington State Energy Code Form #H -7
H.V.A.C. over 2,000 CFM (approximately 5 ton and larger) must be provided with smoke detection shut-
off and will be routed to the Fire Prevention division for additional comments (Uniform Mechanical
Code 1009).
Specifications must be provided to show that replacement equipment complies with the efficiency ratings
and other applicable requirements of the Washington State Nonresidential Energy Code.
Structural engineer's analysis is required for new and the replacement of existing roof equipment
weighing 400 pounds and greater (Uniform Building Code 1632.1). Structural documentation shall be
stamped by a Washington State licensed Structural Engineer.

amended, Uniform Mechanical Code

12: Commercial -type food heat - processing equipment from which grease -laden vapors emanate in normal cooking application shall be
protected by an approved automatic
extinguishing system. The extinguisher system shall be interconnected to the fuel and current supply so that the fuel or current is
automatically shut off to
all equipment under the hood, when the system is actuated. (UFC 1006)
13: A wet chemical portable fire extinguisher having a minimum rating of 2A:1B:C:K shall be installed within 30 feet of commercial
food heat - processing equipment,
as measured along an obstructed path of travel. (UFC 1006.2.7).
14: ** *FIRE ALARM SYSTEMS * ** - City Ordinance #1646 - NFPA 72
15: A fire alarm system is required for this project. The fire alarm system shall meet the requirements of N.F.P.A. 72 and City
Ordinance #1900.
16: The installation of wiring and equipment shall be in accordance with N.F.P.A. 70, Article 760, Fire Protective Signaling Systems.

Before you open for business, you must complete the enclosed application for a permit and return
with the correct fee of ($318.00) for a (6301) permit. If you open before you obtain your permit,
your permit fee will be double.
Before you open you need to schedule a pre - operational inspection by the Health Department.
Although your application for a food service establishment permit from Public Health Seattle and
King County will be approved during this inspection, you may need to obtain additional permits
or approvals from other agencies. It is the responsibility of the food service establishment
operator /owner to obtain all necessary permits and approvals. Operating the establishment
without these required permits or approvals may subject you to legal action by the appropriate
agencies. If you open without health inspection, you may be closed. Once your plumbing permit
has been finalized, contact me at (206) 205 -1903 to schedule the
pre- operational inspection. Failed pre - operational inspections will require a $100.00 fee for a

permit applications. It explains and organizes information needed by the Department of Construction and Land
Use to efficiently review plans and issue permits. DCLU will keep this document as part of the permanent project

I. Makeup air

3. Duct enclosures shall be separated from the duct by at least 3 and not more than 12 inches. Proposed in.

through a weather- protected opening.

L. Multiple hood venting

A single duct system may serve more than one hood located in the same story of the building, provided
that the interconnecting ducts do not penetrate any fire construction.

6. Centrifugal fans used for discharging grease exhaust shall be positioned in a bottom horizontal
discharge position only. A duct that diverts the fan discharge shall not exceed 3 times the diameter of
the fan outlets connected to the fan outlet. The duct shall be provided with an adequate drain opening
at the lowest point to permit drainage of grease to a suitable collection device.
7. Fire extinguishers. Shop drawings shall be submitted to DCLU for Fire Department approval prior to field
installation.
