COUNTY OF KING [please print name] , states as follows: PERMIT NO: 11V J I ' O7'2 1. I have made application for a permit from the City of Tukwila, Washington. 2. I understand that state law requires that all building construction contractors be registered with the, State of Washington. The exceptions to this requirement are stated under Section 18.27.090 of the Revised Code of Washington, a copy of which is printed on the reverse side of this Affidavit. I have read or am familiar with RCW 18.27.090. 3. I understand that prior to issuance of a permit for work which is to be done by any contractor, the City of Tukwila must verify either that the contractor is registered by the State of Washington, or that one of the exemptions stated under RCW 18.27.090 applies. 4. In order to provide verification to the City of Tukwila of my compliance with this requirement, I hereby attest that after reading the exemptions from the registration requirement of RCW 18.27.090, I consider the work authorized under this permit to be exempt under number i Z, and will therefore not be performed by a registered contractor. 5. I understand that the licensing provision of RCW 19.28.161 through 19.28.271 shall not apply to persons making electrical installations on their own property or to regularly employed employees working on the premises of their employer. The proposed electrical work is not for the construction of a new building for rent, sale or lease. I understand that I may be waiving certain rights that I might otherwise have under state law in any decision to engage an unregistered contractor to perform construction work. .r. 6 ‘•SC/.1 111,11 /L, • ss,ONttt .4,,, ii : f f.111147 OF wpke 4" Signed and sworn to before me this 5)14dayof AI/ ,20 NO: ifl`'► 'n a dfor the State of Washington Residing at Name as commissioned: \<iakt !/I► My commission expires: l6141Y1 cokey2016- \k;W ‘ `k , County --W16) ABBREVIATIONS AB AC AD ACT ADA ADD ADDL ADDN Anchor Bolt Acoustic/Alternating Current Area Drain Acoustic Ceiling Tile Americans W/ Disabilities Act Addendum Additional Addition ADJ Adjustable/Adjacent ADJ/SHF Adjustable Shelf AFF Above Finished Floor AGGR Aggregate ALUM Aluminum ALT Alternate/Alteration AMp Amperes ANCH Anchor ANOD Anodized (Anodic Coating) AP Access Panel APROX Approximately) ARCH Architectural ASST Assistant ASSY Assembly AUTO Automatic AUX Auxiliary AV Audio Visual AWP Acoustic Wall Panel BAL Balance BC Bottom of Curb BD Board BITUM Bituminous BL Building Line BLDG Building BLKG Blocking) BM Bench Mark Bm Beam BOT Bottom BRG Bearing BRKT Bracket BS Both Sides/Back Set BSC Bio -Safety Cabinet BSMT Basement BTW Between C Closer CAB Cabinet CARP Carpet CARPT Carpet Tile CB Catch Basin/Chalkboard CD Ceiling Diffuser/Cup Dispenser CER Ceramic CG Corner Guard CH Coat Hook CHAN Channel CHAM Chamfer CIP Cast -in -Place (concrete) CJ Construction Joint CK Change Key CL Clearance CLG Ceiling CU Control Joint CLKG Calking CLOS Closet CLR Clear CLS Closer CMF Composition Mastic Flooring CMS Countersunk Machine Screw CMT Ceramic Mosaic Tile CMU Concrete Masonry Units CO Clean out CO2FE Carbon Dioxide Fire Extinguisher COL Column COMB Combination/Combustible COMM Communication COMP Compressed (ion)/ Compact(or) Composite(ion) CMPT Computer CONC Concrete COND Conduit, Condition CONF Conference CONN Connect(ion) CONST Construction CONT Continue/Continuous COORD Coordinate CORR Corridor CR Cold Rolled/Chemical /Curtain Rod CS Cup Sink CSK Countersink/Countersunk CSP Combination Stand Pipe CSV Conductive Sheet Vinyl CT Ceramic Tile CTG Coating CTR Center cw Cold Water (Domestic) CwS Countersunk Wood Screw D Deep d Penny (Nails) DC Direct Current DBL Double DEG Degree(s) DEM Demolish/Demolition DEPT Department DET Detail DF Drinking Foundation/ Douglas Fir DI Deionized Water DIA Diameter DIAG Diagonal DIFFiffuser DIM imension(s) DIR Directors) DISC Disconnect DISP Dispenser DIV Division/Divider DL Dead Load DN Down DR Door/Drain DS Downspout DSP Dry Standpipep DT Drain Tile DW Dish Washer DWG Drawing(s) DWR Drawer EA EB EG EJ EL ELEC ELEV ELIM EMER EMP ENAM ENCL ENTR EQ EQJ EQUIP ES EST ETC ETR EXH EXIST EXP EXT EXTD EW EWC F F TO F FAC FB FBD FCU FD FDPR FDS FE FEC FF FH FHC FHEC FHMC FHR FHS FHWS FIC FIN FL FLASH FLEX FLG FLR FLUOR FND F.O.
