Dear Dave:
We have revised the Boundary Line Adjustment documents per your review comments dated
May 5, 2005. Your comments have been addressed as follows:
1. We have revised the map to provide the required additional 4 1/2 feet of right -of -way. We
are in the process of obtaining the owner's signature and notary on the Statutory Warranty
Deed and Real Estate Excise Tax Affidavit. A legal description and map exhibit have been
completed and a copy of these documents are enclosed for your reference.
2. The signed and notarized Statutory Warranty deed will be returned to the City for further
processing
3. We have verified that the legal owner name is Open Frame, LLC and have changed all
documents accordingly.
4. All sheets of the Boundary Line Adjustment map are signed and dated by the Land Surveyor.
5. The additional right -of -way dedication area on Lot 2 is shown on Sheet 2 of the Boundary
Line Adjustment map.
6. The legal description on Sheet 3 of the Boundary Line Adjustment has been revised to reflect
the additional right -of -way dedication area.
7. The Trip Generation Impact fee will be paid prior to issuance of the Building Permit.
Three copies of the Boundary Line Adjustment map and one copy of the Statutory Warranty Deed are
enclosed for your review. We will forward the signed and notarized Deed under separate cover.

PLAN REVIEWER: Contact David McPherson at (206) 431 -2448, if you have any
questions /comments regarding the following comments.
1. Andover Park West is on the six year C.I.P. list for roadway improvements. Existing
curb /gutter along ACME Bowling frontage, will be relocated approximately 4' /z feet
further to the East. Therefore the City requires an additional 4 '/2 feet of right -of -way,
to be dedicated. See Statutory Warranty Deed w/ Real Estate Excise Tax Affidavit
form — enclosed. Owner /Applicant needs to provide a legal description and a map
depicting the area to be dedicated. This dedication shall include a portion along Baker
Boulevard, as previously discussed.
2. Return any signed w/Notary documents to Public Works, for completion of Turnover
Process.
3. Verify legal owner's name as Mustang LLC, Mt. Adams Holdings LLC, (or) other.
4. BLA/LA map sheet 1 of 3 — Land Surveyor's Certificate should be signed and dated.
5. BLA/LA map sheet 2 of 3 — Show and label (including King County recording number)

PROJECT DESCRIPTION AND ANALYSIS:

6. Clearly establish status as legal lot(s) of record (per TMC Title 17 and RCW Chapter 58.17),
showing all known easements and encumbrances.
7. Provide any required maintenance agreements, easements or other documents ready for
recording.
8. Provide King County Health Department approval if there are any septic systems on site.
Items 9 through 11 NOT required for lot consolidations or BLAs without vacant lots
9. Document sewer and water availability if provided by other than the City of Tukwila.
10. Technical Information Report (TIR) including feasibility analysis per King County Surface
Water Design Manual (KCSWDM). For additional guidance contact Public Works.
11. Provide sensitive area studies as needed per Tukwila's Sensitive Areas Ordinance (TMC
18.45) for example wetland or geotechnical reports.
RECORDING DOCUMENT: a template is available from the City in AutoCAD format
12 (a) The recording document must meet the King County Assessor's recording format
requirements (letter, legal or record of survey format).
(b) Drawing must include a graphic scale, space for the City of Tukwila file number and
north arrow. It shall include the elements listed at TMC 17.08.030 B.
(c) Provide legal descriptions of all lots before and after the proposed changes.
(d) The recording documents must include signature blocks for the owner(s), Short
Subdivision Committee, Assessor, and Recorder.
(e) Existing and proposed lot lines shall be shown solid with new lines called out and lot lines
to be removed shall be shown dashed.
COMPLETE APPLICATION CHECKILIST
The materials listed below must be submitted with your application unless specifically waived in writing by the
Public Works Department and the Department of Community Development. Please contact each Department if
you feel that certain items are not applicable to your project and should be waived, or should be submitted at a later
date for use at the public hearing (e.g. colored renderings). Application review will not begin until it is determined
to be complete. ADDITIONAL MATERIALS MAY BE REQUIRED.
The initial application materials allow starting project review and vesting the applicant's rights. However, they in
no way limit the City's ability to require additional information as needed to establish consistency with development
standards.

13 (a) Location of all sensitive areas (e.g. streams, wetlands, slopes over 20 %, coal mine areas
and important geological and archaeological sites). For stream frontage provide existing and
proposed top of stream bank, stream bank toe, stream mean high water mark, and base flood
elevation (i.e., 100 yr. flood). Maximum sheet size 24" x 36 ".
(b) Location of all required sensitive area buffers, setbacks tracts and protection measures.
(c) Show all trees over 4" caliper, indicating those to be retained.
(d) Show proposed lot and tract lines.
CIVIL PLANS: NOT required for lot consolidations or BLAs without vacant lots
14 (a) Include a graphic scale and north arrow. Maximum sheet size 24" x 36 ".
(b) Vertical datum NAVD 1988 and horizontal datum NAD 83/91. Conversion calculations
to NGVD 1929, if in a flood zone or flood -prone area.
(c) Existing and proposed utility easements and improvements, on site and in street (water,
sewer, power, natural gas, telephone, cable). Schematic designs to be provided regardless of
purveyor (e.g. site line size, location, and size of public main). No capacity calcs, invert depth,
valve locations or the like are needed.
(d) Storm drainage design at least 90% complete, which meets the TIR and KCSWDM.
Call out total existing and proposed impervious surface in square feet. Include all storm drainage
conveyance systems, water quality features, detention structures, maintenance access and safety
features.
(e) Locate the nearest existing hydrant and all proposed hydrants.
(f) Show the 100 yr. flood plain boundary and elevation as shown on FEMA maps.
(g) Plan, profile and cross - section for any right -of -way improvements.
(h) Show planned access to lots, driveways, fire access lanes and turn - arounds.
(i) Show the location and distance to proposed property lines of all existing structures,
indicating those to be removed.
(j) Show proposed lot and tract lines.
