Tests shall be conducted in the presence of the Plumbing Inspector. Box 9706 Spokane, WA 99209 Tel: (509)-990-3379 RECEIVED CITY OF TUKWILA AUG 1 9 e 2013 PERMIT CENTER 16 August 2013 RE: Site Visit Report and Letter of Completion for the North Entrance Plumbing Line Extension for Francesca's Tenant Improvement, Westfield Shoppingtown Southcenter, Tukwila, Washington Dear Mr. Sharp, Harbor Consulting Engineers is pleased to present our site visit report and completion letter for the North Entrance Plumbing Line Extension Project serving the tenant space for Francesca's Store in the mall. A partial mall floor framing plan and structural slab penetration details were prepared by Harbor Consulting Engineers on 2 August 2013. At the request of the building official and in order to issue a letter of satisfactory completion for the work, a second site visit was performed to view the exposed access openings and the preparation of the existing structural slab prior to repair concrete placement. Digital record field photographs were taken by this office during the site visit in addition to field notes. The following conditions were noted during our second site visit: 1. Instead of three penetrations through the existing concrete structural slab only two penetrations were made; one in the mall concourse and one in the tenant space. 2. The penetration through the concrete structural slab in the mall concourse required cutting only one # 5 reinforcing bar each way in the opening. 3. The opening made through the existing concrete structural slab in the tenant space was completed without the need to cut any of the existing reinforcing steel. 4. Bar -Lock couplers were on-site at the time of our site visit, ready for use in the installation of the re -useable cut out reinforcing steel bar sections also on-site. 5. The size of the access opening in the mall concourse was smaller than allowed by Harbor Consulting Engineers detail "SK2". 6. The size of the penetration made in the existing concrete structural slab in the tenant space was appreciably smaller than permitted by Harbor Consulting Engineers detail "SK1". 7. The periphery of both access penetrations made through the concrete structural slab, while not tapered per the 3 Vertical to 1 Horizontal slope shown on Harbor Consulting Engineers details, did possess adequate roughness to prevent shear displacement of the repair pour section. 8. Three attached photographs furnished by the contractor show the formed bottom of concourse opening and the on-going installation of Bar -Lock Couplers. Based on these observations, it is our professional opinion that to the best of our knowledge and belief, the concrete structural slab access openings have been satisfactorily repaired.
