APPLICATION SUBMITTAL In order to ensure that your application is accepted for plan review, please make sure to fill out the
application completely and follow the plan submittal checklist on the reverse side of this form. Handouts are available at
the Building counter which provide more detailed Information on application and plan submittal requirements.
Application and plans must be complete in order to be accepted for plan review.
VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of
Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting
application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is
subject to possible revision by the Building Division to comply with current fee schedules.
BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor
licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this
permit application and obtain the permit will be required as part of this submittal.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall
expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180
days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition).
No application shall be extended more than once.

APPLICATION RECOMMENDATIONS:
ROOF DECK shall be firm, clean, dry and smooth. All membranes shall be
applied so the flow of water is over or parallel to, but never against the laps.
INTEC /PERMAGLAS FLEX BASE 30 Lap 2" on sides, 4" on ends and turn up
to.top of the cant. Nall on 9" centers through the laps and on 18" centers along
a line 12" from each edge, VALLEYS AND WATERWAYS shall receive an
extra layer of FLEX BASE 3038" wide and shall extend at least 8" up the inclines
out of the valleys, set In a uniform mopping of hot asphalt, optimum of 25 lbs./
square. This ply shall be laid prior to the application of the roofing plies. INTEC/
PERMAGLAS FLEX BASE 30 end laps shall be at least 4" and adjacent end
laps shall be at least 12" apart. Install two plies of FLEX BASE 30 In shingle
fashion, lapping 19" with a 17" exposure. Starter strips of 18" and 36" shall be
used and second full sheet shall be laid a maximum of 16" exposure. Embed
the full width of each ply sheet in hot asphalt, optimum of 25 lbs. /square, Each
ply shall be lightly broomed, as it Is applied. All plies may be turned up 2" above
the top of the cant, and shall be solidly mopped to the cant and vertical walls.
One MWeld pressure relief vent Is required for every 10 squares or less over
Lightweight Concrete Decks. BASE FLASHING: All guaranteed roof systems
must be flashed according to the "Flashing Details" section of this manual.
COAT entire surface with a uniform coating of Flbrated Asphalt Emulsion -or-
Emulsified Aluminum at the rate listed under material requirements .
All smooth surface roofs require periodic re- coating. Re- coating is the respon-
sibility of the owner. The roof coating must be maintained In satisfactory
condition to maintain the guarantee coverage.
All of the Recommendations and Application Techniques set forth in this
manual are considered part of this specification.
Those spocllications are eligible for an INTEC /PERMAGLAS ROOFING GUARANTEE, when In the opinion of an authorized Intec/Perrnaglas Inspector, all conditions In our current 'Roofing
Systems Manual' have been met, and the published requirements of Implementing a'Built•up Roofing Guarantee' have been completed.

All material is guaranteed to be as specified. All work to be completed in a work like manner according to standard practices. Any alteration or
devintion from the above speciacntions involving extra costs will be executed only upon written orders, and will become an extra charge over and
nhove the contract. All ngreentents are contingent upon strikes. accidents or delays beyond our control. Owner to carry fire, extended coverage, and
other necessary insnrnnce. Our workers are fully covered by Workmen's Compensation Insurance. Upon request and nt owner's option for an
ndditionnl fee, contractor will provide performance /payment bonding and excess general linbility .
Any Warranty or Guaranty, written or implied, does not apply to any item not specifically referenced in this contract; such as wells, stucco, siding,
windows, skylights, hatches, mechanical units, I IVAC equipment, ducting or similar items. Should a leak occur, caused by such an item, contractor
will tnkc corrective work only upon mutual ngreement and additional cost to owner, including service call expenses to determine the source of the
problem.
Customer understands that litigation itself breeds complaints nod, to assure against frivolous complaints and allow an opportunity to cure, hereby
waives any complaints or claims for defective workmanship, defective materials, defective application or for damages due to negligence unless such
complaint is specified in writing and delivered to J &M Roofing, Inc. within thirty (30) days from the date of ottr first invoice.
Any controversy or claim arising out of or relating to this contract or breach thereof, shall be settled in accordance with the arbitration riles of
consumcr•business arbitration tribunal of the Better Business Bureau.
This contractor, J &M Roofing Inc. is register with the State of Wnshington, registration no. JMROOI.153M9 as a specialty contrnctor and has posted
with the State a bond or cash deposit of $4,000.00 for the purposes of sntisfying such claims against the contractor for negligent or improper ;trench of
contrnct in the conduct of the contractor's business. 'Iiiis bond or cash deposit may not be sufficient to cover a claim which might nrisc front wrnk done
under your contract. If the supplier of materials used in your construction project or any employee of the contractor or subcontractor is not paid by the
contractor or subcontractor on your job, your property may be liened to force payment. If you wish additional protection. you may request the
contractor to provide you witlt the original 'lien release' documents from each supplier or subcontractor on your project. 'Me contractor is required to
provide you with the further information about lien release documents if you request it. General infbnnntion is nlso available from the Deportment of
Labor & Industries.
If a building permit is required, the owner is responsible for its cost and for expenses associated with its issunncc. Any work changes, modifications or
code enforced changes imposed by permitting and not specifically addressed in the contract "Work Specificntions" are not included, but can be added
or deleted by contract change order.
LIMITED AUTHORIZATION FOR FRAMING AND /OR DECKING REPAIRS
Contractor is authorized to snake repairs to the roof deck sheathing and framing members to insure the stnictural integrity of the roof dinplirngm as
required in preparation for roofing work. 'Bids change order shall not exceed $ without tbrther written authorization.
It is understood and agreed that during the constriction work included in this contract or nny change orders that damage may occur to interior finishes,
such as: nail pops, cracks, breaks, tears, holes to wall or ceiling coverings including painted surfaces and to other building components such as wiring,
fixtures and trim. Also, inadvertent damage may occur to personal property items that are displayed, hung, stored, shelved or attached to the structure.
Owner is responsible for the security and safety of all personal property and the insurance thereof; Contractor will use care and caution in demolition,
repair and constntclion work to avoid and minimize damages to property. Ilowever, should accidental damages occur the cost to correct such damage
is not included in this contract. Corrections to finishes, building components or the structure can be done on either n time and Materials or negotiated
Lump Stint basis.
TERMS AND AGREEMENT: Payment is duo upon receipt of billing. A finance charge equal to 1 1/2% per month of time outstanding balance is
nssessed on all past due accounts. Should it become necessary to inilinle collection of past due balances including finance charges, customer agrees to
pay all attorneys fees, collection fees, court costs, and any other related expenses.

1.- Tear off the existing roof coverings down to the wood substrate. Set ail protruding fasteners and sweep roof
clean and dry as required. During the removal process, the substrate is to be inspected for structural soundness. it is
understood that the owner or their approved representative will be available during the roofing removal and
demolition. This is required in order to n void any delay while the roof is uncovered and exposed to weather.
2.- Remove existing cap metal flashing, scrape flashing clean, store and reinstall when new rooting system is in
place.
3.- Install new lead pipe vents and flash according to the manufacture's standard specifications.
4.- Install according to the manufacture's specifications a 281b heavy duty fiberglass reinforced base sheet, 2- layers
of type iV fiberglass ply sheet set in solid mopping of hot asphalt: Warranty 5 -years according to the J &M Roofing
Inc, written warranty.
5.- Seal and flash around all pipes, conduits, poles and any other roof penetrations using heavy duty roofing mastic
embedded in fiberglass webbing in a 3- course assembly, to stabilize the water proofing work.
6.- Install new lead pipe vents and flash according to the manufacture's standard specifications.
7.- All existing downdrop box scuppers will be removed and cleaned, then reinstalled per manufacturer's
specifications.
8.- A roof coating is designed to protect the roofing membrane from ultraviolet rays, chemical attach, rapid
weathering and other conditions that cause accelerated deterioration. We support the manufacturer's position that
many roofing system should have a protective coating applied. With regular preventative maintenance and the,
appropriate roof coating, the useful life can be extended by many years. For this roofing system we will install
APOC Asphalt Emulsion or equal installed at the rate of 2 to 3 gallons per 100 square.
9.- During the course of this work, J & M ROOFING, INC., Shall use all means necessary to maintain the work
area, safe and orderly manner. All waste material shall be contained in one area as much as possible and removed
from Job site when all site when all work is completed.
