Dear Permit Holder:
Jack Pace, Director
In reviewing our current records, the above noted permit has not received a final inspection by the City of Tukwila Building
Division. Per the International Building Code, International Mechanical Code, Uniform Plumbing Code and/or the National
Electric Code, every permit issued by the Building Division under the provisions of these codes shall expire by limitation and
become null and void if the building or work authorized by such permit has not begun within 180 days from the issuance date of
such permit, or if the building or work authorized by such permit is suspended or abandoned at any time after the work has begun
for a period of 180 days. Your permit will expire on 2/3/2014.
Based on the above, you are hereby advised to:
1) Call the City of Tukwila Inspection Request Line at 206 - 431 -2451 to schedule for the next or final inspection. Each
inspection creates a new 180 day period, provided the inspection shows progress.
-or-
2) Submit a written request for permit extension to the Permit Center at least seven(7) days before it is due to expire.
Address your extension request to the Building Official and state your reason(s) for the need to extend your permit.
The Building Code does allow the Building Official to approve one extension of up to 180 days. If it is determined that your
extension request is granted, you will be notified by mail.
In the event you do not call for an inspection and/or receive an extension prior to 2/3/2014, your permit will become null and void
and any further work on the project will require a new permit and associated fees.
Thank you for your cooperation in this matter.
Sincerely,

RE: Request for Extension Denial
Development Permit D12 -132
Moe's Home Furnishings — 720 Andover Pk E
Dear Mr. Ybarra,
This letter is in response to your written request for extension number three to permit D12 -132. This
request was routed to the Interim Building Official, Gary Schenk, for his consideration. He determined
that permit extensions have been granted in the past and there has not been sufficient effort to complete
the work which would show that another extension is justified. Consequently there will not be an
extension granted.
In order to complete the work a new permit application will need to submitted inclusive of application,
plans, and fees. This new permit can include /reference just the remainder of the work and should
reference this permit number in the description of work.
If you should have any questions, please contact our office at (206) 431 -3670.

FEB 14 2014
I am pleading yet again for an extension for permit #D -12 -132. I have bought the needed
emergency lighting but it is on back order eta of 1St week of March which will put me over on
the permit.
Attached Is a copy from my contractor:
The manufacturer states this increase is due to the growing demand for this high performance high -
output emergency battery pack, as the whole country starts to come into compliance with the UBC
emergency egress illumination rule.
With your approval of this slight fixture cost increase, I will place the order, approximately 3 week
delivery to jobsite.
Thank you for your time in this.

I am pleading for an extension for permit #D -12 -132. As I have not gotten the work done
yet. My contractor misinterpreted the deadline and I also did not catch it. We thought it was
180 days from Feb. 14,2014.
I do apologize, we have also just taken over the entire building of 720 -790 Andover Park
East. Business is picking up for us and we are in the process of drawing up plans to move our
retail location from 720 to the 790 location which would give us a corner lot and a lot more
exposure. As we move our store we will no longer need the exit lighting as the 720 location
would be converted to a warehouse location for us.
If you could please take this into consideration and allow us more time so we can submit
our renovation drawings to the city for our new headquarters of Moes home USA division.
Thank you for your time in this.

Dear Allen,
I am in receipt of your comment letter dated April 27th, in which there was one comment
relative to exiting in an M occupancy. The comment indicated a maximum of 30' around
"merchandise pads" to an exit path of travel connecting the exits.
I have added another exit path of travel so there are two distinct exit pathways and notes are
added for the exit pathways to be 36" wide.
I trust this will answer your concerns.

AFFIDAVIT IN LIEU OF CONTRACTOR REGISTRATION
PERMIT NO: 21T-11,1-
STATE OF WASHINGTON)
) ss.
COUNTY OF KING
N r K tates as follows:
[please print name]
1. I have made application for a permit from the City of Tukwila, Washington.
2. I understand that state law requires that all building construction contractors be registered with the
State of Washington. The exceptions to this requirement are stated under Section 18.27.090 of the
Revised Code of Washington, a copy of which is printed on the reverse side of this Affidavit. I have
read or am familiar with RCW 18.27.090.
3. I understand that prior to issuance of a permit for work which is to be done by any contractor, the
City of Tukwila must verify either that the contractor is registered by the State of Washington, or that
one of the exemptions stated under RCW 18.27.090 applies.
4. In order to provide verification to the City of Tukwila of my compliance with this requirement, I
hereby attest that after reading the exemptions from the registration requirement of RCW 18.27.090,
I consider the work authorized under this permit to be exempt under number _, and will therefore
not be performed by a registered contractor.
5. I understand that the licensing provision of RCW 19.28.161 through 19.28.271 shall not apply to
persons making electrical installations on their own property or to regularly employed employees
working on the premises of their employer. The proposed electrical work is not for the construction
of a new building for rent, sale or lease.
I understand that I may be waiving certain rights that I might otherwise have under state law in any decision
to engage an unregistered contractor to perform construction work.
