Permit Conditions:
1. The applicant must notify the City utility inspector at
206 - 433 -0179 upon commencement and completion of work at
least 24 hours in advance. All inspection requests for
utility work must also be made 24 hours in advance.
2. Temporary erosion control measures shall be implemented as
the first order of business to prevent sedimentation off -
site or into existing storm drainage facilities
3. The site shall have permanent erosion control measures in
place as soon as possible after final grading has been
completed and prior to the Final Inspection.
4. Work affecting traffic flows shall be closely coordinated
with the City Utilities Inspector. Traffic Control Plans
shall be submitted to the Inspector for prior approval.

All construction activity associated with this demolition
shall be limited to within 10' of the building exterior.
6. Sewer and water utilities shall be plugged at the mains if
they are to be abandoned. If they will be used again in
the near future for a new building, they shall be capped
at the property line and at the water meter respectively,

9. All permits, inspection records, and approved plans shall be
available at the job site prior to the start of any con-
struction. These documents are to be maintained and avail-
able until final inspection approval is granted.
10. All construction to be done in conformance with approved
plans and requirements of the Uniform Building Code (1997
Edition) as amended, Uniform Mechanical Code (1997 Edition),
and Washington State Energy Code (1997 Edition).
11. Validity of Permit. The issuance of a permit or approval of
plans, specifications, and computations shall not be con-
strued to be a permit for, or an approval of, any violation
of any of the provisions of the building code or of any
other ordinance of the jurisdiction. No permit presuming to
give authority to violate or cancel the provisions of this
code shall be valid.

Golder Associates Inc. (Golder) is pleased to submit this hazardous material survey
report (HazMat) for the work we conducted at the house located at 15200 52nd Avenue
South in Tukwila, Washington. The report summarizes the results of the asbestos and
lead -based paint survey we conducted pursuant to your demolition of the house and
garage. Our work was conducted based on our Proposal dated November 20.
1. INTRODUCTION
Golder Associates Inc. (Golder) was retained by the ST Corporation to conduct the
asbestos and lead -based paint survey in support of the planned redevelopment work at
the site located at 15200 52nd Avenue South in Tukwila, Washington. Our work focuses
on the required survey work for asbestos containing materials (ACM) and lead -based
paints prior to demolition of the current site building.
The asbestos survey was conducted to identify which materials in both the house and
detached garage contain asbestos, particularly which ones contained >1% asbestos,
which according to EPA is considered an asbestos containing material (ACM). All ACM
must be removed from the site building prior to demolition by a licensed asbestos
abatement contractor according to all applicable federal, state and local requirements.
The objective of the Asbestos Survey is to perform a "good faith inspection" for the

presence of asbestos containing materials (ACM) throughout the building, according to C OF CM TUKWILA
WAC 296 -62 -07707 and the Puget Sound Air Pollution Control Agency (PSAPCA) DEC 1 ��g�
Asbestos Regulation, Article 4, Regulation 3. The inspection was performed in order to
identify ACM in all building materials to the extent practical prior to demolition PERMIT CENTER
activities. The Asbestos Inspection was conducted by William Beck and Tom Stapp,

AHERA accredited inspectors. Homogeneous materials were identified as suspect ACM
according to AHERA, and representative bulk samples were collected and sent to the
Clayton Environmental Consultants laboratory in Seattle, Washington for Polarized
Light Microscopy (PLM) analysis.
The lead paint survey was conducted to identify which major paint types contain lead in
order to inform site demolition workers of potential lead exposures during demolition, as
required by the Washington State Labor and Industries (L &I) and in order to
appropriately designate the post - demolition construction debris pursuant to the
Washington Department of Ecology (DOE) Dangerous Waste Regulations - WAC 173-
303). In addition, we conducted Toxicity Characteristic Leachate Procedure (TCLP)
analysis on one a representative paint type and its substrate after receipt of the paint
chip results. We did this to provide waste designation assessment of the resultant
construction debris after the buildings (house and garage) are demolished.
2. SURVEY METHOD
2.1. Asbestos Survey
A walk - through inspection of all accessible areas of both buildings onsite was performed
to identify suspect asbestos containing materials. Materials that were buried, enclosed
behind walls or plaster ceilings, under metal jackets or otherwise inaccessible without
destructive sampling, such as breaking out plaster walls, were not fully sampled.
Upon identification of a suspect material, its location and type were noted. Samples
were obtained, placed in plastic bags, and labeled with a unique laboratory number.
Samples were collected to achieve a representative characterization of the visible suspect
ACM found. All samples were collected according to EPA guidelines to minimize
potential contamination to the surrounding area. Plans of the sample locations are
presented on Figures 1 through 3. Copies of the laboratory analytical reports are
included in Appendix A.
2.2. Lead -Based Paint Survey
Samples were identified based on their surface color. The samples collected included all
layers of paints to the substrate (i.e., concrete, wood, steel, etc.). Upon identifying a
paint, its location and amount were noted. Seven bulk samples were obtained, placed in
a plastic bag and labeled with a unique number prior to delivery to the laboratory for
analysis by EPA method 3050A/6010A. Table 2 contains all applicable data on lead paint
sample locations and Table 3 presents the results of a construction debris mass balance
model for the site. Plans of the sample Iocations are presented on Figures 1 through 3.
Copies of the laboratory analytical reports are included in Appendix A.

2.3. Lead Waste Designation Services
WAC 173 -303 requires that a "representative" sample be collected of waste streams
generated during building demolitions. However, in order to optimize efficiency of
demolition and disposal activities it is preferable to determine the designations of the
waste streams before demolition. Therefore, we designated the materials based on a
knowledge of the waste characteristics of the various architectural components.
We conducted the following tasks:
• Collected and analyzed a core samples of the light green exterior paint where
lead was detected in widespread applications (representing the most widespread
paint application for the two buildings) and conducted Toxicity Characteristic
Leachate Procedure (TCLP) testing to provide data on leaching in order to
provide a conservatively biased estimate of the CD rubble's designation; and
• Conducted an construction debris mass balance estimate comparing the total
weight of the lead paint versus the total weight of the demolished buildings
(assuming that all the lead leaches).

3. RESULTS
3.1. Asbestos Survey
Table 1 lists all the suspect homogeneous sampling areas identified onsite, the results of
the PLM analysis, the sample numbers, descriptions of the materials and their locations,
the amount and condition of materials identified as asbestos - containing, and other
pertinent information. Appendix A contains copies of the laboratory results.
None of the suspect samples are considered ACM, that is none have asbestos at
concentrations greater than 1% asbestos by weight. However, the joint compound on
drywall surfaces does contain asbestos, but at concentrations less than 1 %, therefore this
is not regulated by EPA or PSAPCA although L &I does consider it as a low asbestos
content.
3.2. Lead -Based Paint Survey
Table 2 lists the results of the analytical tests, the sample numbers, descriptions of the
paint color and the substrate, the paint locations, the amount of the paint type.onsite,
and other pertinent information. Appendix A contains copies of the laboratory results.

Lead was found in all of the paint samples collected. Of these, five were in excess of the
Consumer Products Safety Board definition of lead -based paint (i.e., >600 ppm lead).
Table 2 provides a summary of the lead paint chip sample results.
3.3. Lead Waste Designation Services
The results demonstrate that the light green exterior paint, with the relatively high
concentrations of lead and a very widespread application do not leach detectable
amounts of lead (i.e., less than 0.2 mg/1). The laboratory data is attached.
In addition, in order to assess the impacts of the lead for the entire construction debris
pile, assuming all of the building materials that contain lead -based paints will be
demolished and disposed of together, we have run a construction debris mass balance to
determine the maximum possible lead that could leach in the TCLP procedure (assuming
100% of the lead in the paint would leach — the most conservative assumption). We have
also provided a "best estimate" of how much lead could leach in the TCLP procedure
based on the results of the TCLP analyses we conducted (in the 1% range).
Table 3 presents our construction debris mass balance for this project.
The results of the mass balance indicate that if all of the lead paint in the building was to
leach in the TCLP procedure it would NOT exceed the Washington State Department of
Ecology's Dangerous Waste criteria of 5 mg/1(WAC 173 -303). In addition, this
significantly overestimates the leachability of the paint, given the TCLP test results
which show that lead does not leach from these paints at levels above the method's
detection limit. However, assuming minimal lead leaching of 1/2 of the detection limit
(or 0.1 mg/l) the percentage of lead that would leach from a paint type with a significant
amount of lead and very widespread application is approximately 1 %. Assuming this
leachability for the other painted surfaces, the resultant TCLP concentration from the
entire construction debris pile would be 0.001 mg/l or approximately 5,000 times less than
Ecology's dangerous waste criteria of 5 mg/l.

4. CLOSURE

No ACM was identified in the house and detached garage at 15200 52nd Avenue South
in Tukwila, Washington. However approximately 3000 square feet of wallboard was
identified as being low asbestos containing material (i.e., less than 1% asbestos). This
material is not regulated by EPA and PSAPCA, but it is considered asbestos- containing
by the Washington Department of Labor and Industries. No asbestos abatement work is
required on these two buildings prior to demolition, but you should notify the
demolition contractor that low levels on non - regulated asbestos is present in the
wallboard joint compound. The Washington State Department of Labor and Industries
enforces the Washington Asbestos Standard worker safety regulations for ACM
abatement activities.
Any future project specifications for demolition activities must inform the contractors
that there are lead containing paints in the structure and that the contractor is required
to take the proper precautions according to the applicable regulations to protect his/her
workers and to ensure that environmental releases do not occur.

Per our telephone conversation today, the following permits need to be issued by the Permit Center prior to
the issuance of the new 52 "d Avenue S Office Building (D98 - 0300). These structures will need to be
removed and finaled by the City of Tukwila. The permits are as follows:
1. "`MI99- 0001 -, demolition of a 1,200 sq ft single family residence; and
2. MI99 -0002 — demolition of a 400 sq ft detached garage.
At the time the Permit Center issues these demolition permits, the contractor awarded the job will need to
provide a copy of their current Washington State Contractor Registration Card and pay the outstanding fees
totaling $128.00.

1. Accumulation of combustible waste material is

day as needed.
2. Please contact the fire department in regards to using
the building for training prior to demolition.
