Handouts are available at the Building counter which provide more detailed information on application and plan submittal requirements. Application and plans must be complete in order to be accepted for plan review. VALUATION OF CONSTRUCTION Valuation for new construction and additions are calculated by the Department of Community Development prior to application submittal. Contact the Permit Coordinator at 431 -3670 prior to submitting application. In all cases, a valuation amount should be entered by the applicant. This figure will be reviewed and is subject to possible revision by the Building Division to comply with current fee schedules. BUILDING OWNER / AUTHORIZED AGENT If the applicant is other than the owner, registered architect/engineer, or contractor licensed by the State of Washington, a notarized letter from the property owner authorizing the agent to submit this permit application and obtain the permit will be required as part of this submittal. EXPIRATION OF PLAN REVIEW Applications for which no permit is issued within 180 days following the date of application shall expire by limitations. The building official may extend the time for action by the applicant for a period not exceeding 180 days upon written request by the applicant as defined in Section 304(d) of the Uniform Building Code (current edition). No application shall be extended more than once. Upon arrival on site, drilling was in progress using a 4 foot diameter bucket style auger and a one foot cutting bar to achieve a 6 -foot diameter pier. We observed that the upper portion of the bore had been cased to a depth of approximately 8 feet and a water column was maintained to approximately existing ground level within the shaft to aid in side wall retention. The boring was terminated at a depth of 50 feet, extending approximately 12 feet into a dense glacial till with gravel to boulders as observed in the cuttings. A final cleaning of the bottom, performed approximately 45 minutes after the final depth had been achieved showed no evidence of shaft caving during that period of time. We observed a 48.5 -foot long by 5.5 -foot diameter rebar cage placed unimpeded into the drilled shaft. The concrete was tremied using a pump truck with steel tubing extending approximately 51 feet down the center of the pier. A final depth measurement of approximately 51 feet was taken from the center of the shaft just prior to placement of the concrete. The tremie tube was maintained at 15 to 20 feet below the concrete level in the column with the displaced water pumped from the pier and removed from the site by tanker. The concrete level was brought to just above the bottom of the casing at which point the remaining water which had been maintained at approximately 1 foot below ground level was removed. The casing was then withdrawn with no significant caving observed. The slough and slough contaminated concrete, which made up the upper 1 to 1.5 feet of the concrete column, was removed by pump truck due to some contamination �. W- 7687 -1 Page 2 The anchor bolts were then set into place and the remainder of the concrete was tremied into place with a stinger vibrator used to settle the concrete around the bolts. As an additional check for distortion of the drilled shaft, we monitored the actual versus theoretical shaft volumes throughout the concrete placement. No significant deviation was observed in our opinion. In our opinion, the installation of the drilled pier we observed was performed in accordance with project plans and specifications, and the recommendations of our geotechnical report. It appears that a Land Altering (Grade /Fill) Permit will be required since the fill exceeds three feet in depth. We have the following additional comments: 1. Referring to the requirements for this permit, two foot existing and proposed contours need to be shown for the development. An estimate of the yardage of cut and fill and the quantities of materials, if any, being hauled to and /or from the site should be indicated. IRRIGATION MAINTENANCE GUIDELINES US West, New Vector Riverton, Tukwila Site System Activation Activate system during first mowing visit (March 16). Inspect . system including: point of connection, automatic controller, electronic valves and solenoids, back -flow preventor, manual drain valves, shut -off valve, quick couplers, turf heads, planting area heads. Repair or replace immediately any damaged, defective or missing equipment. All new equipment shall match existing. Following visual inspection of system pressurize system to determine underground leaks or breakage. Conduct a valve sequence test and watering coverage test. Adjust heads and nozzles as requi red. Weekly Inspection During watering season (March 16 to September 16) inspect all components of system prior to each mowing of lawn. Make required adjustments and replacements. Winterization Winterize system no later than October 30. Purge system of water by compressed air blow -out through quick coupler at point of connection. Open all remote valves, close shut -off valve upstream of point of connection, pressurize system with gradual addition of compressed air so that all nozzles are activated. Pressurize for five (5) minutes (do not exceed 90 psi), stop air purge, wait five (5) minutes then repeat. Following air purge, close all remote valves and inspect system to insure all nozzles have fully retracted. LMNDSCAPE MAINTENANCE GUIDELi4ES US West, New Vector Riverton, Tukwila Site The following maintenance guidelines are intended to be used with some flexibility on the part of the maintenance person. Because of changing site conditions, such as weather, temperature, wind, tenant traffic and use, vandalism, etc., it is necessary for the maintenance person to use some common sense in applying these guidelines to achieve the desired result, which is optimum plant growth and health, and a pleasing appearance. Watering Irrigate all planted and lawn areas thoroughly at least twice per week during the growing season: April 1 to October 1. During unusually warm weather, water as often as three times per week. It is necessary to feel by hand (or use an instrument that measures soil moisture) so that an evenly moist soil condition may be maintained. Adjust watering to meet weather conditions. Established lawns require approximately a 1" (one inch) depth of water per week during warm weather. Established shrub areas require a 1/2" (one half inch) depth of water per week during warm weather. Fertilization Apply fertilizer a minimum of three times per growing season, beginning April 15 and ending about July 1, so that the plants may begin to harden off before winter. Fertilizer may be broadcast evenly over the planting areas, using a broadcast type spreader. If fertilizer is applies by hand, keep it out in the area of the drip line of the plant, and avoid concentrating too much in one spot. Water thoroughly after each fertilizing. Additional fertilizings may be done with liquid fertilizer if desired, but is not necessary. Use a well - balanced fertilizer that contains trace elements as well as the three major elements ( Nitrogen- Phosphorus - Potassium). An excellent fertilizer for most plants is a rhododendron -type fertilizer or Sierra blend 18 -7 -10 with trace elements. Follow the manufacturer's directions for all fertilizer applications. For lawn areas, use a well - balanced lawn fertilizer, again containing trace elements such as Agriform 21 -8 -8, apply at 15 lbs. /1,000 sq. ft. Don't use fertilizers that are too high in nitrogen, resulting in quick top growth and more frequent mowings and poorer root growth. Apply the fertilizers as above. except continue to fertilize lawn areas throughout the summer. The last lawn fertilizing should be done in late September or October using a winter fertilizer. At the same time that the winter fertilizer is applied, spread dolomitic limestone 0 100 lbs. /1,000 sq. ft. or at the rate recommended by the manufacturer. Weed Control The best weed control would be removing weeds, including roots, by hand as they appear (and long before they produce seeds). This should be done continuously throughout the growing season, and occasionally during the dormant season. Do not allow any weed buildup to occur. 1 If pre - emergents are necessary for weed control, apply only those that are safe for use with all of the plants. Avoid strong pre - emergents, such as Casaron, in the planting or lawn areas. A contact weed killer, such as Round -up, may be used carefully in gravel or rock areas, or wherever there are no desirable plants or grasses. Follow precisely the manufacturer's directions in applying all chemicals. Most chemicals need to be applied by a person with an applicator's license. Disease and Insect Control Watch for signs of insect or disease damage and treat the problem in the early stages, before much damage occurs. The plants that are most vulnerable to insect damage are the flowering cherries and rhododendrons and evergreen azaleas. The evergreen azaleas need to be sprayed on a regular basis with Orthene to control strawberry root weevil. Again, these chemicals must be applied by a licensed spray applicator. Pruning Remove all broken, dead or diseased branches, and all branches • that are growing in a direction against the natural structure or form of the tree. Avoid shearing, except where it is desired to have a hedge -like appearance for certain shrubs. Tree pruning is an art, and should only be done by experienced persons who are sensitive to the natural structure and shape of a given tree. Dressing Beds Planting beds and lawns should be kept clean on a weekly basis from all litter and debris. Keep mulch raked smooth and even to . present a sharp appearance; do not allow mulch to accumulate more that 1/2 inch in depth around the stems of shrubs. Place new mulch only in disturbed areas to a depth of 1 -1/2 to 2 inches and rake out smooth and even. Any plants that are replaced because of damage or injury should be of the same variety as the original in so far as possible. All dead or damaged plants should be removed immediately. Lawns Lawns should be kept moved to a height of 1 -1/2 inches. All clippings should be disposed of off -site. Now lawns only when the grass is dry and the soil is firm enough so that mowing tracks are not visible. Use only a sharp mower. Trim around trees which are planted in lawn areas by hand. Avoid using string trimmers around trees, as trees can become unknowingly damaged and even girdled. Verbal authorization to proceed with this study was granted by Mr. Peter Stricker of Jean Fraley Architects on 18 June 1991. This report has been prepared in accordance with generally accepted geotechnical engineering practices for the : exclusive use of U.S. West NewVector Group and their agents for specific application to this project. SITE AND PROJECT DESCRIPTION The proposed site is 'located at 13130 44th Avenue South in Tukwila, Washington.. The site is currently occupied by Clark Construction Company. The topography across the site is essentially flat due to site grading and placement of undocumented fill soils on portions of the site which includes the project site. We understand that the proposed design calls for a 120 foot high monopole transmission tower to be built on the site. A small equipment building is to be built relatively close to the monopole. At the time of writing this report, a generalized site plan had been supplied to us. Therefore, the site plan in this report should be considered accurate to the degree implied. RECEIVED I CITY 4 ILA SEP 03 b31 PERMIT CENTER U.S. West NewVector 8 July 1991 f W -7687 Page 2 SUBSURFACE EXPLORATION AND CONDITIONS A single exploratory boring was accomplished to a depth of approximately 48 feet on 22 June 1991 by an exploration drilling company under subcontract to our firm. The ground surface was very soft at the proposed tower location and our exploration was performed as close to the proposed tower location as possible without losing support for the drill rig. The approximate location of boring B -1 is presented on Figure 1, Site and Exploration Plan. The boring operation consisted of advancing a 4 -inch inside diameter hollow -stem auger from a truck - mounted drill rig. The boring was continuously observed and logged by a geotechnical engineer from our firm. During the drilling, soil samples were obtained at 5 foot intervals: The samples were obtained using the Standard Penetration Test procedure, as described in ASTM:D 1586. Soil samples were obtained during the drilling and were taken to our laboratory for further visual classification and possible laboratory testing. An interpretive , log of this boring, describing subsurface conditions, is presented at the end of this report. The soils disclosed by our boring consisted of approximately, 4 feet of loose, moist to wet, silty sand fill underlain by native soils. The native soil sequence beneath the fill consisted of marine and slack water deposits' over glacially deposited soils. Underlying the loose, silty sand /sandy silt fill, approximately 4 feet of very loose silty sand and very soft silty clay /clayey silt was encountered over approximately 3 feet of fibrous peat. The peat was encountered between approximately 8 to 11 feet beneath the existing ground surface. Beneath the peat, approximately 14. feet of interbedded very loose silty sand and very soft sandy silty 'clay was encountered. The soils were so soft that the drilling rods sunk through the soil by their own weight during sampling. At approximately 25 feet beneath the existing ground surface the soils graded from very loose to medium dense. At approximately 38 feet the soils graded from medium dense to very dense. Soils beneath 25 feet typically consisted of silty sand, gravelly silty sand, and gravelly sandy silt. At the time of drilling, groundwater was encountered at a depth of approximately 13 feet. It is our opinion that, given enough time, groundwater levels would have risen closer to the ground surface. Subsurface seepage volumes and groundwater levels should be anticipated to vary with season, precipitation, site usage, and other factors. --- - - U.S. West NewVector 8 July 1991 W -7687 Page 5 an 18 -inch diameter, reinforced augercast piles embedded at least 48 feet deep, a maximum allowable bearing capacity of 60 tons per pile can be used for design. Uplift capacities can be provided by a combination of downdrag forces and side friction. Ultimate uplift capacities of 30 tons can be assumed for short-term seismic uplift. Based on preliminary calculations, and our understanding of typical base shear forces and moments, it appears that augercast piles may not have enough lateral capacity to limit lateral deflections. If necessary, we can provide lateral capacities for given conditions, load and allowable deflection. Lateral capacity could be provided by installing these piles on a batter, typically no steeper than 1 H:4V. Drilled piers may be considered for this project as well. However, the piers will be relatively long and the upper 25 to 30 feet (or more) will require casing due to the soft soils, peat and groundwater encountered. Additionally, the shaft may require pumping of groundwater prior to pouring concrete. If not fully dewatered, concrete would need to be tremied into place. We recommend using the following values for end bearing capacity of drilled piers: Depth Allowable End Bearing (tsf) 25 -35 feet 1.0 D/g 35 -48 feet 4.7 D/B Where p Is the depth of embedment into the bearing layer in feet and B is the diameter of the pier in feet. For frictional resistance, acting both downward and in uplift, we recommend using the following values: Embedment Depth Allowable Skin Friction (tsf) 25 -35 feet .10 • 35 -48 feet .33 For lateral pressures, we assume the piers will be relatively rigid due to the anticipated large diameter and will act as a pole.
