2: No changes shall be made to the approved plans unless approved by the design professional in responsible charge and the
Building Official.
3: All permits, inspection records, and approved plans shall be at the job site and available to the inspectors prior to
start of any construction. These documents shall be maintained and made available until final inspection approval is
granted.
4: All construction shall be done in conformance with the approved plans and the requirements of the International
Building Code or International Residential Code, International Mechanical Code, Washington State Energy Code.
5: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap the sanitary
sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks, wells, and other
excavations. Final inspection approval will be determined by the building inspector based on satisfactory completion of
this requirement.
6: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an approval of,
any violation of any of the provisions of the building code or of any other ordinances of the City of Tukwila. Permits
presuming to give authority to violate or cancel the provisions of the code or other ordinances of the City of Tukwila
shall not be valid. The issuance of a permit based on construction documents and other data shall not prevent the
Building Official from requiring the correction of errors in the construction documents and other data.
7: ** *PUBLIC WORKS DEPARTMENT CONDITIONS * **
8: The applicant must notify the City Project Inspector at (206)433 -0179 upon commencement and completion of work at least
24 hours in advance. All inspection requests for utility work must also be made 24 hours in advance.
9: Any material spilled onto any street shall be cleaned up immediately.
10: Temporary erosion control measures shall be implemented as the first order of business to prevent sedimentation
off-site or into existing drainage facilities.
11: The site shall have permanent erosion control measures in place as soon as possible after final grading has been
completed and prior to the Final Inspection.
12: Applicant shall coordinate with Valley View Sewer District for the location of the sewer stub serving the carwash.
13: The oil/water separator tank shall be removed and the area shall be backfilled with 5/8 crushed top course flush with
existing pavement.

Dear Mr. Cusick:
PBS Engineering + Environmental (PBS) performed an inspection of the former Northfield Car Wash at
14224 Tukwila International Boulevard in Tukwila, Washington for the presence of asbestos - containing
materials (ACMs) and lead- containing paint (LCP). According to your information, this project is in
conjunction with a proposed demolition of the site structure.
The site structure is a one -story concrete block building with eight car wash bays built on a concrete
slab. The structure contains a total of approximately 1,260 square feet. The car wash has recently been
taken out of operation. Interior concrete slab floor. The roof is corrugated exposed

Asbestos - Containing Materials (ACM)
The intent of this letter is to ensure that the City of Tukwila Public Works Department is in compliance
with the Puget Sound Clean Air Agency and Washington State Department of Labor and Industries'
requirement that a "good faith" inspection for ACM be performed prior to renovation or demolition
activities.
While PBS has endeavored to identify all ACM, unidentified ACM may exist in concealed or
inaccessible locations. Inaccessible areas-are defined as those requiring selective demolition, fall
protection or confined -space entry protocols to gain access. PBS recommends that concealed
components and materials be sampled for asbestos prior to impact.
Suspect materials were sampled on October 8, 2008 by AHERA- accredited Inspector, Janet Murphy
(Certification #129790, exp. June /18/09). Samples were assigned unique identification numbers and

The attached bulk sample chain -of- custody forms identify tto contain asbestos during PBS b y materials that were

Based on lead concentrations detected to date, it is not anticipated that demolition debris will require
disposal as "dangerous" per WAC 173 -303, Dangerous Waste Regulations. However, waste stream
characterization for lead by TCLP analysis should be performed by the contractor prior to construction
debris disposal.

For samples: containing more than one separable layer of materials, the report will include
findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The
asbestos concentration in the sample is determined by visual estimation.
For those samples with asbestos concentrations between 1 and 10 percent based on visual
estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40
CFR Part 61). Point counting is a statistically more accurate means of quantification for
samples with low ,concentrations of asbestos. If you would like us to further refine the
concentration estimates of asbestos in these samples using point counting, please .let me
know.
This report is considered highly confidential and will not be released without your approval.
Samples are archived for two weeks following analysis. Samples that are not retrieved by
the client are discarded after two weeks.

Enclosed please find the test results for samples submitted to our laboratory for analysis.
Examination of these samples was conducted using analytical instruments in accordance
to U.S. EPA, NIOSH, OSHA and other ASTM methods.
For matrix materials submitted as paint, dust wipe, soil or TCLP samples, analysis for the
presence of total metals is conducted using published U.S. EPA Methods. Paint and soil
results are usually expressed in mg /Kg which is equivalent to parts per million (ppm).
Lead (Pb) in paint is usually expressed in 'mg /Kg (ppm) , Percent ( %) or mg /cm by area.
Dust wipe sample results are usually expressed in ug/wipe and ug /ft TCLP samples are
reported in mg /L (ppm). For air filter samples, analyses are conducted using NIOSH and
OSHA Methods. Results are expressed in ug /filter and ug /m Other matrix materials are
analyzed accordingly using published methods or specified by client. The reported test
results pertain only to items tested. Lead test results are not blank corrected.
For recent regulation updates pertaining to current regulatory .levels or permissible
exposure levels, please call your local regulatory agencies for more details.

Tukwila Building Division. Per the International Building Code, International Mechanical Code, Uniform
Plumbing Code and /or the National Electric Code, every permit issued by the Building Division under the
provisions of these codes shall expire by limitation and become null and void if the building or work authorized
by such permit has not begun within 180 days from the issuance date of such permit, or if the building or work
authorized by such permit is suspended or abandoned at any time after the work has begun for a period of 180
days. Your permit will expire on 08/18/2009.
Based on the above, you are hereby advised to:
1) Call the City of Tukwila Inspection Request Line at 206 - 431 -2451 to schedule for the next or final
inspection. Each inspection creates a new 180 day period.

2) Submit a written request for permit extension to the Permit Center at least seven (7) days before it is
due to expire. Address your extension request to the Building Official and state your reason(s) for
the need to extend your permit.
The Building Code does allow the Building Official to approve one extension of up to 180 days. If it is
determined that your extension request is granted, you will be notified by mail.
In the event you do not call for an inspection and/or receive an extension prior to 08/18/2009, your permit will
become null and void and any further work on the project will require a new permit and associated fees.

At least 48 hours before starting project site work, notify
the Utilities Inspector at 206- 433 -0179.
3. Request a Public Works utility inspection at least 24 hours in
advance by calling 206- 433 - 0179.
4. The Contractor assumes sole responsibility for worker safety,
and damage to structures and improvements resulting from
construction operations.
The Contractor shall have the perm it(s) and conditions, the
approved plans, and a current copy of City of Tukwila
Development Guidelines and Design and . Construction
Standards available at the job site
All work shall conform to these approved drawings. Any
changes from the approved plans require pre- approval from
the owner, the engineer, and the City of Tukwila.
7. All methods and materials shall meet City of Tukwila
Development Guidelines and Design and Construction
Standards, unless otherwise approved by the Public Works
Director.
8. Contractor shall maintain a current set of record drawings

10. Provide traffic control and street maintenance plan for Public
Works approval before implementation.
11. All surveying for public facilities shall be done under the
direction of a Washington licensed land surveyor. Vertical
datum shall be NAVD 1988. Horizontal datum shall be
Washington State (grid) Coordinates, North Zone, using NAD
83/91 survey control and tied to any two City of Tukwila
Horizontal Control Monuments. For projects within a flood
control zone, the Permittee shall provide conversion
calculations to NGVD 1929.
12. Replace or relocate all signs damaged or removed due to
construction.
13. Retain, replace or restore existing vegetation in
rights —of —way, easements, and Access Tracts.
