Per the International Building Code, International Mechanical Code, Uniform Plumbing Code and/or the National Electric Code, every permit issued by the Building Division under the provisions of these codes shall expire by limitation and become null and void if the building or work authorized by such permit has not begun within 180 days from the issuance date of such permit, or if the building or work authorized by such permit is suspended or abandoned at any time after the work has begun for a period of 180 days. Pacific Daylight Time From: Buffalomaster @aol.com To: Buffalomaster @aof.com To Whom it May Concem, This letter is in regard to the LuBahn project located at 4447 S 148th St. Tukwila Wa. I was the general contractor that was hired to do a rehab on the house, my company name is Quality Renovation Specialists. I was hired to get the house back into living condition, including a re pipe, rewire, new kitchen and bathroom and other repairs. The last date of work that I was there for was 4 -6 -2012. After this date the owner started to do work on the project on his own accord. On 4 -11 and 4 -12 -2012 I loaded up my equipment and have not returned to the project. At the time of my departure plumbing and electrical rough in had been completed and signed off by the inspectors. While I was loading tools on the 4 -11 -2012 1 noticed that the owner had added electrical where two windows had been removed. One location in the master and one in the living room. This had been requested the previous week and I had told the owner I would call the Electrician and have the work done. I am not sure who had done the work but I do know that it was not by the electrician I had hired to do the project. The window openings had been sealed enclosed with sheet rock so I could not see how the installation had occurred. On my second trip for my tools on 4 -12 -2012 I witnessed the owner and a friend attempting to adjust the level of the kitchen floor. I watched the sheet rock tear above the door in the kitchen and watched the center of the. floor bowing upward at least two inches from its original position. At the time I was concerned about damage to the electrical work that had been previously installed. I do not know how much the new wires can stretch without damaging the sheathing. I am also concemed about what else may have been done with out my knowledge. At this time I request the removal of my name from the plumbing and electrical permits. I have discussed this . with the elect ician as well and he wants his name removed as well. I do not know if the can be "transferred" to the owners name or what must be done to accomplish this but I request that it be done sooner than later. I wish no problems on the owner and hope he can accomplish his goals however my responsibilities ended on 4-6 72012 when the owner started his efforts Please let me know how we can expedite this, Mark Smith Quality renovation Specialists A:77012, CITY u MAY . ?(12-- 0291 Monday, May 07, 2012 AOL: Buffalomaster CITY OF TUKWILA Department of Community Development 6300 Southcenter Boulevard, Tukwila, WA 98188 Telephone: (206) 431 -367o FAX (206) 431 -3665 E -mail: tukplan@ ci.tukwila.wa.us Permit Center /Building Division 206 431 -3670 Public Works Department 206 433 -0179 Planning Division 206 431 -367o AFFIDAVIT IN LIEU OF CONTRACTOR REGISTRATION PERMIT NO: STATE OF WASHINGTON) ) ss.
