This permit shall become null and void if the work is not commenced within 180 days for the date of issuance, or if
the work is suspended or abandoned for a period of 180 days from the last inspection.
PERMIT CONDITIONS:
1: 'BUILDING PERMIT CONDITIONS'
2: Work shall be installed in accordance with the approved construction documents, and any changes made
during construction that are not in accordance with the approved construction documents shall be
resubmitted for approval.
3: All permits, inspection record card and approved construction documents shall be kept at the site of work
and shall be open to inspection by the Building Inspector until final inspection approval is granted.
4: All construction shall be done in conformance with the Washington State Building Code and the
Washington State Energy Code.
5: Remove all demolition rubble and loose miscellaneous material from lot or parcel of ground, properly cap
the sanitary sewer connections, and properly fill or otherwise protect all basements, cellars, septic tanks,
wells, and other excavations. Final inspection approval will be determined by the building inspector based
on satisfactory completion of this requirement.
6: All plumbing and gas piping work shall be inspected and approved'under a separate permit issued by the
City of Tukwila Building Department (206-431-3670).
7: All electrical work shall be inspected and approved under a separate permit issued by the City of Tukwila
Permit Center.
8: VALIDITY OF PERMIT: The issuance or granting of a permit shall not be construed to be a permit for, or an
approval of, any violation of any of the provisions of the building code or of any other ordinances of the City
of Tukwila. Permits presuming to give authority to violate or cancel the provisions of the code or other
ordinances of the City of Tukwila shall not be valid. The issuance of a permit based on construction
documents and other data shall not prevent the Building Official from requiring the correction of errors in
the construction documents and other data.

Value of Construction — In all cases, a value of construction amount should be entered by the applicant. This figure will be reviewed and is subject
to possible revision by the Permit Center to comply with current fee schedules.
Expiration of Plan Review — Applications for which no permit is issued within 180 days following the date of application shall expire by limitation.
The Building Official may grant one or more extensions of time for additional periods not exceeding 90 days each. The extension shall be
requested in writing and justifiable cause demonstrated. Section 105.3.2 International Building Code (current edition).

Washington Weather Shield Inc. is pleased to provide the following proposal to provide labor & materials for the following:
■ Remove and dispose all existing roofing from upper and lower roof levels
■ Install a fire rated underlayment
• Install .60 mil white GAF TPO
■ All seams will be welded and all penetrations will be flashed and detailed to manufacturer's specifications
• Guard rails on lower roof level will be removed, sand blasted or wire brushed as needed, painted and reinstalled
■ New metal wall coping will be installed over existing coping on all parapet walls
■ All debris will be cleaned up and disposed
Proposal includes 5 year labor warranty and 20 year material warranty RECEIVED
Total job cost is $66,466.00 + WA State tax CITY OF TUKWILA
Change order: Install 3.5° roofing insulation on lower level, 1.5" on upper level to reach R38 insulation value. 1

SALES TAX, any and all municipal, engineering, or architectural PERMITS and FEES will be added to above total price.
PAYMENT TERMS: 50% down, progress payments upon request, and balance upon completion,
IMPORTANT: All proposals do not include any Asbestos testing or any unforeseen issues, that may arise after project begins that were not previously detected. Client will be notified of any
extra costs that may apply. Upon acceptance of this proposal either by signature, email or by generating a purchase order, client agrees to the terms of this contract and that it supersedes any
other oontract(s) between client and W WS. W WS reserves the right to take photographs and/or video, before and after project, and retain them on file. Client will not soIicItWWS employees or
subcontractors to work directly for them. Such action will void can void contract and all monies due for completion of project will be due immediately.
Upon acceptance of this proposal either by signature, email or by generating a purchase order, client agrees to the terms of this contract and that it supersedes any other contract(s) between
client and WAS. W✓VS reserves the tight to take photographs and/or video, before and after project, and retain them on file. Client will not solicit WWS employees or subcontractors to work
directly for them. Such action will void can void contract and all monies due for completion of project will be due immediately.
PROPOSAL: This document is considered a contract between client and WWS. All labor & materials to be as specified in this document or equal as necessary for each project. Any alteration,
deviation or unforeseen conditions from the specifications (in writing, verbal, or in electronic form) involving extra costs, both parties will attempt to execute change orders in writing. In the event
work is requested and no change order is executed in writing, W WS work will be considered an additional charge over and is an additional charge above and beyond this contract. All plans and
specifications prepared by the Owner or his agent are to be considered part of this contract. All agreements are contingent upon strikes, accidents, or delays beyond our control. Owner is to be
responsible for all causality insurance for the premises. Our company's staff is fully covered by Workman's Compensation and Liability Insurance. This contract may be withdrawn if not
accepted within 30 days of the date above. However, if client does not sign this contract, but work commences on the site from verbal approval from any representative of client, that mutual
acceptance is considered an agreement by both parties of this contract's scope, price and of the terms and conditions in this document. No coupons may be used in conjunction with this
proposal after acceptance. Coupons or discounts may only be applied prior to acceptance of this contract and only one coupon/discount may be applied per contract.
WARRANTIES: WWS reserves the right to be given first opportunity to correct any warranty or defects on project. Customer accepts all work as -is and waives all further claims against
contractor if certain conditions are not met: such as not providing a punch list, performing premature punch list work, or using the contractor's work in any way prior to completion. Labor is
warranted on labor only for 1 year unless otherwise specified. ACNW is not responsible for manufacturer defects on materials provided on projects. Client will be charged time and material if
product defects are found after job is completed. Payment must be received in full to exercise such warranty. If job is not paid in full or any payment dispute is in progress, no warranty will be
extended until resolution is agreed upon. W WS attempts to avoid these defects to the best of their abilities; however, as time goes on, it is impossible to warrant the various product(s). Client
should refer to the manufacturers' warranty. Any non -preventable or normal wear and tear such as: Cracks of any kind, caulk, drywall settling, paint, repeat drain cleaning, wood cracking, roof
patching, etc. are not covered under any warranty by WWS.
LIEN NOTICE: As required by Washington State law, this is a legal notice that Washington Weather Shield Inc. is supptying labor and/or materials to the owner at the address listed above. A
notice to customer (RCW 18.27.114) is provided with this contract. Washington Weather Shield Inc. can and will lien said property in the event of non-payment of this contract. This is only a
notice to satisfy the laws as they are written and is not meant to Imply that such a lien will occur.
ATTORNEY FEES: If this agreement is submitted to court for collection the prevailing party shall be awarded reasonable attorney fees. Any disputes or arbitration will be held in Pierce County
Courts and the client will be responsible for their own expenses.

NOTICE TO CUSTOMER
(RCW 18.27.114)
Washington Weather Shield Inc. is registered with the State of Washington, registration number
WASHIWS887DG, and has posted with the state bonds or deposits $12,000 (Construction) for
the purpose of satisfying claims against the contractor for breach of contract including negligent
or improper work in the conduct of the contractor's business. The expiration date of this
contractor's registration is 03/07/2016.
THIS BOND OR DEPOSIT MIGHT NOT BE SUFFICIENT TO COVER A CLAIM THAT MIGHT
ARISE FROM THE WORK DONE UNDER YOUR CONTRACT.
This bond or deposit is not for your exclusive use, because it covers all work performed by this
contractor. The bond or deposit is intended to pay valid claims up to $12,000 (Construction)
that you and other customers, suppliers, subcontractors, or taxing authorities may have.
FOR GREATER PROTECTION YOU MAY WITHHOLD A PERCENTAGE OF YOUR
CONTRACT.
You may withhold a contractually defined percentage of your construction contract as
retain age for a stated period of time to provide protection to you and help insure that
your project will be completed as required by your contract.
YOUR PROPERTY MAY BE LIENED.
If a supplier of materials used in your construction project or an employee or
subcontractor of your contractor or subcontractors is not paid, your property may be
placed lien on to force payment and you could pay twice for the same work.
FOR ADDITIONAL PROTECTION, YOU MAY REQUEST THE CONTRACTOR TO PROVIDE
YOU WITH ORIGINAL "LIEN RELEASE" DOCUMENTS FROM EACH SUPPLIER OR
SUBCONTRACTOR ON YOUR PROJECT.
The contractor is required to provide you with further information about lien release documents if
you request it. General information is also available from the state Department of Labor and
Industries.
