All correction requests from each department must be addressed at the same time and reflected on your drawings. I have enclosed comments from the following departments: PW DEPARTMENT: Joanna Spencer at 206-431-2440 if you have questions regarding these comments. The City has determined that the subject building, which has two addresses: 350 and 360 Midland Dr. has deficiencies on the domestic water and fire prevention lines. a) Domestic Water- there are (4) four water services for this property. 1) One is for landscape irrigation that has a Double Check Valve Assembly (DCVA) installed that is overdue for required annual testing. The last test was done in August 2013. 2) One of the domestic services listed for 360 Midland Dr. has an (RPPA) Reduced Pressure Principle Assembly installed for premise isolation that is overdue for testing. Last test was done in August 2013. 3) The domestic service listed for 350 Midland Dr. requires installation of a Reduced Pressure Principle Assembly. It shall be installed immediately downstream od the existing 1 " WM. The RPPA shall be installed in a freeze protection enclosure anchored into a minimum 4" thick concrete pad. 4) The last service has been off since 1995. b) Fire Protection Service the building's 8" fire line connection has a Detector Double Check Valve Assembly (DDCVA) installed that is overdue for the required annual testing. This backflow was last tested in August 2013. Please have the above backflows tested ASAP by a certified tester and submit passing test reports. If you need assistance locating these backflows in the field please contact Mr. Todd Reedy, WQ Specialist, Tukwila Water Department at 206 433-1860. The Public Works Director will withhold issuance of the Stelfast Tenant Improvement D15-0072 until the backflows issue is resolved. On April 2, 2015 a separate letter addressing these backflow deficiencies was mailed to Prologis, Inc. representing the property owner. Please address the comments above in an itemized format with applicable revised plans, specifications, and/or other documentation. The City requires that four (4) sets of revised plan pages, specifications and/or other documentation be resubmitted with the appropriate revision block. Xsnn c,,,,th ...f— c,,;*, .,Y7nn . Since the project included alterations to the existing plumbing system, the entire plumbing system must be brought up to the current standards as set forth in the Uniform Plumbing Code including installation of an approved backflow prevention on the domestic, fire prevention and irrigation water supply lines to the building. The program requires elimination or control of any cross -connection between the distribution system and a consumer's water system by the installation of an approved backflow device. The City has determined that the subject building, which has two (2) addresses: 350 and 360 Midland Drive, has deficiencies on the domestic water and fire prevention lines. a) Domestic Water There are a total of (4) four 1" water services for this property. 1) One of the services is for landscape irrigation that has a Double Check Valve Assembly (DCVA) installed that is overdue for required annual testing. The last test was done in August 2013. 2) One of the domestic services listed for 360 Midland Drive has an (RPPA) Reduced Pressure Principle assembly installed for premise isolation that is overdue for testing. The last test was done in August 2013. 3) The domestic service listed for 350 Midland Drive requires installation of a Reduced Pressure Principle Assembly (RPPA), previously called a Reduced Pressure Backflow Assembly (RPBA). It shall be installed immediately downstream of the existing 1" permanent water meter. The RPPA shall be installed in a freeze protection enclosure anchored into a minimum 4" thick concrete pad. Mike Cusick, P.E., Public Works Dept. D15-0072 File (W:PW EnL/Other/Joanna Spencer/Letter Stelfast 033115) Joanna Spencer From: Todd Reedy Sent: Tuesday, March 24, 2015 10:57 AM To: Joanna Spencer Subject: RE: Stelfast,Inc TI @ 350 Midland DR Hello Joanna, Here is the situation at 350 Midland Dr: The addresses of 350 Midland and 360 Midland are one building. There are a total of (4) 1" water services for this property. One of the services is for irrigation that has a DCVA installed for backflow prevention that is overdue for testing. The test due month is August and we did not receive a test report in 2014. The last test was 8/2013. One of the domestic services that is listed for 360 Midland has an RPPA installed for premise isolation that is overdue for testing. Same test schedule as the irrigation DCVA. The domestic service that is listed for 350 Midland does not have an RPPA installed for premise isolation. The last service has been off since 1995. The building's 8" fireline connection has a DCDA installed that is overdue for testing. Same as above (last test 8/2013). The only other issue I saw was that there is a private hydrant at the southwest corner of the building that needs a Storz adapter installed on the pumper port. Let me know if you need any additional info, Todd From: Joanna Spencer Sent: Monday, March 23, 2015 4:40 PM To: Todd Reedy Cc: Han Kirkland Subject: Stelfast,Inc TI @ 350 Midland DR Good Afternoon Todd, Are their backflows up to date?
