Subject: Sign Permits #S11-110 and S11-111 for Puget Sound Pain Center
Dear Sign Permit Applicant,
On December 2, 2011, I e-mailed you to inform you that your sign permits had been approved and were available for
you to pick up at our office. To date, no one has picked up the permits. The permits will not officially be issued until the
permits are picked up. As the work on these signs was originally performed without a permit, you may be subject to a
code enforcement action unless these permits are issued and final approval is granted for the signs after a site visit.
If you are unable to pick up the permits and would prefer to have them mailed to you, please e-mail me back or call me
at the number below and let me know. Otherwise, please come to our office at your earliest convenience to pick up the
permits. They are available to be issued over the counter at our the City of Tukwila permit center, 6300 Southcenter

Thank you for your cooperation, and please let me know if you have any further questions or concerns regarding these

Dear Sign Permit Applicant,
The City of Tukwila Department of Community Development has completed our review of your sign permit
applications. This will serve as an official notice of decision.
Your applications have been approved and are available for you to pick up at our office, 6300 Southcenter Blvd, Ste
100.
The permits are valid for 180 days from the date of issuance. You are required to request a final inspection prior to the
permit expiration date or the permits will expire. To request an inspection, please call our inspection line at 206-431-
2451. You may be granted one extension of not more than 30 days if you submit a request for extension prior to permit
expiration.
Additional Information:
Per our telephone conversation of November 30, 2011, you have confirmed that the property owner intends to remove
the sign on the south face of the building reading "Lotto-Shellan Building." As you are merely replacing or refacing
existing signs and no additional new signage is being proposed, the existence of the "Lotto-Shellan Building" sign will not
affect your permits in any way. The property owner will be held responsible for removing or repairing the "Lotto-Shellan
building" sign.
If you prefer to have this permit mailed to you, or if you have any additional questions, please call 206-431-2164 and
reference the permit number listed above.
