We reviewed the en- vironmental checklist and have the following comments. 1. Notification regarding newly installed tanks must be pro- vided to the Department of Ecology on a state notification form within 30 days of being put into service. The notifica- tion form can be obtained by calling Jean Witt at 1-800-826-7716. 2. Removal of underground tanks should be accompanied by a soil test for contamination in accordance with Department of Ecology procedures. If contamination above Ecology's cleanup levels is encountered, please notify Ecology immediately. The contact persons are Lynn Cashion or Gail Colburn at (206) 867 -7000.
