Project Description:
The City of Tukwila plans to restore and stabilize a 60 foot section of Southgate Creek near 13422
40th Avenue South using rockeries, matting and native plants. This should improve water quality,
reduce erosion, and improve salmon habitat.

Summary of Primary Impacts:
1. Earth - Approximately 10 to 15 cubic yards of soil will be excavated during the process of
cutting back the unstable bank areas to an appropriate slope in order to reduce erosion. Erosion
control and revegetation measures such as placement of fiber mats and planting of water
tolerant native plant species will be used to minimize the effects of this disturbance.
2. Air - There will be exhaust emissions from construction equipment during the project. Dust
control measures in compliance with the Puget Sound Air Pollution Control Authority will be
used as needed. The project will have no air emissions when complete.
3. Water — Currently the project site is actively eroding, creating water quality and sedimentation
problems downstream of the site. The project will reduce the current slope of the incised

streambanks and stabilize them using jute matting, one-man rock and native streamside
vegetation. Instream structures will be incorporated to dissipate high-energy flows and provide
for increased complexity and salmonid passage. No modifications to current collection and
disposal of runoff will occur.
4. Plants — Areas disturbed by construction will be replanted with native riparian species. Current
vegetation is primarily lawn, shrubs and small trees.
5. Animals — After construction the site may become a migration route for Chinook salmon. The
project is expected to enhance salmon habitat through removal of passage barriers and increased
riparian vegetation.
6. Energy and Natural Resources - The project will require energy for construction equipment and
for vehicles coming to the site. No energy will be needed for the project once completed.
7. Environmental Health - No environmental health hazards are anticipated. Construction
equipment operation will need to comply with Tukwila's noise ordinance.
8. Land and Shoreline Use - The project will maintain the existing use of the site as a residential
lot.
9. Housing - The proposal will not result in a change to the housing supply.
10. Aesthetics - The project is not subject to BAR requirements.
11. Light and Glare - The completed project will not generate light or glare.
12. Recreation - The proposal will not affect recreational facilities.
13. Historical and Cultural Preservation - The site is not known to have any historical or cultural
significance.
14. Transportation — The proposal will not have any transportation impacts.
15. Public Services - The project will not increase demand on public services.
16. Utilities - The project will not affect utility services.

Dear Ryan:
Your application for stream bank stabilization and riparian vegetation enhancement located near
13422 40th Avenue South has been found to be complete on July 22, 1999 for the purposes of
meeting state mandated time requirements.
The next step is for you to install the notice board with a laminated copy of the Notice of
Application on the site within 14 days of the date of this letter. Once the notice board has been
installed the comment period will start. After installing the sign with the laminated notice, you
need to return the signed Affidavit of Posting to our office.
This determination of complete application does not preclude the ability of the City to require that
you submit additional plans or information, if in our estimation such information is necessary to
ensure the project meets the substantive requirements of the City or to complete the review process.
This notice of complete application applies only to the permit identified above. It is your
responsibility to apply for and obtain all necessary permits issued by other agencies. In our initial
review it appears the project is subject to a Hydraulics Project Approval (HPA) from the State
Department of Fish and Wildlife. You should contact them directly to find out what their
application requirements are. There may be permits from other agencies required which we have
not identified.

The project consists of providing localized bank stabilization, instream habitat (woody debris and
step pool structures), and streamside vegetation planting. The intent of this project is to provide an
improvement to water quality by reducing erosion and augment the streamside riparian vegetation.
12. Location of the proposal. Give sufficient information for a person to understand the precise
location of your proposed project, including a street address, if any, and section, township, and
range, if known. If a proposal would occur over a range of area, provide the range or
boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic
map, if reasonably available. While you should submit any plans required by the agency, you
are not required to duplicate maps or detailed plans submitted with any permit applications
related to this checklist.
The project is located in the City of Tukwila. Section SW10, Township 23 N, Range 4E.
Specifically the site is located at 13422 40`h Ave S. Tukwila, WA 98168.

Unknown.
B. ENVIRONMENTAL ELEMENTS
1. Earth
a. General description of the site (circle one): Flat, rolling, hilly, steep slopes, mountainous,
other:
Rolling.
b. What is the steepest slope on the site (approximate percent slope) ?
Several places within the project site currently have a 90% slope. These slopes are the
current incised stream banks. A majority of the site is at a 6% slope or less.

c. What general types of soils are found on the site (for example, clay, sand, gravel, peat,
muck)? If you know the classification of agricultural soils, specify them and note any
prime farmland.
The soils include silty sands with gravels.
d. Are there surface indications or history of unstable soils in the immediate vicinity? If so,
describe.
Several places of the streambank are heavily incised and are actively eroding during rain
events. Otherwise the surface conditions are stable.
e. Describe the purpose, type, and approximate quantities of any filling or grading proposed.
Indicate source of fill.
Unstable streambanks will be excavated and splayed back to the lowest appropriate slope for
the site. This is being done to reduce streambank erosion. The removed material will not
leave the project area and will be relocated on the residential property. The "fill" material
will be distributed on site in such a manner as to not allow any material to re-enter the stream
system or to be allowed to be transported off site by rain water.
f Could erosion occur as a result of clearing, construction, or use? If so, generally describe.
Yes. Some minor erosion will occur during installation. Erosion control techniques will be
used to limit this to a minimum. Construction activities will not be conducted during rain
events or during above average flow rates.
g.
About what percent of the site will be covered with impervious surfaces after project
construction (for example, asphalt or buildings)?
None. No new impervious will be constructed.
h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any:
Temporary erosion control will be employed during construction as required. Construction
activities will not be conducted during rain events or during above average flow rates.
2. Air
a. What types of emissions to the air would result from the proposal (i.e., dust, automobile
odors, industrial wood smoke) during construction and when the project is completed? If
any, generally describe and give approximate quantities if known.

3. Water
a. Surface:
1) Is there any surface water body on or in the immediate vicinity of the site (including
year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe
type and provide names. If appropriate, state what stream or river it flows into.
Yes. Southgate Creek is a small tributary to the Duwamish.
2) Will the project require any work over, in, or adjacent to (within 200 feet) the described
waters? If yes, please describe and attach available plans.
Yes. The overall project will occur in and along the banks of approximately 60 feet of
Southgate Creek. Currently, the immediate project site is heavily incised and is actively
eroding. This erosion is creating water quality and sedimentation issues downstream of the
project site. Work will be conducted between the streambanks and below the ordinary high
water mark through this section of stream. The project will reduce the current slope of the
incised streambanks and stabilize them using jute matting, one-man rock and native
streamside vegetation. Instream structures will be incorporated to dissipate high-energy
flows and provide for increased complexity and salmonid passage. Woody debris will also
be placed where appropriate. Site plans are attached as are detail illustrations.
3) Estimate the amount of fill and dredge materia! that would be placed in or removed from
surface water or wetlands and indicate the area of the site that would be affected.
Indicate the source of fill material.
Approximately 10 to 15 cubic yards of sand and gravel may be removed from within the
project site. Most of this material will be removed while reducing the slope of the
strambank.
4) Will the proposal require surface water withdrawals or diversions? Give general
description, purpose, and approximate quantities, if known.

Future salmon habitat enhancements include the installation of instream enhancement
structures, removal of passage barriers, and increasing the riparian vegetation throughout the
entire drainage.

On the site is located a dilapidated concrete channel which was constructed in the 40's. This
structure is a barrier to fish migration. There is also a house and a detached garage on the
project site (see attached site plan).

Some of the dilapidated concrete channel will be removed or reoriented within the stream
channel to assist in the stabilization of the banks within the project site.
