These inspections and tests are normally required by the local building inspection authority having jurisdiction (AHJ) when equipment has not been certified by a Nationally Recognized Testing Laboratory (NRTL) or other recognized testing agency. These inspections address only the electrical systems on the equipment listed in Section 5.0 of this report. 2.0 SUMMARY This project was initiated at the request of Mr. Jim Hagen with JR Electric. Preliminary inspection and testing of the equipment referenced in Section 5.0 was performed at Bralco Metals in Kent, WA by eti Conformity Services Compliance Engineer Donald Preiss on November 05, 2010. Final verification of corrections, testing and labeling was performed on February 18, 2011 The electrical discrepancies observed during the preliminary inspection have been verified as corrected by eti. An eti Conformity Services equipment label has been affixed to the equipment. Note that final approval is under the jurisdiction of the Tukwila L &I. 3.0 GENERAL CONDITIONS OF ACCEPTANCE In the event of modifications resulting in a change in the materials, manufacturing methods, loading, or environment that would affect the use of the accepted equipment under the provisions of the noted electrical standards, this acceptance will be considered automatically canceled. The applicant will be required to request re- examination of this equipment to determine acceptability of the modifications. By acceptance of the equipment referenced in Section 5.0, eti Conformity Services does not assume or discharge the responsibility of the equipment manufacturer, installer, or other relevant parties. Equipment evaluation is based upon adherence to sound engineering practices, and upon compliance with the specific sections quoted from the electrical standards referenced in Section 4.0 of this report. Assessment of required interrupting rating and arc -flash labeling of the equipment being evaluated is beyond the scope of this field evaluation. Protective devices are verified to be properly identified, and of a type suitable for the circuit applications as installed. The marking shall be located so as to be clearly visible to qualified persons before examination, adjustment, servicing, or maintenance of the equipment." eti Conformity Services would be happy to provide a proposal for above services upon receiving a request from the customer. 6.6 Internal Wiring Internal wiring and wiring methods are evaluated for compliance with the applicable codes and standards referenced in Section 4.0. Wiring is verified to be properly sized and rated, with a temperature rating suitable for the installed application. 6.7 Safety Interlocks and Operator Controls The equipment safety interlocks, emergency stops, operator controls and human machine interfaces (HMI) are verified to be properly identified. The safety features and emergency machine off (EMO) switches are tested for proper operation. 6.8 Field Testing Field testing is performed on the equipment, to meet the field testing requirements of the standards referenced in Section 4.0, unless production test results have been provided by the equipment manufacturer and accepted by eti Conformity Services. If no production tests are submitted, then testing is performed to verify the equipment to be operating within normally expected parameters as detailed in Section 8.0. Reference No.: 3127436 Page 4 7.0 EQUIPMENT EVALUATION 7.1 Shear 7.1.1 System Description The equipment inspected consists of Shear manufactured by Cincinnati. The equipment is rated at 480VAC, 3 Phase, 60HZ, 19 Amps and is used to cut metal parts. The equipment is installed indoors in an ordinary (non- hazardous) location, and has been evaluated for use in this location only. 7.1.2 Grounding Exposed non - current carrying parts of the equipment were verified to be effectively grounded in accordance with the applicable provisions of NEC ®, Article 250. 7.1.3 Guarding of Live Parts All internal components are enclosed in a grounded, screw closed, metal enclosure, and are effectively guarded per NEC ®, Section 110.27. 7.1.4 Overcurrent Protection Overcurrent protection was provided by a 25 Amp fuses. 7.1.5 Wiring All wiring was found to be UL listed or recognized type MTW/THHN or equivalent. _ 7.1.6 Equipment Discrepancies The discrepancies observed during the evaluation of the equipment listed in Section 5.0 were as described below. These items require corrective action in order to successfully complete the final acceptance of the equipment. Reference No.: 3127436 Page 5 MAIN PANEL .01 Fuse Replacement Markings Required The fuseholders installed in this control panel are not provided with the required fuse replacement markings. Action: A permanent marking shall be installed showing the fuse replacement type, current rating, and voltage rating for each fuse installed, or a table listing each fuse with its corresponding replacement marking is acceptable provided it is mounted inside the enclosure in a readily visible location. Reference: UL 508, Paragraphs 63.20, 63.21 UL 508A, Paragraph 56.1 Correction: Fuse replacement information was installed in this panel. .02 Arc Flash Warning Marking Required This enclosure is not provided with a cautionary marking warning of the hazardous hazards contained within. Action: A cautionary marking shall be installed in a plainly visible location on the outside of the enclosure stating the following or equivalent: NEC ®, Article 110 -16 Correction: This warning marking was installed on the panel. .03 Door Grounding Required The door of this control panel on which several devices are mounted is not provided with the required ground bonding jumper necessary to assure an adequate ground path for any fault current likely imposed on the door. Reference No.: 3127436 Page 6 Action: A ground bonding jumper sized in accordance with Table 6.4 in UL 508 shall be installed from the enclosure ground connection to the enclosure door. Any non - conductive paint, enamel, or other non - conductive coating shall be removed at the threads and contact surfaces, or be connected by fittings designed to make such removal unnecessary. Action: The line lugs shall be guarded against accidental contact by a suitable permanent partition that prevents persons from coming into accidental contact with any live parts. Action: All unused openings shall be closed to afford protection substantially equivalent to that of the wall of the enclosure. Reference: UL 508, Paragraph 6.8.3 NFPA 79, Subclauses 12.4.1, 12.4.11 NEC ®, Section 110 -12(a) Correction: The openings were covered. .08 Components Improperly Secured to Mounting Surface Several components installed in this enclosure are not properly mounted. Action: All electrical equipment must be firmly secured to the surface on which it is mounted. Action: The following items will require correction: a) Clean Reference: NFPA 79, Subclause 14 NEC ®, Sections 110 -3, 110 -12 Correction: The panel was cleaned. .10 Motor Improperly Grounded The Motor on this equipment is not provided with an equipment grounding conductor as required. Action: An equipment grounding conductor shall be installed from the ground terminal strip inside the enclosure to the Motor frame and or terminal box. The bonding conductor shall have the capacity to conduct safely any fault current likely to be imposed on it. Reference: UL 508, Paragraph 40.1 NFPA 79, Subclauses 8.2.1.1, 8.2.1.2.2 NEC ®, Section 250-43 Correction: A motor ground was added. .11 Disconnect Mechanism Requires Repair The door mounted disconnect is in need of repair or replacement. Reference: NFPA 79, Subclauses 12.1.1, 12.5.1 NEC ®, Section 110 -16 Correction: This condition was corrected. 7.2 Twin Mill 7.2.1 System Description The equipment inspected consists of Twin Mill manufactured by WH Nichols Co. The equipment is rated at 480VAC, 3 Phase, 60HZ, 20 Amps and is used to mill metal parts. The equipment is installed outdoors in an ordinary (non- hazardous) location, and has been evaluated for use in this location only. 7.2.2 Grounding Exposed non - current carrying parts of the equipment were verified to be effectively grounded in accordance with the applicable provisions of NEC ®, Article 250. 7.2.3 Guarding of Live Parts All internal components are enclosed in a grounded, screw closed, metal enclosure, and are effectively guarded per NEC ®, Section 110.27. 7.2.4 Overcurrent Protection Overcurrent protection was provided by a 25 Amp fuses. 7.2.5 Wiring All wiring was found to be UL listed or recognized type MTW/THHN or equivalent. Correction: A listed GFCI device was added. .05 Conductor Incorrectly Colored Green At least one ungrounded conductor located within the Panel is incorrectly provided with white colored insulation. Action: All ungrounded conductors provided with Green colored insulation shall be replaced with ones having an outer covering that is colored other than green, White, natural gray and shall meet the color- coding requirements for the circuit voltage on which it is to be used. Phase tape markings at EACH termination point is acceptable. Action: An equipment grounding conductor sized in accordance with NEC ®, Section 250 -95 shall be run with the supply conductors and landed to a grounding lug or ground terminal block in this enclosure. Any non - conductive paint, enamel, or other non- conductive coating shall be removed at the threads and contact surfaces or be connected by fittings designed to make such removal unnecessary. Actuation of each EMO switch was verified to properly stop the operation of the equipment. 8.2 Test Equipment All equipment has been calibrated to NIST reference standards.
