With regard to the above form, it has come to my attention that
a slight scrivener's error exists in the legal description and
the map. The error consists of the new Lot 12 being shown
15 feet too far to the north. The enclosed legal description
(Exhibit A to the form) and map (page 3 of the form) corrects
this error. This correction has been made in'all copies of the
form except yours. If you will attach the enclosed Exhibit A
and Page 3 to your form, the matter will be resolved.
