ENVIRONMENTAL CHECKLIST FORM
This questionnaire must be completed and submitted with the application for
permit. This questionnaire must be completed by all persons applying for a
permit from the City of Tukwila, unless it is determined by the Responsible
Official that the permit is exempt or unless the applicant and Responsible
Official previously agree an Environmental Impact Statement needs to be completed.
A fee of $50.00 must accompany the filling of the Environmental Questionnaire
to cover costs of the threshold determination.

6. Nature and Brief Description of the Proposal (including but not limited
to its size, general design elements, and other factors that will give
an accurate understanding of its scope and nature):
Provision of asphalt paved parking area for 53 vehicles on approximately 120
foot X 220 foot site - by minor grading, filling, installation of storm sewer,
gramlar base, 2" of asphalt concrete, provision of 10 foot wide X 290 foot long
landscaped buffer strip along south edge of the site. Approximate cost = $30,000.
7. Location of Proposal (describe the physical setting of the proposal, as
well as the extent of the land area affected by any environmental im-
pacts, including any other information needed to give an accurate under-
standing of the environmental setting of the proposal):
Proposed parking lot will provide supplemental parking for BEC employees
now moving into existin 4 office building approximately one block north
on Andover Park East. Lot will be located south of and adjacent to the
Baker Building located at 505 Baker Boulevard (S.E. corner of Baker
Boulevard and Andover Park East.)

removed, minor grading and filling of depressions will be performed
and the area will be asphalt paved and landscaped. The maximum change
in ground surface elevation will be approximately one foot.

result due to the paving of an area which; in the past, was grass and weed
covered and resulted in most of the rainfall being absorbed on site. The
amount of surface water run -off will increase due to the addition of
relatively impervious pavement.

employees walking to and from the proposed parking lot and their office
