Box 9195 Ogden, UT 84409 RE: Sizzler Restaurant - Dining Room Addition (Control No. 88 -096) Dear Don: Per our telephone conversation on April 21, 1988 you identified that the square footage of the work to be done for the new dining room addition /canopy is 1,238 square feet, and also the square footage of the entire building is 6,137 square feet, including the addition. Per the sidewalk ordinance for the City of Tukwila, the additional square footage of floor space is over 20% of the square footage of the original building structure and therefore the sidewalk requirement for the construction of sidewalks by this development fronting Southcenter Parkway is required as part of the development. Requested is four sets of site plans for the proposed sidewalk construction. Per our discussion, you may submit a preliminary design proposal for this sidewalk construction, along with a schedule and a construction estimate for this sidewalk, and a request to place 150% of that estimated cost with the City until its design and construction is complete. A schedule shall accompany this proposal which shall go no longer than 90 days after the City approves the building plan. Enclosed you will find a copy of the utility permit application, a copy of the sidewalk ordinance and a copy of the City's Standard Sidewalk Plans for use in developing your application for this sidewalk submittal to the City. The total number of fire extinguishers required for your establishment is calculated at one extinguisher for each 1,500 sq. ft. of area. The extinguisher should be of the "all purpose" (2A, 20 B:C) dry chemical type. The travel distance to any extinguisher must be 75' or less. (NFPA 10,3 -1.1) (UFC 10.301) Extinguishers shall be installed on the hangers or in the brackets supplied, mounted in cabinents, or set on shelves (NFPA 10, 1 -6.6), and shall be installed so that the top of the extinguisher is not more than 5 ft. above the floor. (NFPA 10, 1 -6.6) (UFC 10.301) Extinguishers shall be located so as to be in plain view (if at all possible), or if not in plain view, they shall be identified with a sign stating, "Fire Extinguisher," with an arrow pointing to the unit. Construction not specifically shown, shall be accomplished per minimum requirements of UBC. Contractor shall compere all dimenslona and conditions on contract documents and at the site. Any oa iseion or conflict shall be brought to the attention of the architect aced /or engineer. In case of any conflict follow the most stringent requirement as dlrected by architect and /or engineer. Contractor shall coordinate with all trades all items that are to be integrated into the structural system. Order of construction to be responsibility of the contractor. He shall provide all items necessary for his chosen procedure. Until all permanent members, including walls, slab*, floors & roof are in place and all cosutections are completed, stability of structure and all parts thereof shall be Contractor's responsibility. During construction contractor shall keep construction loads within the design floor and roof load limits shown on drawing. After construction is completed building manager shall keep loads on roof and floor within design limits shown on drawings. Any deviation from contract documents shall be approved by architect and shall be so docuatented. This documentation shall bacons part of contract documents. Deviation from contract documents shall not be allowed on shop drawings. Contractor ehall review and verify all shop drawings to assure they comply with requirements of the contract documents. Engineer will review the shop drawings for general conformance with design concept. This review by the engineer shall not be construed as approval. Entire building area shall be *scraped to remove 4 inches of top soil including vegetation and debris. All soft spots, man placed non structural fille, voids end coaeetructio n debris shall be removed and replaced with compacted structural fill. .No concrete shall be placed on muddy or frozen soil. All footings and slabs to be on firm undisturbed soil. Natural undliatur'oed eon below all footings *hall be proof ro.11ed prior to placing concrete. Remove all soft spots found and replace with compacted structural fill, 8xtend fill beyond edge*+ of the footings equal to depth of the fill. Structural fill material shall be a well graded granular material with i axite i size lees than 4 inches and with eot more than 12% passing a number 200 sieve. It shall be compacted in 8 inch lobs* layers to 95% of the maximum dry density based on ASTM D 1557. All etruatural fill shall be toasted. Unless, noted otherwise, top of all exterior footings shall be 12 inches below adjacent finish grade and top of all interior footings shall be 8 inches below adjacent finish floor. All footings are proportioned and designed for 1600 PSF soil bearing. All exterior type backf i l le that will support any structure (slab on grade, stairs, etc.) shall be structural compacted fill. All concrete shall have a compressive strength equat .to !*semi within 28 day* after placing. The owner will hire a testing laboratory to test all concrete. Submit a copy of test reports for review.
