VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is subject to possible revision by the Building Division to comply with current fee schedules. BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the permit will be required as part of this submittal. EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition). No application shall be extended more than once. After hearing Dave's story I asked him to give me a memo explaining what had happened (memo attached). I then tried to call Dan Cotter, Oaktree Homes, 433 -1115, who is shown as the contractor on the permit application. I didn't get an answer so I called Don Farrell, 824 -1968, who is shown as the property owner on the permit application. I got an answering machine there and left a message asking him to call me. While I was gone to lunch Mr Farrell called and talked to Dave. He asked that we contact Mr Cotter to discuss the problem. I then made contact with Mr Cotter and we discussed what happened at the job site. Cotter said he was the job foreman but had contracted out the foundation work and hadn't been on the jobsite when Dave was there. He said he had gone past the site about noon and when he saw nobody was there had assumed the inspector had not signed off on the permit because of the bad weather and ground conditions. We discussed what Dave had reported to me about the verbal abuse he had taken from the person on the job site regarding the inspection, and I pointed out that this was entirely uncalled for and that Dave had rightfully left the site • without signing anything on the permit or engaging in any further conversation with the person can site. Mr Cotter said he was the contractor that would be doing all the work except the foundations and he didn't want to get the jobs started off on the wrong foot with us. He said he would check the situation out, schedule the inspections himself when ready, and be on site when the inspector comes to make the inspection. MEMORANDUM TO: DUANE. GRIFFIN, BUILDING OFFICIAL FROM: DAVE LARSON, BUILDING INSPECTOR DATE: APRIL 26, 1993 RE: FOOTING INSPECTIONS FOR PERMIT #B93 -0035 THRU 38 On 4 -26 -93 at about 10:00 a.m., I arrived at the site to do requested footing inspections. The site appeared to be very muddy. A person met me appearing to be in charge and I did not get his name. I asked to. see the permit package including the plans and then proceeded on site to do the footings inspection. A large portion of the first site had mud and water problems on the west end of the building and I requested the removal to firm bearing ground. The person began scooping out water with a coffee can. I moved on to the second footings and asked for a piece of rebar to probe the soil. Much of the area for footing placement was questionable and on the north side my 1/2" rebar probe penetrated about 9" or 10" without much effort. I offered to come back at 11:15 a.m. to reinspect, if he could get the footings in shape. This would give him time to cancel his concrete order in case I still disapproved the condition of his footings. As I was walking from the site, I stepped on his forms. This upset him and he began to criticize me for my field savvy. Foul language was included. I proceeded to the car and apologized for stepping on his footing. While in my car, he yelled at me to not bother coming back. I told him in that case I would have to fill in today's report. I said I could give him my report. I said I needed to sign -off on the inspection record. By this time he had become highly irrational and was using alot of foul language. I decided to leave without signing today's inspection on his inspection record. Bill Turner, a local resident passed by and he may have heard some of our conversations.
