Call the City of Tukwila Inspection Request Line at 206 - 431 -2451 to schedule for the next or final inspection.
Jim Haggerton, Mayor
Department of Community Development Jack Pace, Director
This inspection is intended to determine if substantial work has been accomplished since issuance of the permit or last inspection; or if
the project should be considered abandoned.
If such determination is made, the Building Code does allow the Building Official to approve one or more extensions of time for
additional periods not exceeding 90 days each. Extension requests must be in writing and provide satisfactory reasons why
circumstances beyond the applicants control have prevented action from being taken.
In the event you do not call for the above inspection and receive an extension prior to 05/25/2009 , your permit will become null and
void and any further work on the project will require a new permit and associated fees.
Thank you for your cooperation in this matter.
Sincerely,
I D‘ ALQ C— CfLAAN) 1)-

April 30, 2009
To whom this may concern:
We would like to request for extension on our current permit number D08 -492 located at 13714 42
Ave South Tukwila WA 98168. The reason for the extension has to do with my husband's health and
constant admissions into the ER relating to gallbladder attacks. Furthermore, on April 16, 2009 my
spouse was taken into the ER once again and admitted into the First Hill Hospital and it was determined
by his physician that a laparoscopic cholecystectomy needs to be done. The surgery is scheduled for May
22, 2009 and he would need to take a few weeks to recuperate afterwards. Attached is the proof of the
date and time for the surgery scheduled May 22, 2009.
On a different note we have not received approval from our homeowners association to begin the
project and we are afraid with the current health conditions of my husband and the permit expiration
we will not have enough time to complete this project.
If you should have further questions about this letter, please call us at 206.242.0644
We thank you,

AFFIDAVIT IN LIEU OF CONTRACTOR REGISTRATION
STATE OF WASHINGTON)
COUNTY OF KING )
•
ss.
C/e/VES i- -s'' C / ' 4 y , states as follows:
[please print]
1. I have made application for a permit from the City of Tukwila, Washington.
2. I understand that state law requires that all building construction contractors be registered with
the State of Washington. The exceptions to this requirement are stated under Section 18.27.090
of the Revised Code of Washington, a copy of which is printed on the reverse side of this Affidavit.
I have read or am familiar with RCW 18.27.090.
3. I understand that prior to issuance of a permit for work which is to be done by any contractor, the
City of Tukwila must verify either that the contractor is registered by the State of Washington, or
that one of the exemptions stated under RCW 18.27.090 applies.
4. In order to provide verification to the City of Tukwila of my compliance with this requirement, I
hereby attest that after reading the exemptions from the registration requirement of RCW
18.27.090, I consider the work authorized under this permit to be exempt under No. 1 , and
will therefore not be performed by a registered contractor.
5. I understand that the licensing provision of RCW 19.28.161 through 19.28.271 shall not apply to
persons making electrical installations on their own property or to regularly employed employees
working on the premises of their employer. The proposed electrical work is not for the
construction of a new building for rent, sale or lease.
I understand that I may be waiving certain rights that I might otherwise have under state law in any
decision to engage an unregistered contractor to perform construction work.
