This questionnaire must be completed by all persons applying for a permit from the City, of Tukwila, unless it is determined by the Responsible Official that the permit is exempt or unless the applicant and Responsible Official previously agree an Environmental Impact Statement needs to be completed. Explanation:The right turn lane will result in an additional 12 feet of asphalt surfacing for approximately 1250 feet along the existing roadway. This should not create any large disruption to the existing soil subgrade or ground surface relief features, since the existing topography is relatively flat and the required excavation is limited. 2.
