This permit shall become null and void if the work is not commenced within 180 days for the date of issuance, or if
the work is suspended or abandoned for a period of 180 days from the last inspection.
PERMIT CONDITIONS:

2: Work shall be installed in accordance with the approved construction documents, and any changes made
during construction that are not in accordance with the approved construction documents shall be
resubmitted for approval.
3: All permits, inspection record card and approved construction documents shall be kept at the site of work
and shall be open to inspection by the Building Inspector until final inspection approval is granted.
4: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap
the sanitary sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks,
wells, and other excavations. Final inspection approval will be determined by the building inspector based
on satisfactory completion of this requirement.
5: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an
approval of, any violation of any of the provisions of the building code or of any other ordinances of the City
of Tukwila. Permits presuming to give authority to violate or cancel the provisions of the code or other
ordinances of the City of Tukwila shall not be valid. The issuance of a permit based on construction
documents and other data shall not prevent the Building Official from requiring the correction of errors in
the construction documents and other data.
6: Any material spilled onto any street shall be cleaned up immediately.
7: Temporary erosion control measures shall be implemented as the first order of business to prevent
sedimentation off -site or into existing drainage facilities.
8: The site shall have permanent erosion control measures in place as soon as possible after final grading has

On April 18, 2013, Todd P. Carter of Pacific Rim Environmental, Inc. (PacRim) performed limited lead -based
paint testing in the PacRim laboratory. The two (2) paint samples were collected and submitted by client. The
samples were tested using a Niton XRF device. Field inspection, data collection, and report generation were
performed according to the following Scope of Work:
1. XRF testing of suspect lead -based paints (LBP) using Niton XLp-303A portable XRF device.
2. Written descriptions of testing combinations and painted component locations
3. Prepare final written report including: Sample descriptions, condition, locations, analytical
results, and recommendations.
All requirements for the NITON XRF usage contained in the Performance Characteristics Sheet for the specific
XRF were followed.
Limited Lead -Based Paint Screening
Limited testing for lead -based paint was conducted on April 18, 2013. Testing was performed using a NITON X-
Ray Fluorescence Spectrometer (XRF) model XLp-303A, serial number 80662. Paints found to contain lead in a
concentration equal to or greater than the Federal threshold of 1.0 mg/cm2 of surface as measured by a XRF
are considered lead -based paints by EPA / HUD guidelines.
No lead -based paint (LBP) above the EPA/HUD threshold was identified in the samples submitted by client.
it is important to keep in mind that although the EPA/HUD standard uses a criterion of 5,000 parts per million
dry weight or 1.00 milligrams per square centimeter (1.00 mg/cm2) for lead -based paint, there still may be
lead present in those results reported as negative. In the event that lead is present, Federal OSHA and
Washington State Department of Labor Et Industries regulations wilt still apply, since neither agency has
established a concentration of lead in paint below which the lead in construction standards do not apply.
Workers wearing respiratory protection and who have received proper training in the handling of lead
contaminated materials must be used for any construction activities (including manual scraping, manual/power
sanding, heat gun applications, general cleanup, and demolition) that affect a paint film containing lead.

FIELD OPERATION GUIDANCE
OPERATING PARAMETERS:
Lead -in -Paint K+L variable reading time mode.
XRF CALIBRATION CHECK LIMITS:
0.8 to 1.2 mg/cm2 (inclusive)
The calibration of the XRF instrument should be checked using the paint film nearest 1.0 mg/cm2 in the NIST
Standard Reference Material (SRM) used (e.g., for NIST SRM 2579, use the 1.02 mglcn2 filfn).
If readings are outside the acceptable calibration check range, follow the manufacturer's instructions to bring
the instruments into control before XRF testing proceeds.
SUBSTRATE CORRECTION:
For XRF results using Lead -in -Paint K+L variable reading time mode, substrate correction is rgt needed for:

BACKGROUND INFORMATION
EVALUATION DATA SOURCE AND DATE:
This sheet is supplemental information to be used in conjunction with Chapter 7 of the HUD Guidelines for
the Evaluation and Control of Lead -Based Paint Hazards in Housing CHUD Guidelines"). Performance
parameters shown on this sheet are calculated from the EPA/HUD evaluation using archived building
components. Testing was conducted in August 2004 on 133 testing combinations. The instruments that
were used to perform the testing had new sources; one instruments was installed in November 2003 with
40 mCi initial strength, and the other's was installed June 2004 with 40 mCi initial strength.
OPERATING PARAMETERS:
Performance parameters shown in this sheet are applicable only when properly operating the instrument
using the manufacturer's instructions and procedures described in Chapter 7 of the HUD Guidelines.
SUBSTRATE CORRECTION VALUE COMPUTATION:
Substrate correction is not needed for brick, concrete, drywall, metal, plaster or wood when using Lead -in -
Paint K+L. variable reading time mode, the normal operating mode for these instruments. If substrate
correction is desired, refer to Chapter 7 of the HUD Guidelines for guidance on correcting XRF results for
substrate bias.
EVALUATING THE QUALITY OF XRF TESTING:
Randomly select ten testing combinations for retesting from each house or from two randomly selected
units in multifamily housing. Use the K+L variable time mode readings.
Conduct XRF retesting at the ten testing combinations selected for retesting.
Determine if the XRF testing in the units or house passed or failed the test by applying the steps below.
Compute the Retest Tolerance Limit by the following steps:
Determine XRF results for the original and retest XRF readings. Do not correct the
original or retest results for substrate bias. In single-family housing a result is defined as
the average of three readings. In multifamily housing, a result is a single reading.
Therefore, there will be ten original and ten retest XRF results for each house or for the
two selected units.
Calculate the average of the original XRF result and retest XRF result for each
testing combination.
Square the average for each testing combination.
Add the ten squared averages together. Call this quantity C.
Multiply the number C by 0.0072. Gall this quantity D.
Add the number 0.032 to D. Call this quantity E.
Take the square root of E. Call this quantity F.
Multiply F by 1.645. The result is the Retest Tolerance Limit
Compute the average of all ten original XRF results.
Compute the average of all ten re -test XRF results.
Find the absolute difference of the two averages.

If the difference is less than the Retest Tolerance Limit, the inspection has passed the retest. If
the difference of the overall averages equals or exceeds the Retest Tolerance Limit, this
procedure should be repeated with ten new testing combinations. If the difference of the overall
averages is equal to or greater than the Retest Tolerance Limit a second time, then the
inspection should be considered deficient.
Use of this procedure is estimated to produce a spurious result approximately 1% of the time. That is,
results of this procedure will call for further examination when no examination is warranted in
approximately 1 out of 100 dwelling units tested.
TESTING TIMES:
For the Lead -in -Paint K+L variable reading time mode, the instrument continues to read until it is moved
away from the testing surface, terminated by the user, or the instrument software indicates the reading is
complete. The following table provides testing time information for this testing mode. The times have
been adjusted for source decay, normalized to the initial source strengths as noted above. Source
strength and type of substrate will affect actual testing times. At the time of testing, the instruments had
source strengths of 26.6 and 36.6 mCi.

CLASSIFICATION RESULTS:
XRF results are classified as positive if they are greater than or equal to the threshold, and negative if
they are less than the threshold.
DOCUMENTATION:
A document titled Methodology for XRF Performance Characteristic Sheets provides an explanation of
the statistical methodology used to construct the data in the sheets, and provides empirical results from
using the recommended inconclusive ranges or thresholds for specific XRF instruments. For a copy of
this document call the National Lead Information Center Clearinghouse at 1-800-424-LEAD.
This XRF Performance Characteristic Sheet was developed by the Midwest Research Institute (MRI)
and QuanTech, Inc., under a contract between MRI and the XRF manufacturer. HUD has determined
that the information provided here is acceptable when used as guidance in conjunction with Chapter 7,

1. I have made application for a permit from the City of Tukwila, Washington.
2. I understand that state law requires that all building construction contractors be registered with the
State of Washington. The exceptions to this requirement are stated under Section 18.27.090 of the
Revised Code of Washington, a copy of which is printed on the reverse side of this Affidavit. I have
read or am familiar with RCW 18.27.090.
3. I understand that prior to issuance of a permit for work which is to be done by any contractor, the
City of Tukwila must verify either that the contractor is registered by the State of Washington, or that
one of the exemptions stated under RCW 18.27.090 applies.
4. In order to provide verification to the City of Tukwila of my compliance with this requirement, I
hereby attest that after reading the exemptions from the registration requirement of RCW 18.27.090,
I consider the work authorized under this permit to be exempt under number , and will therefore
not be performed by a registered contractor.
5. I understand that the licensing provision of RCW 19.28.161 through 19.28.271 shall not apply to
persons making electrical installations on their own property or to regularly employed employees
working on the premises of their employer. The proposed electrical work is not for the construction
of a new building for rent, sale or lease.
