This was from gridlines B/1.7 to 3.4. Verified the embedment depth of 3" minimum into the wall and 6" minimum into the footings. All holes were cleaned according to the manufacturers recommendations. Contractor was using USP- CIA - GEL -7000 Epoxy, which had been recently approved by Daniel Munn (engineer). This product had an expiration date of 05/07. No deficiencies were noted. CORRECTIVE ACTION TAKEN: Approved was given by the engineer, Daniel Munn for use of USP- CIA - GEL -7000 epoxy as stated in Jaimie Gordon's report dated on 8/11/06. The majority of the holes for the #4 rebar were drilled to a diameter of W. A phone call was placed by MTE to the Technical Engineer for COVERT OPERATIONS epoxy. He verbally reassured inspector Gordon that, although they do recommend in their specifications that holes be drilled no more than 1/8" over the required bar size, they have done testing with over size holes and found no decrease in strength as this particular epoxy is consistently very non - shrink. Also, per a phone conversation between Daniel Munn and Dennis Philpot, approval was given for the holes to be oversized by 1/8 ". CORRECTIVE ACTIONS TAKEN: Resolved preliminary issues from 8/10/06. MTE verified embeds of 3" in walls and witnessed cleaning of holes per manufacturers instruction prior to epoxy placement. Today all but approximately 10 bars remaining were placed horizontally in the walls. No vertical bars were placed in footings, which require a 6" embed. The remaining horizontal wall bars will be placed with the footing bars. As noted in MTE report dated 08/10/06, "Approved plans call out use of Simpson Strong Tie epoxy. Per Dennis Philpot (foreman subcontractor on site) verbal acceptance from the engineer of an alternative epoxy was accepted as it was in his words, "higher strength ". MTE was provided the written request/submittal to Daniel Munn PE which included the material specifications for COVERT OPERATIONS USP -CIA -GEL 7000 ". Today, over the phone, Daniel Munn, gave approval for this epoxy. Epoxy used today was again this product, with an expiration of 05/07. Specification on tube was ICC -ES ESR -1702. Per Dennis Philpot (foreman subcontractor on site) verbal acceptance from the engineer of an alternative epoxy was accepted as it was in his words, "higher strength ". MTE was provided the written request/submittal to Daniel Munn PE that included the material specifications for COVERT OPERATIONS USP- CIA -GEL 7000. Epoxy used was this product with an expiration of 05/07. Specification on tube was ICC -ES ESR -1702. Inspector Gordon witnessed the placement of all dowels into concrete until 2:00 PM and was then relieved by MTE inspector Daniel Quehl for continued observation.
