2: All mechanical work shall be inspected and approved under a separate permit issued by the City of Tukwila Permit Center
(206/431- 3670).
3: All permits, inspection records, and approved plans shall be at the job site and available to the inspectors prior to
start of any construction. These documents shall be maintained and made available until final inspection approval is
granted.
4: Truss shop drawings shall be provided with the shipment of trusses delivered to the job site. Truss shop drawings shall
bear the seal and signature of a Washington State Professional Engineer. Shop drawings shall be maintained on the site
and available to the building inspector for inspection purposes.
5: All construction shall be done in conformance with the approved plans and the requirements of the International
Building Code or International Residential Code, International Mechanical Code, Washington State Energy Code.
6: Structrual Observations in accordance with I.B.C. Section 1709 is required. At the conclusion of the work included in
the permit, the structural observer shall submit to the Building Official a written statement that the site visits have
been made and identify any reported deficiencies which, to the best of the structural observer's knowledge, have not
been resolved.
7: Fire retardant treated wood shall have a flame spread of not greater than 25. All materials shall bear identification
showing the fire performance rating thereof. Such identification shall be issued by an approved agency having a service
for inspection at the factory.
8: There shall be no occupancy of a building until final inspection has been completed and approved by Tukwila building
inspector. No exception.
9: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap the sanitary
sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks, wells, and other
excavations. Final inspection approval will be determined by the building inspector based on satisfactory completion of
this requirement.
10: Manufacturers installation instructions shall be available on the job site at the time of inspection.
11: All construction noise to be in compliance with Chapter 8.22 of the City of Tukwila Municipal Code. A copy can be
obtained at City Hall in the office of the City Clerk.
12: Ventilation is required for all new rooms and spaces of new or existing buildings and shall be in conformance with the
International Building Code and the Washington State Ventilation and Indoor Air Quality Code.
13: All plumbing and gas piping work shall be inspected and approved under a separate permit issued by the Cityof Tukwila
Permit Center.
14: All electrical work shall be inspected and approved under a separate permit issued by the City of Tukwila Building
Department (206- 431- 3670).

A limited inspection of the above property was performed on April 22" 2008. The sole purpose
is the investigation of the fire damage to the structure. Dave Pioli, P.E., performed this
inspection.
This inspection is limited to observations made from visual evidence. No destructive or invasive
testing was performed. The report is not to be considered a guarantee of condition and no
warranty is implied. This inspection and report have been conducted in compliance with the
Standards of Practice of the National Academy of Building Inspection Engineers. As
Professional Engineers, it is our responsibility to evaluate available evidence relevant to the
purpose of this inspection. We are not responsible for conditions that could not be seen or were
not within the scope.
All directions (left, right, etc.) are taken from the viewpoint of facing the front of the building, or via
north, south, east, and west.
Our evaluation of this structure is based on many indirect observations. We cannot see some of
the framing We look for cracks, bulges and other evidence of distress or deterioration to help us
evaluate the condition. As with any limited inspection, it is possible there are structural
deficiencies that cannot be known.
This house is a single family, 2 -story building with wood on the exterior walls. As we
understand this building was constructed in the 1980's.
OBSERVATIONS
There is obvious fire damage to the framing around the chimney chase at the upper level. At the
time of the inspection, much of the wallboard around this damaged area had been removed.
We are not commenting on upgrading the chimney assemble to modern code, such as the
LICEfl9E�cy of the existing chimney flue or boiler exhaust, presence of fire stops, or similar details.

CONCLUSIONS AND RECOMMENDATIONS
The plan of repair is to remove and replace fire damaged wood like with like.
No additional engineering design is warranted. This is an easy project for any competent framer.
Refer to attached sketch for area generalized area of damage. It is reasonable to assume that the
City of Tukwila code enforcement officer will want to inspect the completed repairs.
We do recommend that additional wallboard be removed (beyond what was removed at the time
of our visit). This is intended to expose all of the damaged wood. For example the top plate of
the stud wall between the hallway and Bedroom 2 should be exposed to check for fire damage.
Also the beam running in the north south direction in the centre of the Master Bedroom should
be exposed to check for fire damage.
This report is the complete response to the inspection and should be read in full. It supersedes

DAVE PIOLI, P.E.
Dave Pioli is a licensed Civil Engineer in Washington. He provides inspection services and
specializes in the area of structural evaluations and environmental issues for residential and
commercial buildings.
Qualifications
Mr. Pioli is the Principal of Criterium —Pioli Engineers, a private engineering firm that specializes
in building inspections. He has inspected thousands of residential and commercial buildings in
the greater Seattle area. Evaluations have included structural soundness, Capital Needs
Assessments, cost estimates for repairs or replacements, design plans of repair, as well as
hazardous materials, slope stability, and safety. Additional projects include Comprehensive
Needs Assessments for multi - family housing, fraternity /sorority houses and funeral homes.
Mr. Pioli also has extensive experience as a residential remodeling contractor.
Prior to his affiliation with Criterium —Pioli Engineers, he was the sole proprietor of
Environmental Engineering where he was responsible for the design of methane gas studies for
construction near abandoned landfills, and preparation of permit and construction drawings for
topsoil manufacturing.
Mr. Pioli was employed with EMCON as a Project Engineer for the design of several solid waste
landfills and hazardous waste projects. He also served as Project Manager for the design and
construction of recycling facilities.
Previously, he was Construction Manager for Glacier Environmental Services, Inc. He was
responsible for the removal of underground storage tanks and environmental clean up projects.
He also served as environmental technician for the installation of ground water treatment
systems.

In reviewing our current records the above noted permit has not received a final inspection by the City of Tukwila Building Division.
Per the International Building Code and/or the International Mechanical Code, every permit issued by the Building Division under the
provisions of this code shall expire by limitation and become null and void if the building or work authorized by such permit is not
commenced within 180 days from the date of such permit, or if the building or work authorized by such permit is suspended or
abandoned at any time after the work is commenced for a period of 180 days.
This inspection is intended to determine if substantial work has been accomplished since issuance of the permit or last inspection; or if
the project should be considered abandoned.
If such determination is made, the Building Code does allow the Building Official to approve one or more extensions of time for
additional periods not exceeding 90 days each. Extension requests must be in writing and provide satisfactory reasons why
circumstances beyond the applicants control have prevented action from being taken.
In the event you do not call for the above inspection and receive an extension prior to 12/31/2008 , your permit will become null and
void and any further work on the project will require a new permit and associated fees.
