July 3, 2002 Richard Creamer Living Care Senior Housing Development 2819 Elliott Avenue Suite 200 Seattle, WA 98121 RE: Riverton Creek Lodge BLA Dear Richard, City of Tukwila Department of Community Development Steve Lancaster, Director Your application for a boundary line adjustment located at 13112 Military Road South has been found to be complete on July 3, 2002 for the purposes of meeting state mandated time requirements. In a phone conversation today Mick Mathison from Triad Associates indicated to Jill Mosqueda that the Owner wishes to dedicate the 30' of right -of -way during the BLA process. The Owner must provide the City a Statutory Warranty Deed and a completed Real Estate Excise Tax Affidavit. On the Affidavit, under item 6 refer to WAC 458 -61 -420. The Owner must sign the Affidavit. Public Works will get the Mayor's signature and then all of the paper work can be recorded with the BLA. Your BLA recording documents will need to reflect the dedication (see attached redlines) including a legal description of the property being dedicated. The old permit number should also be replaced with the new one, L02 -023, on both sheets. Please submit the signed and notarized mylars for final approval along with the dedication paperwork.
