APPLICATION SUBMITTAL In order to ensure that your application is accepted for plan review, please make sure to fill out the
application completely and follow the plan submittal checklist on the reverse side of this form. Application and plans
must be complete in order to be accepted for plan review.
BUILDING OWNER /AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor licensed
by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit
application and obtain the permit will be required as part of this submittal.
VALUATION OF CONSTRUCTION The valuation is for the work covered by this permit and must be filled in by the applicant. This
figure is used for budget reporting purposes only and not to calculate your fees.
EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall
expire by limitation. The Building Official may extend the time for action by the applicant for a period not exceeding 180
days upon written request by the applicant as defined in Section 304(d) of the Uniform Mechanical Code (current
edition). No application shall be extended more than once.

Structural calculations stamped by a Washington State licensed engineer may be
required if structural work is to be done (2 sets)
Note: Hood and duct systems require a building permit for the duct shaft.
Water heaters and vents are included in the UMC — please include any water heaters or
vents being installed or replaced.

Here are the energy code compliance forms for Washington. All you will need, to my
knowledge, is the first page labeled Component Performance Method. I included the other sheets to
show you how I calculated the areas.
You may want to adcl the wall area sheet with the calculations as the state sends the
calculation table to other firms to check our calculations. Of course you may already know that.
The way I calculated the U -Value for the Loose Fill walls is shown on the first block wall
calculation sheet.
If you have any questions about anything please call me and I will try to help. I will send a
