The plans and specifications for the above new project have been reviewed and, in accordance
with the provisions of Title 5, the Code of the King County Board of Health (The Food Code) are
hereby APPROVED and subject to the following conditions:
• Install a splash guard between the handwash sink and the 3- compartment sink.
• The menu is limited to customer self -serve yogurt only. There is no ice machine and
sufficient space for storing dirty or clean utensils /equipment. Per Title 5 (The Food
Code), "Drainboards, utensil racks, or tables large enough to accommodate all soiled and
cleaned items that may accumulate during hours of operation shall be provided for
necessary utensil holding before cleaning and after sanitizing."
• There shall be no changes made without Health Department approval.
Your establishment has been assigned the following plan review service number (SR1161084).
Please use this SR# in all future contact with us.
As required in The Food Code, upon completion of the construction and before opening for
business, the food service establishment operator /owner shall:
1. Complete an application for the annual operations permit if you don't have a current permit.
Include a copy of this letter when applying for the annual permit. Please call me prior to
paying for your permit to verify the correct fee. Be advised that the penalty for commencing
operation of a food service establishment without the required permit is 50% of the applicable
permit fee.
Obtain a preoperational inspection approval. Contact me at 206 - 205 -1903 at least one
week in advance to schedule a preoperational inspection. Be sure all other inspections

The plans and specifications for the above new project have been reviewed and, in accordance with the provisions
of Title 5, the Code of the King County Board of Health (The Food Code) are hereby APPROVED and subject to
the following conditions:
• Install a splash guard between the handwash sink and the 3- compartment sink.
• The menu is limited to customer self -serve yogurt only. There is no ice machine and sufficient space for
storing dirty or clean utensils/equipment.' Per Title 5 (The Food Code), °Drainboards, utensil racks, or
tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of
operation shall be provided for necessary utensil holding before cleaning and after sanitizing."

millimeters) glass in conjunction with our Patch Hardware, or our Header for Overhead Door Closers with Fixed Sidelites
and Door Rail Systems. The design allows it to be surface or recessed mounted, with the Top Load Roll -In Glazing
Gasket securing the glass.
This multi - functional extrusion comes in six standard finishes, and has a family of accessories that facilitate different
installations The 4 inch (101 6 millimeter) long AluminumAluminium and Neoprene Setting Blocks provide a rigid support
for the glass in the bottom channel They are the proper height to provide 9/16 inch (14 3 millimelter) glass bite in both
the top and bottom channels when glazed with the 'It and drop' method.

Installation of Glass
Install in accordance with the glass system provider's requirements and the shop drawings.
Employ only experienced glaziers who have had previous experience with the materials and systems
being applied. Use tools and equipment recommended by the manufacturer.
Plate -to -plate joints of glass are to be sealed with silicone sealant. Joint dimensions will be
designed to be compatible with sealant properties and live load movement of the structure.
Bolt torque: torque bolts to torques specified on shop drawings using a calibrated tool. Lock torque
bolts into position to prevent back -off. Reset calibrations regularly to ensure an accurate torque.
Clean glazing connectors receiving glazing materials of deleterious substances that might impair
the work. Remove protective coatings that might fail in adhesion or interfere with the bonding of
materials of deleterious substances that might impair the work. Remove protective coatings that
might fail in adhesion or interfere with bond of sealants. Comply with the manufacturer's
instructions for final wiping of surfaces immediately before the application of primer and glazing
sealants. Wipe metal surfaces with an appropriate cleaning agent.
Inspect each unit of glass immediately before installation. Glass that has significant impact damage
at edges, scratches, abrasion of faces or any other evidence of damage will not be installed.
Sealants: prime surfaces are to receive glazing sealants where required, in accordance with the
manufacturer's recommendations, using recommended primers.
Locate setting blocks, if required by the drawings, at the quarter points of the sill, but no closer
than 6 inches to corners of the glass. Use blocks of proper sizes to support the glass in
accordance with the manufacturer's recommendations.
Provide spacers to separate the glass from attachment plates.
Set the glass in a manner that produces the greatest possible degree of uniformity in appearance.
Face all glass, which has a dissimilar face with matching faces in the same direction.
Use masking tape or other suitable protection to limit the coverage of glazing materials on the
surfaces intended for sealants.
Tool the exposed surfaces of glazing materials.
Clean excess sealant from the glass and support members immediately after the application, using
solvents or cleaners recommended by the manufacturers.
Curing, Protection and Cleaning
Cure sealants in accordance with the manufacturer's instructions to attain maximum durability and
adhesion to glass.
Clean all surfaces after the installation, leaving all in a clean and workmanlike manner.
Final cleaning and protection after installation are the responsibilities of others.

CRL Regular U- Channel With Top Load Roll -In Glazing Gasket was designed to be used with 3/8 Inch or 1/2 inch (10 or 12
millimeters) glass in conjunction with our Patch Hardware, or our Header for Overhead Door Closers with Fixed Sidelites
and Door Rail Systems. The design allows it to be surface or recessed mounted, with the Top Load Roll -In Glazing
Gasket securing the glass.
This multi - functional extrusion comes in six standard finishes, and has a family of accessories that facilitate different
installations The 4 inch (101 6 millimeter) long AluminumAluminium and Neoprene Setting Blocks provide a rigid support
for the glass in the bottom channel They are the proper height to provide 9/16 inch (14 3 millimelter) glass bite in both
the top and bottom channels when glazed with the 'It and drop' method.
NOTE. For installations using 1/2 inch (12 millimeter) glass, the Roll -In Glazing Gasket is included in the package If
using 3/8 inch (10 millimeter) glass the Roll -In Glazing Gasket must be ordered separately

Installation of Glass
Install in accordance with the glass system provider's requirements and the shop drawings.
Employ only experienced glaziers who have had previous experience with the materials and systems
being applied. Use tools and equipment recommended by the manufacturer.
Plate -to -plate joints of glass are to be sealed with silicone sealant. Joint dimensions will be
designed to be compatible with sealant properties and live load movement of the structure.
Bolt torque: torque bolts to torques specified on shop drawings using a calibrated tool. Lock torque
bolts into position to prevent back -off. Reset calibrations regularly to ensure an accurate torque.
Clean glazing connectors receiving glazing materials of deleterious substances that might impair
the work. Remove protective coatings that might fail in adhesion or interfere with the bonding of
materials of deleterious substances that might impair the work. Remove protective coatings that
might fail in adhesion or interfere with bond of sealants. Comply with the manufacturer's
instructions for final wiping of surfaces immediately before the application of primer and glazing
sealants. Wipe metal surfaces with an appropriate cleaning agent.
Inspect each unit of glass immediately before installation. Glass that has significant impact damage
at edges, scratches, abrasion of faces or any other evidence of damage will not be installed.
Sealants: prime surfaces are to receive glazing sealants where required, in accordance with the
manufacturer's recommendations, using recommended primers.
Locate setting blocks, if required by the drawings, at the quarter points of the sill, but no closer
than 6 inches to corners of the glass. Use blocks of proper sizes to support the glass in
accordance with the manufacturer's recommendations.
Provide spacers to separate the glass from attachment plates.
Set the glass in a manner that produces the greatest possible degree of uniformity in appearance.
Face all glass, which has a dissimilar face with matching faces in the same direction.
Use masking tape or other suitable protection to limit the coverage of glazing materials on the
surfaces intended for sealants.
Tool the exposed surfaces of glazing materials.
Clean excess sealant from the glass and support members immediately after the application, using
solvents or cleaners recommended by the manufacturers.
Curing, Protection and Cleaning
Cure sealants in accordance with the manufacturer's instructions to attain maximum durability and
adhesion to glass.
Clean all surfaces after the installation, leaving all in a clean and workmanlike manner.
Final cleaning and protection after installation are the responsibilities of others.

millimeters) glass in conjunction with our Patch Hardware, or our Header for Overhead Door Closers with Fixed Sidelites
and Door Rail Systems, The design allows it to be surface or recessed mounted, with the Top Load Roll -In Glazing
Gasket securing the glass.
This multi - functional extrusion comes in six standard finishes, and has a family of accessories that facilitate different
installations The 4 inch (101,6 millimeter) long AluminumAluminium and Neoprene Setting Blockss provide a rigid support
for the glass in the bottom channel. They are the proper height to provide 9/16 inch (14 3 millimetter) glass bite in both
the top and bottom channels when glazed with the 'lift and drop' method
NOTE For installations using 1/2 inch (12 millimeter) glass, the Roll -In Glazing Gasket is inclJdeid in the package. If
using 3/8 inch (10 millimeter) glass the Roll -In Glazing Gasket must be ordered separately
Installation of Glass
Install in accordance with the glass system provider's requirements and the shop drawings.
Employ only experienced glaziers who have had previous experience with the materials and systems
being applied. Use tools and equipment recommended by the manufacturer.
Plate -to -plate joints of glass are to be sealed with silicone sealant. Joint dimensions will be
designed to be compatible with sealant properties and live load movement of the structure,
Bolt torque: torque bolts to torques specified on shop drawings using a calibrated tool. Lock torque
bolts into position to prevent back - off. Reset calibrations regularly to ensure an accurate torque.
Clean glazing connectors receiving glazing materials of deleterious substances that might impair
the work. Remove protective coatings that might fail in adhesion or interfere with the bonding of
materials of deleterious substances that might impair the work. Remove protective coatings that
might fail in adhesion or interfere with bond of sealants. Comply with the manufacturer's
instructions for final wiping of surfaces immediately before the application of primer and glazing
sealants. Wipe metal surfaces with an appropriate cleaning agent.
Inspect each unit of glass immediately before installation. Glass that has significant impact damage
at edges, scratches, abrasion of faces or any other evidence of damage will not be installed.
Sealants: prime surfaces are to receive glazing sealants where required, in accordance with the
manufacturer's recommendations, using recommended primers.
Locate setting blocks, if required by the drawings, at the quarter points of the sill, but no closer
than 6 inches to corners of the glass. Use blocks of proper sizes to support the glass in
accordance with the manufacturer's recommendations.
Provide spacers to separate the glass from attachment plates.
Set the glass in a manner that produces the greatest possible degree of uniformity in appearance.
Face all glass, which has a dissimilar face with matching faces in the same direction.
Use masking tape or other suitable protection to limit the coverage of glazing materials on the
surfaces intended for sealants.
Tool the exposed surfaces of glazing materials.
Clean excess sealant from the glass and support members immediately after the application, using
solvents or cleaners recommended by the manufacturers.
Curing, Protection and Cleaning
Cure sealants in accordance with the manufacturer's instructions to attain maximum durability and
adhesion to glass.
Clean all surfaces after the installation, leaving all in a clean and workmanlike manner.
Final cleaning and protection after installation are the responsibilities of others.
