At the September 28, 2006 public hearing, the Planning Commission approved the applicant's
Conditional Use Permit (CUP) application. The Planning Commission approved the applicant's
request for a Conditional Use Permit (CUP) to install wireless communication antennas within a
shroud located on a cross at the Riverton Methodist Church.
The Planning Commission approved the application with the following condition:
1. The antenna shroud shall match the color of the cross.

APPEALS

The period for administrative appeals is 14 -days, starting from the issuance of this Notice of
Decision. The administrative body hearing the appeal is the City Council. All appeal materials
shall be submitted to the Department of Community Development. Appeal materials shall
contain:
1. The name of the appealing party,
2. The address and phone number of the appealing party, and if the appealing party
is a corporation, association or other group, the address and phone number of a
contact person authorized to received notices on the appealing party's behalf, and
3. A statement identifying the decision being appealed and the alleged errors in that
decision. The Notice of Appeal shall state specific errors of fact or errors in
application of the law in the decision being appealed, the harm suffered or
anticipated by the appellant, and the relief sought. The scope of an appeal shall
be limited to matters or issues raised in the Notice of Appeal.
4. An appeal fee of $100.
The public notice sign must be removed from the site by the applicant after the appeal period has
expired, unless an appeal is filed with the City.

PROPOSAL: T- Mobile has submitted a Conditional Use Permit application
and State Environmental Protection Review application to the
Department of Community Development to install and operate
a wireless communication monopole at the Riverton Park
Methodist Church. A fifty foot tall steel pole will be used to
support the antenna equipment. The associated ground
equipment will be located along the eastside of the existing
church building. A six -foot tall chain -link fence with privacy
slats will be used to secure the ground equipment.

The Department of Community Development received your additional information for your conditional use
permit on March 31, 2006 for the installation of a wireless communication facility at 3118 S. 140 Street.
Based on a review of your application for submittal requirements for Conditional Use Permit, your
application is deemed complete as of May 30, 2006. A notice of land use application will be distributed
on June 10, 2006. The next step is for you to install the notice board on the site by June 10, 2006. You
received information on how to install the sign with your application packet. If you need another set of those
instructions, you may obtain them at the Department of Community Development (DCD). Also, you must
obtain a laminated copy of the Notice of Application to post on the board. This notice is also available at
DCD. After installing the sign with the laminated notice, you need to return the signed Affidavit of Posting to
our office.
This determination of complete application does not preclude the ability of the City to require that you submit
additional plans or information, if such information is necessary to ensure the project meets the substantive
requirements of the City or to complete the review process.
The application has been routed to other City Departments for their review and comment. In the coming
weeks you will receive a formal comment letter regarding the CUP application.

Please accept my apology for the delay on my part in getting corrected labels to you. The
drawing revision is reflected on sheet A -2. The height was raised 3' in order to meet the
minimum height required based on the RF drive test. Also, the cross is increased in size
by 25 %. The proposed dimensions are approximately 5' 7" x 11' 3 ". Rather than have to
slip sheet the changes I am providing complete sets.

The City of Tukwila's Planning Department received your application for a Conditional Use Permit
and SEPA application on December 30, 2005. On January 17, 2006 it was deemed to be incomplete
because certain requirements of the application were not provided.
In order for the application to be complete please provide the following:
1. Two sets of mailing labels for all tenants both residential and commercial within 500 feet of
the subject property.
Please note that the Department shall cancel an incomplete application if the applicant fails to submit
the requested information within 90 -days following notification from the Department that the
application is incomplete. The Department may extend up to 120 days if the applicant submits a
written request for an extension prior to cancellation. The request must clearly demonstrate that the
delay is due to circumstances beyond the applicant's control (such as the need for seasonal wetland
data) or unusual circumstance not typically faced by other applicants, and that a good faith effort has
been made to provide the requested materials (TMC 18.104.070).
Once the above - mentioned information is provided, City staff can begin to review the application. If

(h) Show the 100 yr. flood plain boundary and elevation as shown on FEMA maps.
(i) Plan, profile and cross - section for any right -of -way improvements.
(j) Show planned access to lots, driveways, fire access lanes and turnarounds.

Criteria for Conditional Uses (TMC 18.64.030)
1. The proposed use will not be materially detrimental to the public welfare or injurious to the property or improvements in
the vicinity.
2. The proposed use shall meet or exceed the same standards for parking, landscaping, yards and other development
regulations that are required in the district it will occupy.
3. The proposed development shall be compatible generally with the surrounding land uses.
4. The proposed use shall be in keeping with the goals and policies of the Comprehensive Land Use Policy Plan.
5. All measures shall be taken to minimize the possible adverse impacts which the proposed use may have on the in area

THEMSELVES WITH ALL CONDITIONS AFFECTING THE PROPOSED PROJECT. CONTRACTORS SHALL VISIT THE
CONSTRUCTION SITE WITH THE CONSTRUCTION /CONTRACT DOCUMENTS TO VERIFY FIELD CONDITIONS AND
CONFIRM THAT THE PROJECT WILL BE ACCOMPLISHED AS SHOWN. PRIOR TO PROCEEDING WITH CONSTRUCTOR,
ANY ERRORS, OMISSIONS, OR DISCREPANCIES SHALL BE BROUGHT TO THE ATTENTION OF THE
ARCHITECT/ENGINEER VERBALLY AND IN WRITING.
4. THE GENERAL CONTRACTOR SHALL RECEIVE WRITTEN AUTHORIZATION TO PROCEED WITH CONSTRUCTOR PRIOR
TO STARTING WORK ON ANY ITEM NOT CLEARLY DEFINED BY THE CONSTRUCTION DRAWINGS/CONTRACT
DOCUMENTS.
5 THE CONTRACTOR SHALL SUPERVISE AND DIRECT THE PROJECT DESCRIBED IN THE CONTRACT DOCUMENTS. THE
CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR ALL CONSTRUCTION MEANS, METHODS, TECHNIQUES,
SEQUENCES, SAFETY, AND PROCEDURES FOR COORDINATING ALL PORTIONS OF THE WORK UNDER THE CONTRACT.
6 THE CONTRACTOR SHALL INSTALL ALL EQUIPMENT AND MATERIALS ACCORDING TO MANUFACTURER'SNENDORS
SPECIFICATIONS UNLESS NOTED OTHERWISE OR WHERE LOCAL CODES OR ORDINANCES TAKE PRECEDENCE.
7 ALL WORK PERFORMED ON THE PROJECT AND MATERIALS INSTALLED SHALL BE IN STRICT ACCORDANCE WITH ALL
APPLICABLE CODES REGULATIONS. AND ORDINANCES. CONTRACTOR SHALL GIVE ALL NOTICES AND COMPLY WITH
ALL LAWS, ORDINANCES, RULES, REGULATIONS AND LAWFUL ORDERS OF ANY PUBLIC AUTHORITY, MUNICIPAL AND
UTILITY COMPANY SPECIFICATIONS, AND LOCAL AND STATE JURISDICTIONAL CODES BEARING ON THE
PERFORMANCE OF THE WORK.
8 GENERAL CONTRACTOR SHALL PROVIDE, AT THE PROJECT SITE, A FULL SET OF CONSTRUCTION DOCUMENTS
UPDATED WITH THE LATEST REVISIONS INVOLVED WITH THE PROJECT. THIS SET IS A VALID CONTRACT DOCUMENT
ONLY IF THE TITLE SHEET IS STAMPED IN RED INK FOR CONSTRUCTION" AND EACH SUCCESSIVE SHEET BEARS THE
ARCHITECTS/ENGINEERS SIGNED WET STAMP.
9 THE CONTRACTOR IS RESPONSIBLE FOR ADEQUATELY BRACING AND PROTECTING ALL WORK DURING
CONSTRUCTION AGAINST DAMAGE. BREAKAGE. COLLAPSE. ETC. ACCORDING TO APPLICABLE CODES. STANDARDS,
AND GOOD CONSTRUCTION PRACTICES.
10. THE CONTRACTOR SHALL MEET ALL OSHA REQUIREMENTS FOR ALL INSTALLATIONS.
11 THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL DAMAGES TO THE EXISTING CONSTRUCTOR AND REPAIR
ALL DAMAGES TO BETTER THAN NEW CONSTRUCTION. THE CONTRACTOR SHALL NOTIFY THE CONSULTANT OF ANY
DAMAGE TO 1110 BUILDING SITE OR ANY ADJACENT STRUCTURES AROUND THE PROJECT. THE T- MOBILE
REPRESENTATIVE SHALL BE SOLE AND FINAL JUDGE AS TO THE QUALITY OF THE REPAIRED CONSTRUCTOR. ANY
ADDITIONAL MODIFICATIONS WHICH MUST BE MADE SHALL BE MADE AT THE CONTRACTORS EXPENSE.
12. WHERE NEW PAVING, CONCRETE SIDEWALKS OR PATHS MEET EXISTING CONSTRUCTION, THE CONTRACTOR

CODE (NEC).
2. MINIMUM BENDING RADIUS FOR A GROUNDING CONDUCTOR IS W.
3. NO SPUCES SHALL BE PERMITTED IN GROUND CONDUCTORS.
4. ALL GROUNDING CONNECTORS ARE TO BE CLEAN AND FREE OF PAINT AND DEBRIS AT
THEIR BONDING SURFACES, AND INSTALLED PER MANUFACTURERS
RECOMMENDATIONS. USE PENETROX OR EQUIVALENT ANTIOXIDANT GREASE.
5. ALL GROUND BAR CONNECTIONS ARE TO BE 2 HOLE LUG COMPRESSION TYPE.
STACKED CONNECTIONS ARE NOT ACCEPTABLE. BACK TO BACK CONNECTIONS ON
OPPOSITE SIDES OF THE GROUND BAR ARE PERMITTED.

TESTING INSTRUMENT. THREE TESTS SHALL BE ISSUED FOR DETERMINING THE
RESISTANCE OF THE GROUNDING SYSTEM: 1 TEST AT 100 FEET, 1 TEST AT 70 FEET,
AND 1 TEST AT 35 FEET. THESE TEST LOCATIONS ARE GENERIC DISTANCES, ARE
SUBJECT TO CHANGE, AND SHOULD BE VERIFIED ON A SITE BY SITE BASIS.
VOICESTREAM WIRELESS SHALL BE GIVEN 24 HOURS NOTICE PRIOR TO THE TESTING
OF THE GROUNDING SYSTEM. THE COST ASSOCIATED WITH GROUND TESTING SHALL
BE AT THE EXPENSE OF THE CONTRACTOR.
13. ALL CONDUCTORS BONDING EQUIPMENT TO THE GROUND RING SHALL BE 82 SOLID

SWEEP TEST PROCEDURE:
THE FOLLOWING PROCEDURE DESCRIBES THE TEST AND FAULT
FINDING PROCEDURE TO BE FOLLOWED FOR ALL ANTENNAS
1. TEST EQUIPMENT SHOULD CONSIST OF A SWEEP OSCILLATOR SET TO RUN BETWEEN 1800 AND 2000 MHz,
DIRECTIONAL COUPLER WITH AT LEAST 3548 DIRECTIVITY AND SCALAR NETWORK ANALYZER WITH RESOLUTION OF
BETTER THAN 0.248. A MATCHED 50 Ohm LOAD AND SHORT CIRCUIT TERMINATION ARE ALSO REOUIRED.
2. SET UP THE RETURN LOSS MEASURING SET AS PER THE MANUFACTURERS INSTRUCTIONS AND CALIBRATE WITH
THE SHORT (04B RETURN LOSS VSWR = INFINITY.)
4. COMPOSITE RETURN LOSS: DISCONNECT THE BOTTOM JUMPER AT THE BTS AND CONNECT IT TO THE MAIN PORT
OF THE BRIDGE. RETURN LOSS OF THE COMPOSITE JUMPERS, FEEDER AND ANTENNA SHOULD BE < -184B (VSWR >
1.5: 1) BETWEEN 1800 AND 2000 MHZ
4. FEEDER RETURN LOSS: TURN OFF THE SWEEP GENERATOR, DISCONNECT THE TOP JUMPER AT THE ANTENNA AND
CONNECT A DUMMY LOAD TO THE END OF THE TOP JUMPER WITH A DIN - N ADAPTER. POWER UP THE GENERATOR
AND MEASURE THE RETURN LOSS LOOKING INTO THE BOTTOM JUMPER. THE RETURN LOSS SHOULD NOT EXCEED
-1848 ( VSWR > 1.3 : 1) BETWEEN 1800 AND 2000MHZ
5. FEEDER INSERTION LOSS: REPLACE THE LOAD WITH A SHORT CIRCUIT TERMINATION AND MEASURE THE MAXIMUM
AND MINIMUM RETURN LOSS BETWEEN 1800 AND 2000. ADD THIS TOGETHER AND DIVIDE BY 4 TO GIVE THE AVERAGE 1
WAY INSERTION LOSS WHICH SHOULD BE < 34B.
8. IF THE CONDITIONS IN 4, 5 AND 6 ARE MET THEN THE TEST IS COMPLETE. IF 4 IS BAD BUT 5 & B ARE OK THEN
REPLACE THE ANTENNA AND RETEST. IF 58 ARE BAD THEN MEASURE RL OF MAIN FEEDER ONLY. IF RETURN LOSS
IMPROVES TO -20 OR BETTER OR INSERTION LOSS IMPROVES BY MORE THAN 248 THEN REPLACE OR RETERMINATE
THE BOTTOM AND TOP JUMPER. OTHERWISE REPLACE THE MAIN FEEDER RUN. NOTIFY PROJECT MANAGEMENT AND
RF ENGINEERING OF ANY FAULTY HARDWARE.
7. ALL TEST RESULTS SHOULD BE CLEARLY MARKED WITH SITE, FEEDER NUMBER, DATE, MEASURED TIME.

SWEEP TEST PROCEDURE:
THE FOLLOWING PROCEDURE DESCRIBES THE TEST AND FAULT
FINDING PROCEDURE TO BE FOLLOWED FOR ALL ANTENNAS
1. TEST EQUIPMENT SHOULD CONSIST OF A SWEEP OSCILLATOR SET TO RUN BETWEEN 1800 AND 2000 MHz,
DIRECTIONAL COUPLER WITH AT LEAST 3548 DIRECTIVITY AND SCALAR NETWORK ANALYZER WITH RESOLUTION OF
BETTER THAN 0.248. A MATCHED 50 Ohm LOAD AND SHORT CIRCUIT TERMINATION ARE ALSO REOUIRED.
2. SET UP THE RETURN LOSS MEASURING SET AS PER THE MANUFACTURERS INSTRUCTIONS AND CALIBRATE WITH
THE SHORT (04B RETURN LOSS VSWR = INFINITY.)
4. COMPOSITE RETURN LOSS: DISCONNECT THE BOTTOM JUMPER AT THE BTS AND CONNECT IT TO THE MAIN PORT
OF THE BRIDGE. RETURN LOSS OF THE COMPOSITE JUMPERS, FEEDER AND ANTENNA SHOULD BE < -184B (VSWR >
1.5: 1) BETWEEN 1800 AND 2000 MHZ
4. FEEDER RETURN LOSS: TURN OFF THE SWEEP GENERATOR, DISCONNECT THE TOP JUMPER AT THE ANTENNA AND
CONNECT A DUMMY LOAD TO THE END OF THE TOP JUMPER WITH A DIN - N ADAPTER. POWER UP THE GENERATOR
AND MEASURE THE RETURN LOSS LOOKING INTO THE BOTTOM JUMPER. THE RETURN LOSS SHOULD NOT EXCEED
-1848 ( VSWR > 1.3 : 1) BETWEEN 1800 AND 2000MHZ
5. FEEDER INSERTION LOSS: REPLACE THE LOAD WITH A SHORT CIRCUIT TERMINATION AND MEASURE THE MAXIMUM
AND MINIMUM RETURN LOSS BETWEEN 1800 AND 2000. ADD THIS TOGETHER AND DIVIDE BY 4 TO GIVE THE AVERAGE 1
WAY INSERTION LOSS WHICH SHOULD BE < 34B.
8. IF THE CONDITIONS IN 4, 5 AND 6 ARE MET THEN THE TEST IS COMPLETE. IF 4 IS BAD BUT 5 & B ARE OK THEN
REPLACE THE ANTENNA AND RETEST. IF 58 ARE BAD THEN MEASURE RL OF MAIN FEEDER ONLY. IF RETURN LOSS
IMPROVES TO -20 OR BETTER OR INSERTION LOSS IMPROVES BY MORE THAN 248 THEN REPLACE OR RETERMINATE
THE BOTTOM AND TOP JUMPER. OTHERWISE REPLACE THE MAIN FEEDER RUN. NOTIFY PROJECT MANAGEMENT AND
RF ENGINEERING OF ANY FAULTY HARDWARE.
7. ALL TEST RESULTS SHOULD BE CLEARLY MARKED WITH SITE, FEEDER NUMBER, DATE, MEASURED TIME.
