This permit may be rescinded pursuant to RCW 90.58.140(8) in the event the per - mittee fails to comply with the terms or conditions hereof. (25 /SHOR.PMT) ATTACHMENT "A" (1) Prior to completion of this demolition project and refill of the excavation, submit letters certifying one of the following: A. The contaminated soil has been removed and properly disposed of,or B. The soil has been decontaminated. The letters shall come from (1) the general contractor (2) The laboratory that tested the soil and, (3) the D.O.E. (2) The Environmental Protection Agency requires the results of an excavation zone assessment be maintained at least three years after completion of permanent closure or change -in- service. Results shall be recorded at King County Records Office so they may be available to all subsequent property owners. These records should be maintained by successive owners to fulfill liability requirements under the Washington Model Toxics Act, Initiative 97. (3) Obtain a temporary erosion and sedimentation control permit from the City of Tukwila Public Works Department. (4) After the excavation and the refill the site is only suitable for conventional, light loaded structure such as warehouse.This determination is based upon a survey dated May 12, 1989 by Geo Engineers.
