Authorization to perform this "Inspection" was received by CTC on Friday, January 28, 2000 via phone conversation between Mr. Robert Parsons of CTC and Mr. Steve Silvi. This residence consists of one, single story, two (2) bedroom home with a separate garage. This "Inspection" was conducted on the morning of 28 January, 2000. Suspect building materials were looked for and sampled by Mr. Philip Kronholm. Ten (10) samples were taken, transported to, and analyzed by the NIST NVLAP accredited analytical laboratory NVL Laboratories Inc. in Seattle, Washington. Asbestos was identified in the material. It is thereby stated that there are asbestos containing building materials in this building. SCOPE OF SERVICES CTC was retained by Mr. Steve Silvi of Demolition and Disposal Systems to conduct a "Good Faith Survey" for suspect asbestos containing building materials (ACBM) due to the planed demolition of the residence. A visual inspection and sampling survey of the facility was conducted in accordance with general Environmental Protection Agency / Asbestos Hazard Emergency Response Act (EPA/AHERA) sampling guidelines to determine the presence of exposed or accessible suspect asbestos containing building materials (ACBM). A physical hand pressure test was used to determine the friability of selected suspect materials. Bulk samples were obtained only from suspect materials that were readily exposed or accessible during the inspection. Bulk samples obtained were analyzed by NVL Laboratories, Inc. of Seattle, using Polarized Light Microscopy with dispersion staining (PLM). The analytical laboratory results are included as an attachment to this report. WARRANTY CTC warrants that the findings contained herein have been prepared in general accordance with accepted professional practices as applied by similar professionals in the community at the time of this report preparation. Changes in the state of the art or in applicable regulations cannot be anticipated and have not been addressed in this report. The field and laboratory results reported herein are considered sufficient in detail and scope to determine the presence, condition, and hazard potential of accessible or exposed suspect asbestos containing building materials in the facility at the time of inspection. Test results are valid only for the materials tested. MICSIVID CITY OPTUlKWILA MAR 1 4 2000 PERMIT CENT'EWH There is a distinct possibility that conditions may exist that could not be identified within the scope of survey or that were not apparent during the site visit. This inspection covered only those areas that were exposed or physically accessible to the inspector. The inspection is limited to the information available from the client at the time it was conducted. No other warranties are implied or expressed. METHODOLOGY GENERAL REFERENCES Inspection, sampling, and assessment procedures were performed in general accordance with the guidelines published by the EPA in 40 CFR Part 763 Subpart E, October 30, 1987. The inspection consisted of the three major activities: visual inspection, quantification, and sampling. Quantification was estimated by CTC. VISUAL INSPECTION The visual inspection was performed by Philip E. Kronholm, an EPA Certified AIERA Building Inspector. An initial facility "preliminary walkthrough" was conducted to determine homogeneous areas and the presence of suspect materials that were accessible or exposed. Materials that were similar in general appearance were grouped into homogeneous sampling areas. Homogeneous Material Classifications The walkthrough of the building areas was also conducted to determine areas of materials that were visually similar in color, texture and general appearance and that appeared to have been installed at the same time. Such materials are termed to be "'homogeneous materials" by the EPA. During this walkthrough, the approximate locations of these homogeneous materials were noted, Only materials that were accessible or exposed and suspected to contain asbestos were identified. SAMPLING PROCEDURES Following the walkthrough, the inspector collected selected samples of exposed or accessible materials identified as suspect ACBM. Destructive sampling was limited to accessible materials. No samples were taken that would cause damage to the structural integrity of the building. EPA guidelines were used to determine the sampling protocol. Sampling locations were chosen to be representative of the homogeneous materials. Samples of surfacing material were collected in general accordance with the EPA random sampling protocol outlined in the EPA publication, "Asbestos in Buildings: Simplified Sampling Scheme for Friable Surfacing Materials" (EPA 560/5 - 85-030a, October, 1985), Samples of miscellaneous materials were taken as randomly as possible, while attempting to sample already damaged areas so as to minimize disturbance of the material. 2 CIT'Y#PTIMMO k MAR 1 4 2000 PERMIT CENTER QUANTIFICATION Quantities of accessible or exposed Asbestos Containing Building Materials (ACBM) are listed on the Homogeneous Sample Groups Worksheet. There are asbestos building materials in this facility. LABORATORY PROCEDURES Method of Analysis Analysis was performed by visually observing the bulk sample and preparing slides for microscopic examination and identification. The samples were mounted on slides and then analyzed for asbestos (chrysotile, amosite, crocidolite, anthophyllite, and actinolite /tremo ite), fibrous non - asbestos constituents (mineral wool„ paper, etc.) and non - fibrous constituents. Asbestos was identified by refractive indices, morphology, color, pleochroism, birefringence, extinction characteristics, and signs of elongation. The same characteristics were used to identify the non - asbestos constituents. • The microscopist used a stereoscope to visually estimate relative amounts of each constituent using a stereoscope to determine the volume of each constituent in proportion to the total volume of the sample. All bulk samples were analyzed by Polarized Light Microscopy (PI .,M) with dispersion staining as described by the "Interim Method of the Determination of Asbestos in Bulk Insulation ", Federal Register, Volume 47, No. 103, May 27, 1982. This is a standard method of analysis in optical mineralogy and the currently accepted method for the determination of asbestos in bulk samples. A suspect material is immersed in a solution of'known refractive index and subjected to illumination by polarized light. The characteristic color displays that result enable mineral identification. It should be noted that some ACBM may not be accurately identified or quantified by PLM if the building material involved milling of asbestos fibers to extremely small fibers. As a result, these fibers may go undetected under the standard polarized light microscopy method, Transmission Electron Microscopy (TEM) analysis is available for a more definitive analysis of these very small sized asbestos fibers in those types of building materials. Laboratory Quality Control Program NVL Laboratories, Inc. is certified by NVLAP as PLM Lab #102063, and maintains an in -house quality control program. This program involves blind reanalysis of ten percent of all samples, precision and accuracy controls, and use of standard bulk reference materials. FINDINGS AND OBSERVATIONS GENERAL SUMMARY Asbestos containing building materials were found within this facility. 3 MEM cnyo TUKWIIA MAR 1 4 2000 PERMIT CENTER A material is considered by the EPA and the State of Washington to be asbestos containing if at least one sample collected from the homogeneous area shows asbestos present in an amount greater than one percent (> 1 %) asbestos. Please refer to the sample analysis sheets for a more detailed description of the microscopic analysis of these samples. Asbestos containing building materials (ACBM) are regulated by Federal, state, and local agencies, which include but may not be limited to the EPA, OSHA, WISHA, Washington Department of Ecology, and local air agencies. The EPA National Emission Standards for Hazardous Air Pollutants ( NESHAP) regulations (40 CFR 61, subpart M, National Emission Standards for Asbestos) require an inspection for asbestos be done on facilities that are to undergo demolition or renovation,work. Materials found to contain asbestos may need to be removed prior to the start of such demolition /renovation work. NESHAP defines a Regulated Asbestos Containing Material (RACM) as: (a) Friable ACM, (b) Category 1 non - friable material that has become friable, (c) Category I non - friable material that has or may be subject to sanding' grinding, cutting, or abrading, and (d) Category I1 non - friable material that may or has become friable during demolition or renovation. Category I non - friable asbestos - containing material (ACM) means asbestos- containing packings, gaskets, resilient floor covering, and asphalt roofing products containing more than 1 percent asbestos as determined using the method specified in appendix E, subpart E, 40 CFR part 763, section 1, Polarized Light Microscopy.
