PATH (Projects for Assistance in Transition from Homelessness) deploys outreach case workers that meet with homeless
where they are at, build relationships, get residents connected to much needed services (EBT, disability, veteran services,
food resources, clothing, transportation, id and social security card replacement, etc.)
I &R year -to -date stats show a slight increase in calls from last month, a 35% increase in case management and a significant
increase in contract from homeless individuals.
Next meeting is scheduled for September 11, 2015, from l la -lpm, Human Services conference room.
