CHANGE This change order provides formal approval for work elements authorized by the Field Directive 5 (FD #5) as outlined below (copies of FD's attached): Description FD #5 added 6 additional loop detectors on Interurban at northbound 52 This directive provided for a $5,000 LS cost for a new item "Six additional loops" under Traffic Signals. The LS amount includes the installed loops, remobilization, traffic control and prime contractor mark -up and supervision. The technical requirements are as provided in the contract documents. This C.O. adds a 2 calendar day work window to perform the work during an 11 calendar day window thereby making November 10, 2011 as the contract completion date. Overlay Program Project No.: 91110401 Contract No.: 11 -105 WORKS Work to be performed.: You are requested to install 6 additional stop bar loops in the northbound lanes of Interurban Avenue at 52n as shown on the attached drawing. These new loops are not depicted in the project plans as the need for the loops was not realized until very recently. Recognizing the loop installation crew has left the project and will have to re- mobilize and that additional traffic control will be required, the work shall be performed at a negotiated cost that exceeds the contract unit cost for the loop installation. The negotiated cost for installing 6 additional loops including mobilization, traffic control and all other related cost shall be $5,000. This amount will be full payment for traffic loop installation that takes loop wire via existing stub outs to junction box, splice kit and testing as well as all related costs including re- mobilization, traffic control, supervision to provide a complete operational loop installation. Since this is a calendar day contract the additional work for Field Directive #4 result in a calendar extension that made the final contract day October 30, 2011. This Field Directive #5 will provide a separate 2 calendar day period to perform the work outlined in this directive to be performed based upon installation crew availability and acceptable weather conditions prior to 11- 11 -11. Contract close -out cannot be completed until this work is performed. Reason for modification: It was discovered in the field by the City signal technician while reviewing the other signal work that existing stop bar loops are not functional. The expanded work at unit prices at this site is not expected to increase total contract price as it accounts for underuns in quantities on other sites.
