Purpose The above address was inspected to locate asbestos in accordance with federal and state laws. The sampling methods specified in 40 CFR 763.86, a sub - section of the Asbestos Hazards Emergency Response Act (AHERA), were employed with the following specific purpose in mind : ❑ Safety - Intended operations present the potential for disturbance of an ACBM. This report was prepared to facilitate employees safe performance of normal activities. ❑ Removal. Specifically this survey anticipates : ❑ Renovation. This may not include use of destructive search methods (i.e., - breaking into walls, etc.) unless expressly directed by client in writing. Demolition. Destructive search techniques are highly recommended and may be implied by statutes which require identification of all suspected ACBM's prior to demolition. [as in 40 CFR 763.85 (a)(4)(i)] ❑ Management. Information herein will be used by the property owner to develop an appropriate Operations & Management (O &M) policy to reduce the likelihood of exposure to building occupants for ACBM's which will be left in place. ❑ Property Sale - Public buildings, including commercial and industrial facilities are required to use an accredited inspector [see the Asbestos School Hazard Abatement Reauthorization Act (ASHARA)]. Private buildings may be inspected to compliment an appraisal. Methods Currently only persons having the AHERA Building Inspector training are considered acceptab accredited by all governmental agencies. Mandated AHERA procedures are : • training (for the inspector) • examining (materials which the inspector suspects) • identify (by sampling or by assuming material is an ACBM) • assessing (friability, condition and potential for damage) • documenting (the above with locations) Procedures for sample collection often seem complicated to the layperson. What appears to be the same material located in different areas, may, or may not, require additional samples, depending on the usage of each area. Sprayed and troweled on materials require at least three tests to confirm they are not ACBM. Differing quantities of suspect homogeneous materials also affect the number of samples needed. Analysis of samples must be done by an accredited laboratory. Assessing friability is done per the EPA definition that the material can be crumbled to a powder y using-hand-- ensure-.- Theaet --ert be anticipate_ by this inspector. An accredited Asbestos Management Planner should be consulted for assessments of condition oi* potential for disturbance. These assessments are usually only required for inspections of schools. Estimated quantities are given as a courtesy and should not be relied on for bidding purposes. An accredited Asbestos Proiect Designer is required for quantifying ACBM's. The inspector who prepared this report hereby certifies by signature below, that : • has received training pursuant to 40 CFR Part 763, Sub -part E, Appendix C • has had at least fifteen (15) years professional experience in the asbestos abatement industry - p5 Bill Schroeder Occupied Unlimited access Limited scope Destructive sampling 1029271 May 7, 2009 Certification # Expiration Date Limitations This inspection was prepared in a manner intended to comply with current statutes governing the specific purpose stated herein for the locality in which it was performed. This report can be expecte1 to become outdated as regulations, site conditions or usage of this information change over time. Reasonable diligence was excercised to discover ACBM's during the inspection. Standard industry practices recognize that every square inch of a building cannot reasonably be exposed or sampled during the survey process, especially in occupied structures. Wall or chimney interiors, underground pipes, liners of septic tanks, woodstove flues or water heaters, 'Bakelite' toilet bowls and junction boxes, every area under carpets or bum debri, are all examples of this. Items not attached tq structures are not normally addressed by this type of report. Consequently, some materials may remain concealed, or untested, which are not identified in this report. Materials not addressed i;n this report should be assumed to contain asbestos until determined otherwise by an accredited professional. The results of such analysis must be incorporated by addenda into this report prior disturbance of these materials. A copy of this report must remain onsite during such activities. Samples are believed to be representative of similar materials found throughout the areas inspected, however, the required random sampling protocol could miss some ACBM's. Inspector is not responsible for conclusions based upon third party laboratory inaccuracies. Full depth sampling is required when roofing will be disturbed. Attempts by inspector to fill holes aft r such sampling by use of tar, caulking, etc. are beyond the scope of this report and if performed shoud be considered temporary leak prevention, not warranted to be a professional roofing repair. Drawings are not to scale. Site maps are included on multi - structure projects only. Advice for the handling of asbestos is beyond the scope of this report and nothing herein should be construed as such. Multiple agencies regulations may differ in these regards. This inspection was performed for compensation, and as such it is not warranted complete and it cannot be used for any purpose if the preparer voids it's use due to lack of payment for this service. This document is protected under USC 17 (aka Copyright Act of 1979). This inspection is also limited by site conditions and those specified by the client. The den (East half of home) was likely added c later. General Notes Following is a description of the construction systems noted at the site. y n Roofing - The West half of the home has a pitched roof with asphaltic 3 -tab shingles over tar paper. The East half has a nearly flat roof with elastomeric sealant painted over a Torched Down Membrane (TDM) system. The TDM system consists of a rubberized membrane, to and felt. Walls (interior) & ceilings Wall and most ceiling substrates are a smooth sheetrock with tape joints system. The den has exposed tongue and groove (T &G) 'ca deck' ceilings. Wall surfaces, exterior - Siding is both T -111 type wood panels and lapped wood product . Beneath siding tar paper felt is installed. Floors - Floors are a mix of hardwood, carpet, sheet vinyls and vinyl tiles. Insulation, structural - Ceiling insulation is blown in cellulose. Wall insulation is fiberglas batts. No floor insulation was found. Heat sources - A gas furnace heats West sections of the home. No suspe materials were noted on this appliance. The den is heated using electric baseboard heaters. The fireplace is most likely used fcr recreational purposes. Insulation, thermal system - No TSI's were found. Ducts are bare metal. Water pipes are n t insulated. Glazing compounds - Windows are aluminum framed types which do not have glazin compounds attached. Wiring - Wiring has plastic sheathing. Doors - Doors are wood or glass. Adhesives - Adhesives were found under vinyl floor tiles. Cove base is wood.
